Objective:This study investigated the turnover intention,perceived social support,psychological resilience,stress overload and fatigue status of junior operating room nurses in some secondary and higher hospitals in nine urban areas of Fujian Province,meanwhile investigated the influence of demographic characteristics,perceived social support,psychological resilience,stress overload and fatigue on the turnover intention of junior operating room nurses(leaving operating room tendency,leaving hospital tendency and leaving nursing profession tendency),and constructed the intermediary effect model of turnover intention through structural equation model,and formulated effective intervention strategies for hospital managers and nursing managers,to improve the current situation of the loss of junior nurses and to stabilize the nursing staff in the operating room.Methods:A descriptive multi-center cross-sectional survey design was conducted via purposive sampling of 439 junior nurses in operating room in 76 secondary and higher hospitals in nine urban areas of Fujian Province;and administering a demographic questionnaire for individual and work-related variables,the Chinese version of the psychological resilience scale(CD-RISC),perceived social support scale(PSSS),stress overload scale(SOS),fatigue scale-14(FS-14)and turnover intention scale(TIS).Using SPSS 26.0 statistical analysis software to analyze the survey results,using descriptive analysis,independent sample t test,single factor ANOVA analysis of junior operating room nurses’ perceived social support,psychological resilience,stress overload,fatigue,turnover intention and influencing factors;using Spearman correlation analysis to analyze the correlation between perceived social support,psychological resilience,stress overload,fatigue and turnover intention of junior operating room nurses;using binary logistic regression analysis to screen the main variables;A structural equation model is constructed using AMOS 26.0 to explore the mediating effect between variables.Results:1.Descriptive analysis(1)The total score of turnover intention was(13.03±6.14),leaving nursing profession(2.16±1.01),leaving organization(7.12±3.56),leaving operating room(3.74±1.97)in each dimension.(2)The total score of perceived social support level was(65.04±13.89),including family support(21.84±4.92),friend support(21.60±4.81)and other support(21.60±4.73),respectively.(3)The total score of psychological resilience was(66.16±0.87),the scores of each dimension were tenacity(33.31±0.47),strength(22.46±0.29),optimism(10.39±0.15).(4)The total score of stress overload was(59.64±17.07),each dimension was divided into event load(29.27±8.10)and individual vulnerability(25.34±8.26).(5)The total score of fatigue level was(6.17±3.44),including physiological fatigue(4.02±2.76)and psychological fatigue(2.15±1.4)respectively.2.Influencing factors analysis(1)A single-factor analysis showed that there was a statistically significant influence of chronic diseases on the tendency to leave nursing specialty(P<0.05).(2)Effects of monthly income,family support to work,personal views on operating room work on professional tendency to leave nursing,organizational tendency and tendency to leave operating room were statistically significant(P<0.05).3.Correlation analysis(1)The turnover intention of junior operating room nurses and their dimensions were negatively correlated with psychological resilience and perceived social support(r=|-0.140~-0.297| and r=|-0.174~-0.246|,P<0.01).(2)The turnover intention of junior operating room nurses and their dimensions were positively correlated with stress overload and fatigue(r=|0.309~0.509|and r=|0.191~0.385|,P<0.01).4.Binary logistic regression analysis(1)The variables included in the regression equation of leaving profession and organization tendency were: the OR value of independent child support group in the child support group was 1.683(95% CI 1.042~2.718),the OR value of strength in mental resilience was 0.939(95% CI 0.901~0.978),the OR value of working stress overload was 1.115(95% CI 1.069~1.162),the value of event load was 0.887(95%CI0.815~0.966)and the OR value of physical fatigue was 1.122(95% CI1.020~1.233).(2)The variables included in the regression equation of leaving operating room tendency were :the OR value of tertiary hospitals,special hospital,living with family,living with friends,monthly income less than 3000 yuan,between 3000 ~ 3999 yuan and 4000~4999 yuan are 2.348(95%CI 1.178~4.678)、0.414(95%CI 0.183~0.94)、2.313(95%CI 1.045~5.119)、3.414(95%CI 1.411~8.259)、4.215(95%CI 1.748~10.162)、4.099(95%CI1.985~8.464)and 2.48(95%CI 1.212~5.075)respectively.The strength OR in resilience was 0.847(95% CI 0.763~0.942),the tenacity OR was 1.078(95% CI1.009~1.152),the stress overload OR was 1.155(95% CI 1.098~1.216),the event load OR was 0.83(95%CI 0.747~0.921),and the age was 1.093(95% CI 1.008~1.185).5.Model Analysis of Intermediary Effect(1)Perceived social support has a negative correlation with stress overload,fatigue and turnover intention;Perceived social support has a positive correlation with psychological resilience,has a direct effect on psychological resilience,and psychological resilience has a direct effect on fatigue.(2)Stress overload has a positive correlation effect on fatigue and turnover intention,while stress overload has a direct effect on fatigue,but stress overload can not directly adjust turnover intention,fatigue has a partial mediating effect between stress overload and turnover intention.Therefore,stress overload can affect turnover intention through fatigue.(3)Psychological resilience has a negative correlation effect on fatigue and turnover intention,psychological resilience has a direct effect on fatigue,fatigue has a direct effect on turnover intention,so psychological resilience can affect turnover intention through fatigue as an intermediary variable.Conclusion:1.The junior nurses in operating room of Fujian province mainly take part in shift work and often work overtime.The proportion of nursing human resources in operating room is insufficient,so attention should be paid to it.2.The turnover intention is related to chronic diseases,monthly income,family support for work,personal views on the work of the operating room,and should be paid attention to and supported in relevant aspects.3.Perceived social support is negatively correlated with stress overload,fatigue and turnover intention;Perceived social support has a positive correlation and direct effect on psychological resilience;psychological resilience has a direct effect on fatigue and has an intermediary effect between perceived social support and fatigue,perceived social support can also affect turnover intention through psychological resilience and fatigue as intermediary variables,stress overload can not directly regulate turnover intention,so managers should adopt effective forms to enhance nurses’ psychological resilience,give more care and help in life and work,improve their social support level,and reduce nurses’ working pressure and fatigue through reasonable and flexible scheduling.
