Objective:Sarcopenia is an age-related health condition.Normal people lose about 1% to2% of muscle mass every year from the age of 30.By the age of 80,more than 30% of the muscle mass has been lost.Depending on the race and the method of diagnosing sarcopenia,the incidence of sarcopenia is about 10% to 25%,but the incidence can be as high as 50% in elderly people aged 80 to 89.Vitamin D deficiency is a global health problem.It is more common for the elderly due to restrictions on their own activities,low vitamin D intake or synthesis,and chronic diseases.Studies have shown that 70% to 90% of the elderly over 60 years old in my country have vitamin D deficiency,and more than 90% of the 80~89 year old people have vitamin D deficiency.However,few studies have evaluated the relationship between vitamin D and sarcopenia.Therefore,this study aimed to evaluate the relationship between plasma 25-hydroxyvitamin D(25(OH)D)concentration and the risk of sarcopenia.The results will help increase the popularization of sarcopenia knowledge,strengthen early diagnosis and intervention for sarcopenia,and will further improve the quality of life of the elderly in their later years and promote the realization of healthy aging.Methods:The Ishii sarcopenia score developed by Japanese scholars Ishii and others in2014 is used to assess the probability of individuals suffering from sarcopenia.The Ishii score only requires simple physical measurements,avoids the cumbersome determination of muscle mass and function,and can provide a cheap and safe method for extensive screening of sarcopenia.The specific calculation formula of Ishii scoring is as follows: Male score =0.62(age-64)-3.09(grip strength-50)-4.64(calf circumference-42);Female score =0.80(age-64)-5.09(grip strength-34)-3.28(calf circumference-42).Judging the probability of sarcopenia in the elderly based on the score of the formula.The recommended cut-off point is 105 points for males and 120 points for females.The collected venous blood was stored at-80°C and frozen until analysis,and the concentration of 25(OH)D in plasma was determined by enzyme-linked immunosorbent assay.The coefficient of variation between and within the experiment was <10% and <8%,respectively.A total of 2335 people aged 65 years or older were included in the study.Univariate Logistic regression was used to analyze the influencing factors of sarcopenia in the elderly.According to 25(OH)D level,participants were divided into5 categories according to quartile,with cut-off values of 15.5,20.5,25.2,and 31.5 ng/m L,respectively.The relationship between the two was analyzed by multivariate logistic regression model,and possible confounding factors were adjusted for.Associations were assessed using odds ratios(ORs)and 95% confidence intervals(CIS),and the dose-response relationship between 25(OH)D and sarcopenia was explored using restricted cubic splines.Subgroup analysis was used to explore whether age,marriage,smoking,alcohol consumption,and exercise status confused the association between plasma 25(OH)D concentration and sarcopenia.Results:Multivariate Logistic regression analysis showed that plasma 25(OH)D was significantly associated with sarcopenia,and the risk of the disease decreased with the increase of vitamin D level.Participants with plasma 25(OH)D concentrations of >31.6 ng/ml had a significantly lower risk of sarcopenia than those with plasma concentrations <15.5 ng/ml(OR: 0.53,95%CI: 0.34~0.82,P<0.05).The dose-response curves of the elderly aged 65 to 79 years showed a U-shaped correlation,with the lowest risk concentration of 27.32 ng/ m L and the best range of25.03 to 29.85 ng/ m L.When the plasma concentration of 25(OH)D reached 22.78 ng/m L,the risk of sarcopenia tended to be stable.Conclusion:1、It is suggested that being married,high cognitive score,regular physical exercise,high body mass index,good diet score,and suffering from central obesity are protective factors for sarcopenia.2、The results of the study showed that older age(≥80 years old),women,Han nationality,and smoking are risk factors for vitamin D deficiency in the elderly.At the same time,it is suggested that living in rural areas,being married,and having a high cognitive score may be protective factors for vitamin D deficiency.3、This study determined the association between plasma 25(OH)D concentration and the occurrence of sarcopenia.The results show that lack of vitamin D will significantly increase the risk of sarcopenia,and as the level of vitamin D increases,the incidence of sarcopenia gradually decreases.This suggests that vitamin D supplementation may help improve or prevent sarcopenia.4、The results of the study showed that the protective effect of increasing plasma 25(OH)D content on the occurrence of sarcopenia was particularly prominent in elderly people over 80 years old,who did not exercise regularly,received education,did not smoke,and did not drink alcohol(P<0.05).5、The dose-response curve of plasma 25(OH)D and sarcopenia,and the optimal concentration of vitamin D were different with age and sex.
