| Objective:To explore the statistical relationship between the level of HbA1c and blood lipid and abnormal semen in patients with T2DM.Methods:This study included 152 patients with T2DM,including 111patients with abnormal semen quality group,41 patients with normal semen quality group,50 patients with control group.Pearson linear correlation and Spearman correlation were used to analyze the relationship between HBA1C,blood lipid level and semen quality.Patients with T2DM semen quality abnormality were divided into groups according to sperm count or concentration,sperm motility and sperm morphology,to analyse the influence of HbA1c and blood lipid level on semen quality of T2DM by binary Logistic regression analysis.Results:1.The levels of HbA1c and TG in T2DM group were significantly higher than that in healthy group,HDL-C was significantly lower than that in healthy group,and the levels of HbA1c and TG in T2DM group were higher than that in T2DM group,HDL-C level was significantly lower in T2DM group than that in T2DM group(P<0.05).2.The results of Pearson linear correlation or Spearman correlation analysis showed that HbA1c level was negatively correlated with sperm count,semen concentration,NP+PR,PR and normal sperm morphological rate(r=-0.279,-0.312,-0.515,-0.377,-0.482;P<0.05),HDL-C was positively correlated with these semen parameters(r=0.202,0.326,0.560,0.220,0.260;P<0.05),TG and Hcy were negatively correlated with NP+PR and sperm normal morphology(r=-0.202,-0.152;-0.203,-0.191;P<0.05),APOA was positively correlated with total sperm count,semen concentration and PR(r=0.226,0.199,0.155;P<0.05).3.(1)The patients with T2DM with abnormal sperm quality were divided into oligospermia group and non-oligospermia group according to the standard of total sperm number<39×10~6/1 ejaculation or sperm concentration<15×10~6/ml,the results of binary Logistic regression analysis showed that the increase of HbA1c level had statistical significance(OR=1.824;P<0.05);(2)PR<32%or PR+NP<40%,the patients with abnormal sperm quality of T2DM were divided into asthenospermia group and non-asthenospermia group.The results of binary Logistic regression analysis showed that the risk of asthenospermia increased with the increase of HbA1c level and the decrease of HDL-C(OR=3.811,0.169;P<0.05).(3)The patients with abnormal sperm quality in T2DM were divided into two groups:teratospermia group and non-teratospermia group,the results of binary Logistic regression analysis showed that the increase of HBA1C and the decrease of HDL-C in T2DM patients increased the risk of teratospermia(OR=3.688,0.174;P<0.05).Conclusions:(1)The results showed that the high level of HbA1c in T2DM patients increased the risk of oligospermia,asthenospermia and teratospermia.(2)The increase of TG level in patients with T2DM may lead to the decrease of sperm motility.At the same time,the decrease of sperm quality in T2DM is related to the decrease of HDL-C level.The lower HDL-C leads to the higher the probability of asthenospermia and teratospermia in T2DM. |
