ObjectiveThe purpose of this study is to describe the status of Intensive Care Unit(ICU)nurses' organizational silence?job satisfaction and their turnover intention;To analysis the influencing factors of these factors and the correlation between them;To provide evidence for the measures to improve the job satisfaction of ICU nurses,reduce the organizational silence and turnover intention of ICU nurses,and provide reference for nursing administrators to stabilize ICU nursing team.MethodsTotally 232 ICU nurses from three tertiary hospitals in QingDao were investigated by questionnaires.The questionnaires included:(1)General information questionaire(2)Organizational Silence Behavior Scale(3)Mc Closkey/Mueller Job Satisfaction Scale(MMSS)(4)Turnover Intention Scale.The data were input by SPSS19.0 software,and the population status,organizational silence,job satisfaction and turnover intention of ICU nurses were statistically analyzed by mean,standard deviation,composition ratio,median,quartile spacing and so on.The effects of demographic variables on the above three aspects were analyzed by Independent t Test,variance analysis,rank sum test,and the scores of ICU nurses were compared.Spearman correlation analysis,multiple stepwise regression analysis,Structural Equation Model were used to analyze the relationship between them.ResultsThe organizational silence score of ICU nurses was(2.83 ± 0.49)points,the score of acquiescence silence was(3.35±0.75),the score of defensive silence was 2.75(2.50,3.25),the score of ignore silence was 2.25(2.00,2.69).The job satisfaction score was 3.00(2.80,3.23),and the overall job satisfaction score was 3.00(3.00,3.00).The scores of each dimension from the highest to lowest were the relationship between colleague was 4.00(3.50,4.00),commended and recognized was 3.25(3.25,3.75),interaction opportunity was 3.25(3.00,3.50),control and responsibility was3.00(2.80,3.20),work and family balance was 3.00(2.67,3.25),career opportunity was2.75(2.50,3.00),scheduling was 2.80(2.40,3.20),welfare was 2.67(2.00,3.00).The score of turnover intention of ICU nurses was 2.33(1.67,2.67),turnover intention 1 was3.00(2.00,3.00),turnover intention 2 was 2.00(2.00,2.75)points,and turnover intention3 was 2.00(1.00,3.00).Organizational silence was significantly negatively correlated with job satisfaction(r=-0.36,P<0.01)and positively correlated with turnover intention(r=0.30,P <0.01).In the multiple regression analysis of the ICU nurses' turnover intention,acquiescence silence and ignore silence enter into the regression equation,totally explained 8% of the turnover intention.In all dimensions of job satisfaction,control and responsibility,commended and recognize?scheduling?welfare?work and family balance enter into the regression equation,totally explained 24% of the turnover intention.From Structural Equation Model,the direct effect of organizational silence on job satisfaction is-0.42,and the direct effect on turnover intention is 0.24.Job satisfaction was a partial mediator effect on organizational silence and turnover intention.When job satisfaction act as a madiator effect,the indirect effect of ICU nurses' silence on turnover intention was 0.16,accounting for 40.0% of the total effect.Conclusion1.ICU nurses' organizational silence was at a moderate level.Job satisfaction was at a moderate low level.Turnover intention was at a high level.Organizational silence and job satisfaction had significant differences in age,working years,and average monthly income.Turnover intention had significant difference in working years.2.ICU nurses' organizational silence,job satisfaction,turnover intention were significantly correlated.3.Organizational silence,job satisfaction have a direct predictive effect on turnover intention.4.Organizational silence can directly affect the turnover intention,but also indirectly affect the turnover intention through the intermediary effect of job satisfaction.Job satisfaction played a mediating role between organizational silence and turnover intention.Leaders of ICU nurses should pay attention to the impact of organizational silence?job satisfaction on their turnover intention.Taking appropriate measures according to thecharacteristics of different nurses to minimize or reduce the nurse' organizational silence,in order to improve job satisfaction?reduce the turnover intention to stabilize the ICU care team.
