| Objective: To analyze the curative effect of nano-silver dressing combined with mussel adhesive protein spray on wound healing after third-degree burn operation,and to provide reference for clinical treatment of third-degree burn.Methods: 66 patients with third-degree burn were randomly divided into group A,group B and group C,with 22 cases in each group.A group was given vaseline gauze dressings,B group was given nano-silver dressing,C group was given mussel adhesive protein spray in combination with nano-silver dressing.Wound healing,healing time,visual analogue scale before and after the treatment,germiculture positive compare three groups of patients before and after treatment of wound secretion,the skin graft survival rate and secondary debridement,The incidence of systemic or local adverse reactions before and after treatment was recorded.Results:1.On the 7th day after the operation,the wound of group A had purulent discharge,and the skin turned yellow,but did not form fiber healing.On the 28 th day after the operation,the wound healed,and the unhealed root was bright red,and the skin expanded and fused.In group B,the bottom of the wound was bright red,the skin was good,and the color was light yellow.The wound healed 28 days after operation.In group C,on the28 th day after surgery,the root of the wound was bright red,the skin was firmly bonded,and the wound healed.The wound heals completely,forming a dry Sc.The wound healing rate of group C was(83.27±4.24)%,higher than that of group B(72.55±2.58)%,and significantly higher than that of group A(60.32±3.18)%(P <0.01).2.After 7 days,the VAS scores of the three groups were lower than those before treatment(P <0.01).After treatment,the VAS scores of group C were(3.32±0.99),which was lower than that of group B(5.18±0.85).And significantly lower than group A after treatment(5.36±0.85)(P <0.01).3.7 days after operation,the survival rate of skin grafting in group C was 90.91%,which was higher than that in group B(81.82%)and significantly higher than that in group A(59.09%)(P <0.05).4.After treatment,the wound healing time of group C was(20.64±2.06)days,shorter than that of group B(23.50±1.79)days,but significantly shorter than that of group A(28.27±1.93)days(P <0.01).5.One week after surgery,the positive rate of bacterial culture in wound secretions of the three groups was lower than that before treatment(P <0.05).The positive rate of bacteria culture of wound secretion in group C was 13.64%,which decreased by 77.27%compared with 90.91% before treatment.The positive rate of bacteria culture in the wound of group B was 22.73%,which was 72.72% lower than that before treatment(95.45%).The positive rate of bacterial culture of wound secretion in group A was63.64%,which was 27.27% lower than that before treatment(90.91%).The positive rate of bacterial culture in wound secretion of group C was significantly better than that of group A and group B,and better than that of group A(P < 0.01).6.After treatment,the secondary debridement rate of group C was 4.55%,which was lower than that of group B(13.64%),and significantly lower than that of group A(31.82%)(P <0.05).7.In group A,5 patients developed fever about 1 week after surgery,and nearly half of them developed pruritus within 2-3 weeks.In group B,1 patient developed fever approximately 1 week after surgery and 6 patients developed pruritus 2-3 weeks after surgery.In group C,two patients developed pruritus at 2-3 weeks.Conclusion:Nano-silver dressing combined with mussel adhesive protein spray has a significant effect on wound healing after scab cutting and skin grafting for third-degree burn,which is worthy of clinical promotion. |
