Objective: Numerous studies have shown that lumbar disc herniation is closely related to facet joint degeneration,but the relationship between the two has not been widely recognized.Therefore,to explore the correlation between lumbar disc herniation and degeneration of lumbar disc and facet joint through imaging analysis can provide certain reference value for the relationship between the three.Therefore,through the imaging findings of patients with lumbar disc herniation,this study analyzed and discussed the correlation between lumbar disc herniation and degeneration of lumbar disc and facet joint,so as to provide scientific basis for clinical diagnosis and scientific selection of treatment of lumbar disc herniation and degeneration of facet joint and other spin-related diseases.Methods: MRI and CT imaging data were used to record and observe the relevant imaging findings,such as the grade of lumbar disc degeneration,the grade of bilateral facet joint degeneration,the grade of the degree of frontal disc herniation,the segment of herniation,whether there are multiple herniations,etc.The facet joint Angle was measured under the CT bone window,and the Angle between the outer edge points of the spinous process and facet joint space and the spinous process(hereinafter referred to as the spinous process-joint space Angle)was measured,and the horizontal distance and vertical depth of the joint space between the spinous process and facet joint were measured.At the same time,the patient’s name,gender,age and other relevant information were recorded.Finally,relevant data statistics and analysis.RESULTS: A total of 122 cases were included in this study,including 73 male patients and 49 female patients,with no significant gender difference(P > 0.05),ranging in age from 24 to 88 years old with an average age of56±13.06 years old.The results showed that there was no significant difference in the grade of facet degeneration and the symmetry of facet joints among different genders.There was no significant difference in the facet joint angles of L3/4 and L4/5 segments among different genders.The facet joint angles of L5/S1 segments were significantly different between men and women,and the facet joint angles of females were about 3° larger than that of males.There was no significant correlation between the grade of facet joint degeneration in the L3/4 lumbar vertebrae and age,while the grade of facet joint degeneration in the L4/5 and L5/S1 lumbar vertebrae was positively correlated with age.The size of facet joint Angle of the same lumbar segment had no statistical significance in different age groups.There was no significant correlation between facet joint symmetry of different lumbar segments and age.The degree of lumbar disc degeneration was positively correlated with age(P < 0.05).There was a significant positive correlation between frontal herniation of lumbar intervertebral disc and grade of facet joint degeneration(P < 0.05).There was no significant correlation between the degree of intervertebral disc degeneration in L3/4 and L4/5 segments and the degeneration of facet joints(all P < 0.05).There was a significant positive correlation between the degree of intervertebral disc degeneration at L5/S1 segment and the grade of facet joint degeneration(P < 0.05).There was no significant correlation between the degree of disc degeneration in L3/4 and L5/S1 segments and the symmetry of facet joints(all P = > 0.05).There was a significant positive correlation between the degree of disc degeneration in the L4/5 segment and the symmetry of the facet joints(P < 0.05).There was no significant correlation between lumbar disc herniation and facet joint symmetry(P = > 0.05).There was no significant difference in the grade of bilateral facet joint degeneration in the same lumbar segment(P = > 0.05 for all).However,the overall grade of facet joint degeneration in L5/S1 segments was higher than that in L3/4 and L4/5 segments.There was no significant difference in bilateral facet joint angles between L4/5 and L5/S1 segments(all P > 0.05).The angles of the left and right facet joints in the L3/4 segment of the lumbar spine were different(P < 0.05).The size of bilateral spinous process and joint space Angle in L3/4 and L4/5 segments was different(all P< 0.05),while there was no significant difference in the size of bilateral spinous process and joint space Angle in L5/S1 segments(P > 0.05).The horizontal distance between bilateral spinous process and joint space at L3/4 and L4/5 levels was significantly different(all P < 0.05),while there was no significant difference in the horizontal distance between bilateral spinous process and joint space at L5/S1 levels(P > 0.05).There was no significant difference in the vertical depth of the spinous process talar joint space between the two sides of the same lumbar segment(P > 0.05).The spinous process and joint space Angle of L4/5 and L5/S1 segments were negatively correlated with age(P < 0.05 for all),while the spinous process and joint space Angle of L3/4 segments was not significantly correlated with age(P >0.05).There was no significant correlation between the horizontal distance of spinous process and joint space of each lumbar segment and age(all P > 0.05).The vertical depth of spinous process to joint space in L3/4 and L5/S1 segments was significantly positively correlated with age(P < 0.05),while the vertical depth of spinous process to joint space in L4/5 segments was not significantly correlated with age(P > 0.05).There was no significant correlation between lumbar disc herniation and the Angle of spinous process-joint space,the horizontal distance of spinous process-joint space,and the vertical depth of spinous process-joint space(all P < 0.05).Conclusion: 1.Protrusion of lumbar intervertebral disc is positively correlated with the degree of degeneration of lumbar intervertebral disc and degeneration of facet joints,that is,the more serious the herniation of lumbar intervertebral disc is,the more obvious the degeneration of lumbar intervertebral disc and facet joints is,and vice versa.2.In the same segment,the size of facet joint Angle has no correlation with age,but as the lumbar segment goes down,the facet joint Angle becomes larger and larger,that is,the direction of joint space becomes more and more coronal.3.The degree of degeneration of lumbar intervertebral disc and facet joint are positively correlated with age,but the relationship between lumbar intervertebral disc degeneration and facet joint degeneration still needs to be further clarified.4.There was no significant correlation between facet joint symmetry and lumbar disc herniation or age,and there was no significant difference between facet joint symmetry of different lumbar segments.5.There was no significant correlation between the degree of lumbar disc degeneration and facet joint symmetry.The degree of intervertebral disc degeneration at L3/4 and L4/5 segments was not significantly correlated with facet joint degeneration,while the degree of intervertebral disc degeneration at L5/S1 segment was positively correlated with facet joint degeneration grade.6.There was no significant difference in the grade of degeneration of the facet joints on both sides.There was no significant difference in the size of the facet Angle on both sides of the L4/5 and L5/S1 segments,while the facet Angle on the right side of the L3/4 segment was slightly greater than that on the left.7.The Angle of spinous process to joint space and the horizontal distance of spinous process to joint space on both sides were greater on the right side than on the left side at the levels of L3/4 and L4/5,but there was no significant difference at the L5/S1 levels.The lower the lumbar level,the larger the spinous process-joint space Angle and the longer the horizontal distance between the spinous process and joint space.There was no significant difference in the depth of bilateral spinous process-joint space,but the lower the segment,the shallower the depth.8.There is no significant correlation between lumbar disc herniation and the shape of lumbar joint complex.9.The horizontal distance between spinous process and joint space was not significantly correlated with age,while the vertical depth of spinous process joint space was positively correlated with age,and the L4/5 and L5/S1 spinous process joint space angles were negatively correlated with age.10.There was no significant difference in the depth of spinous process to joint space on both sides.The spinous process to joint space Angle and the horizontal distance of spinous process to joint space on both sides were greater on the right side than on the left side at the levels of L3/4 and L4/5,while there was no significant difference at the L5/S1 levels.The lower the lumbar segment,the larger the spinous process-joint space Angle,the longer the horizontal distance between the spinous process-joint space,and the shallower the depth of the spinous process-joint space.
