| Objective:In this study,the postoperative wounds of patients with horseshoe-shaped perianal abscess after the treatment of low postoperative horseshoe-shaped perianal abscess were treated with Kangfuxin liquid flushing and sterile gauze dipped in Kangfuxin liquid.The wound healing effect of patients before and after treatment was observed and the levels of related inflammatory cytokines were detected,Th1/Th2 balance index and other indicators,so as to further explore the clinical efficacy and related mechanisms of Kangfuxin liquid in promoting wound healing after the operation of posterior horseshoe-shaped perianal abscess.Methods:Sixty patients with posterior horseshoe-shaped perianal abscess who were hospitalized in the Anorectal Department of Anhui Provincial Hospital of Traditional Chinese Medicine from November 2019 to January 2021 were enrolled.They were randomly divided into a control group of 30 cases and a Kangfuxin liquid group of 30 cases.The two groups of patients underwent surgery.The dressing was changed 1 day later.Before dressing,the patients in both groups were bathed and washed with hemorrhoid lotion for10 minutes,and the dressing was changed twice a day until the wound was completely healed.In the control group,the wound was thoroughly washed with normal saline and drained with petroleum jelly.The new liquid group was thoroughly washed with Kangfuxin liquid and then used sterile gauze dipped in Kangfuxin liquid to compare the clinical efficacy of 1 month after surgery.Wound healing quality scores were scored on3 d,7d,14 d,and 21 d after surgery.The levels of inflammatory cytokines and Th1/Th2 balance index at 1d and 7d after operation.Result:(1)One month after the operation,the total effective rate of Kangfuxinye group(93.33%) was significantly higher than that of the control group(73.33%)(P<0.05).(2)The results of the comparison between the groups indicated that the wound healing quality scores of the Kangfuxinye group were significantly lower than those of the control group at 7d,14 d and 21 d after operation(all P<0.05).There was no significant difference in the wound healing quality scores of the patients in the Kangfuxinye group compared with the control group at 3 days after operation(all P>0.05).(3)The results of intra-group comparison of wound VAS scores indicated that the wound VAS scores of patients in the control group decreased significantly at 3d,7d,14 d and 21 d after operation,and the differences within the group were significant(all P<0.05).On the 3rd,7th,and 14 th day after operation,the wound VAS scores of the Kangfuxinye group were significantly decreased(P<0.05).Compared with the 14 d after the operation,the wound VAS scores of the Kangfuxinye group were not significantly different(P>0.05).(4)The results of intra-group comparison of secretion scores showed that the secretion scores of patients in the control group were reduced,but the difference was not significant(P> 0.05);The secretion scores of patients in the control group were significantly reduced on the 14 th and 21 st days after the operation compared with the3 rd and 7th days after the operation(all P<0.05).At 3d,7d,and 14 d after operation,the wound secretion scores of the Kangfuxinye group decreased significantly(all P<0.05);21 days after the operation,the wound secretion scores of the Kangfuxinye group were not different from those on the 14 th day after the operation.Significant(P>0.05).(5)The results of intra-group comparison of granulation tissue scores showed that the comparison of granulation tissue scores of patients in the control group was not statistically significant(P>0.05)compared with 3 days after surgery and 21 days after surgery compared with 14 days after surgery(P>0.05);The granulation tissue scores of patients in the control group decreased significantly after 14 days and 21 days compared with 3 days and 7 days after operation(all P<0.05);The granulation tissue scores of the patients in the Kangfuxinye group were significantly lower on the 7th day after the operation and the 3rd day after the operation(P<0.05);14 days after the operation compared with the 7d after the operation and 21 days after the operation compared with the 14 d after the operation,the Kangfuxin liquid group There was no significant difference in granulation tissue scores(all P>0.05),but the scores of granulation tissue in the Kangfuxinye group were significantly lower in the 14 days after the operation compared with the 3 days after the operation,and the 21 days after the operation compared with the 3 and 7 days after the operation(P<0.05).(6)At 1d before operation,1d after operation,and 7d after operation,the levels of pro-inflammatory cytokines(IFN-γ,IL-12)in the two groups showed a trend of first increasing and then decreasing,while anti-inflammatory cytokines(IL-4,IL-10)levels show a trend of first decreasing and then increasing.There were no significant differences in the levels of pro-inflammatory and anti-inflammatory cytokines between the two groups at 1 day after operation and 1 day before operation(P>0.05);compared with 1 day before operation,the two groups had pro-inflammatory cytokine levels.The level of cytokine in the group was significantly increased(P<0.05),and the levels of anti-inflammatory cytokines in the two groups were significantly lower than that in the group(P<0.05);7 days after operation was compared with 1 day before and after operation.Compared with 1 day,the levels of pro-inflammatory cytokines in the two groups were significantly reduced between the groups(P<0.05),and the Kangfuxinye group decreased more significantly(P<0.05).The anti-inflammatory cells of the two groups of patients The factor level increased significantly in the group(all P<0.05),and the increase in the Kangfuxinye group was more significant(all P<0.05).(7)At 1d before operation,1d after operation and 7d after operation,the Th1/Th2 balance index(IL-4/IFN-γ)of the two groups of patients showed a trend of first decreasing and then increasing.There was no significant difference in Th1/Th2 balance index between the two groups at 1 day after operation and 1 day before operation(P>0.05);1 day after operation compared with 1 day before operation,the Th1/Th2 balance index of the two groups was within the group Comparisons were significantly lower(P<0.05);7 days after operation,compared with 1d before operation and 1d after operation,the Th1/Th2 balance index of the two groups increased significantly(P<0.05);and recovery The increase in the new liquid group was more significant(P<0.05).Conclusion:In the treatment of promoting postoperative wound healing of posterior horseshoe-shaped perianal abscess,the use of Kangfuxin liquid to flush the wound combined with gauze to fill the wound improves the total effective rate of treatment,relieves the pain of the wound,and reduces the production of secretions.It promotes the formation of fresh granulation tissue on the wound,and regulates the level of postoperative inflammatory cytokines to reduce the body’s inflammatory response.At the same time,it helps the body’s immune response to stabilize by maintaining the body’s Th1/Th2 cell immune balance.After treatment,the horseshoe-shaped anus The curative
