| ObjectiveWe will monitor the initial Procalcitonin level and Procalcitonin clearance and examine the clinical value study of the initial level and Procalcitonin clearance in sepsis with acute kidney injury.MethodsIn the emergency intensive care unit,159 patients with sepsis were collected,the PCT level at admission(PCTD1)and the 72 h PCT level(PCTD3)were collected,and the Procalcitonin clearance(Δ PCT)was calculated.The sepsis group is divided into AKI group and non-AKI group according to the diagnostic criteria by the global kidney disease(improving global outcomes,KDIGO),and the AKI group is further divided into AKI1,AKI2,and AKI3 according to the staging criteria for acute kidney injury.The AKI group was divided into an elderly group and a non-elderly group according to the age.Furthermore,according to the death in AKI group is divided into survival group And death group.ROC was used in prognosis of the value of PCT and Δ PCT and in patients with sepsis with acute kidney injury.ResultsThe PCT level in the AKI group was significantly higher than that in the non-AKI group,and the difference was statistically significant(P<0.05);the PCT level and Δ PCT had nothing to do with the severity of acute kidney injury(P>0.05);There is no significant correlation with age(P>0.05);PCTD1 and PCTD3 have certain predictive value for the occurrence of sepsis AKI,the AUC is 0.642 and 0.745 respectively;both are smaller than SCr(0.931)and APACHE Ⅱ(0.772)And SOAF(0.746).△ PCT has a certain early evaluation value for the survival and prognosis of patients with sepsis.The area under the AUC curve is 0.644,the sensitivity is 0.467,and the specificity is 0.814Conclusions1.The PCT level in patients with sepsis and AKI are significantly higher than those in non-AKI patients,and the PCT level may be correlated with AKI but are clearly more independent of age and severity of renal dysfunction.2.PCTDi and PCTD3 area evaluation indicators for the sepsis AKI,but the values are lower than the SCr,APACHE Ⅱ,and sofa scores,and the evaluation value of SCr is the highest.3.Procalcitonin clearance can be used as a prognostic indicator for the survival of patients with sepsis and AKI. |
