Objective To evaluate the relationship and mechanism between facet joint osteoarthritis and intervertebral disc degeneration in lumbar spine.Method Collect Comuputed Tomography (CT)and magnetic resonance imaging (MRI) datas of 120 cases of patients with lumbar disc herniation. According to Pfirriman standard,Disc degeneration will be divided intoⅠ,Ⅱ,Ⅲ,Ⅳ,Ⅴgrades, By Weishaupt standard,the lumbar facet joint osteoarthritis will be divided into 0,1,2,3,4 grades, measuring 100 cases lumbar MRI examination was normal in L3-4, L4-5, L5-S1 intervertebral segment height of the normal distribution of average normal values is L3-4:11.6±0.87mm, L4-5:11.8+0.92 mm, L5-S1:10.3±0.91mm,The loss of intervertebral disc heights will be divided into 0,1,2,3 4grades. Grade 0:intervertebral disc height is equal or less than normal 0-10%, grade 1:less> 10%<20%, grade 2 less>20%<30%, grade 3 less>30%<40%, grade 4 less > 40%. All patients were divided into four groups by age. Group 1:equal or less than 30 years old, group2:31-49 years olde, group 3:50-69 years old, group 4: above 70 years old. All the data were analyzed by Logistic Ordinal Regression ways of SPSS 17.o statistics for it's e correlation.Result:1.After the model likelihood ratio test, lumbar facet joint ostesoarthritis modelχ~2=146.188, P<0.001, Deviance test P= 0.959. Age and lumbar facet joint osteoarthritis was positively correlated (P<0.001); relative to the normal disc (DL= 5),Ⅰ°~Ⅳdegree of disc degeneration by (DL= 4~DL= 1) of the lumbar facet joints the degree of osteoarthritis were significantly increased (P≤0.024). relative to the intervertebral disc height between the normal (GL= 5), only theⅡdegree of intervertebral disc height reduction segment with osteoarthritis of the lumbar facet joints is significantly increased (P= 0.037), whileⅠ,ⅢandⅣdegree of intervertebral disc height reduction no statistically significant difference (P≥0.183).2. After the model likelihood ratio examination, lumbar disc degeneration modelχ~2= 133.271, P<0.001, Deviance test (P= 0.964) were statistically significant. Age and lumbar disc degeneration was positively correlated (P= 0.003). Relative to the normal, mild, moderate and severe lumbar facet joint osteoarthritis, whose disc degeneration were significantly increased (P≤0.002), compared with intervertebral disc height between the normal,Ⅰ°~Ⅳdegree of intervertebral disc height reduction of intervertebral disc degeneration were significantly increased (P <0.001).Conclusion:1.It has a good fit and a positive correlation between lumbar facet joint osteoarthritis imagingwith lumbar intervertebral disc degeneration. The normal lumbar intervertebral disc plays an important role to avoid the occurrence of a facet joint osteoarthritis, The disc degeneration easily induces and enhances the lumbar facet joint osteoarthritis.2. Only theⅡdegree of of intervertebral disc height reduction among those with osteoarthritis of lumbar facet joints increased the severity of radiological changes, In the other classification of lumbar facet joint osteoarthritis imaging changes is consistent with the normal process,Between the majority of vertebral facet joint osteoarthritis imaging with disc height reduction was no significant correlation.The view in the group cases can not be supported that the lumbar facet joint osteoarthritis is caused by loss of height of intervertebral disc degeneration.3. It has a good fit and a positive correlation between lumbar intervertebral disc degeneration and facet joint osteoarthritis. The normal lumbar facet jointc plays an important role to avoid the lumbar intervertebral disc degeneration. The facet joint osteoarthriti easily induces and enhances the lumbar intervertebral disc degenerationfacet joint osteoarthritis.4. Nature aging is basic reason for Lumbar disc degeneration and facet jiont osteoarthritis. but not a purely natural aging process, the mechanical abnormalities and instability resulted by intervertebral discs degenerative and facet joints osteoarthritis is an important factor of causing and increasing each other loading through degenerative process. It suggests facet jiont osteoarthritis and the degeneration of lumbar intervertebral disc might be the cause and effect of each other during occuring and developping.5. The authors used a new simple measurement method for lumbar intervertebral discs height, can overcome the depression caused by the measurement of end-plate distortion, and generally applicable to the measurement of low lumbar intervertebral disc height in clinical practice.6.Lumbar intervertebral disc and facet joint is the main factor of stability and activities. Facet joints, intervertebral disc is bound links amang biomechanics, pathology and anatomy.It is must be avoid new instability induced by excessive damage facet jiont on lumbar decompression operation. Natural aging and abnormal mechanical 1 is the main factors loading throug lumbar facet joint osteoarthritis and disc degeneration.So, it is must fully consider the integrity and connectivity in the diagnosis and treatment for keep more retention lumbar motion segment function, better eliminate back pain and rebuild the biomechanical stability of lumbar spine.
