| OBJECTIVES: This study is designed to apply Acticoat(nanocrystalline silver dressing) on the wounds of the second degree burn patients and observe the healing time and the number of bacteria in different time of wounds,as well as the patiente adverse reaction.It manifests the influence of Acticoat(nanocrystalline silver dressing) on prevention of infection and healing of the second degree burn wounds,and its safety,as well.BACKGROUNG:The The local application of antimerobial agent is still one of major measurement on preventing invasive infection of burn wouns.Silver is an effective antimicrobial agent,but older silver-containing formulations are rapidly inactively by the wounds. Environment,frequent replenishment. Acticoat is a relatively new form of silver antimicrobial barrier dressing, which helps avoid the problem of earlier agent.It has rapid and sustained bacterial activity and reduces inflammation,so that promotes healing.METHODS: Sixty burn patients with superficial second degree and deep second degree,whose burn wounds of relative consistence, were enrilled.The burn wounds of deepness equal consistence of the each case were randomly divided into three comparison area.Acticoat was applied onto the wounds of the trial group(A group), the silver sulfadiazine(SD—Ag) cool frost (B group) and Arnebia root oil dressing(C group) were used in comparison groups,respectively.Their healing time were recorded , and bacterial cultures were also performed before and after management in each patient. The treatment of wounds is in the same way: after management of water blister and removaling sepatated epidermis, chlorhexidine is used to bathing the wounds. A group:after Acticoat is wetted out,its blue side is put on the wounds,then wounds is bandaged with many dressing.B group: the silver sulfadiazine(SD—Ag) cool frost is used on wound(80 cm~2/5g) ,and its thickness is 2-3mm.C group:a single Arnebia root oil dressing is used on wounds.All the wounds dressing are changing by every 3 days.Bacteria is cultured when patients admitted,the fifth day of superficial second degree after management,the seventh day of deep second degree after management,respectively. Excretion is taken by the cotton bud,its extent 3mm×3mm,then excretion is put in sterile tubeData handling is used by SSPS10.0:the comparison of numeration data and ratio is used by X~2-test; measurement data is used by T-test; comparison of healing time is used by q-test; comparison of bacteria cultured rating is used by line X array X~2-test; tendency of healing is used by tendency graph,respectively.RESULTS: In their bacterium colonization on wounds, the germ tested rate after management was: A group 0%,B group 13.3%,C group 28.3% respectively.The bacterial cleaning in group A was higher than in group B (P<0.05) and in group C (P<0.01). In group A ,the wound healing time of superficial and deep second degree was shorter than those in group B(P<0.05) and in group C(P<0.01).CONLUTIONS: Apllying Acticoat(nanocrystalline silver dressing) is effectly shortened healing time and preventing bacteria colonization.It need no replacement of dressing frequently,and its manipulation is convenient and simple.Acticoat(nanocrystalline silver dressing) can be used on second degree burn wounds and accelerated wounds healing ,and have notable contend with germ. |
