Background: Myopenia is a kind of disease in which the quality,strength and function of skeletal muscle are weakened or lost in the elderly.Vitamin D plays an important mediating role in both bone metabolism and muscle metabolism,and the relationship between Vitamin D and myositis is attracting more and more attention.However,less research has been done on sarcopenia in Chinese people,and the related data are thin.Especially in the north and South regions of China,the span is large,the research data in the north are not enough.The relationship between myositis,bone mineral density and vitamin D remains unclear.Objective: To analyze the correlation among muscle strength,muscle mass,bone mineral density and vitamin D in elderly women over 65 years old in north of China,so as to enrich clinical data and provide reasonable Suggestions for the prevention and treatment of the disease.Methods: 30 elderly women with sarcopenia in Tianjin area were selected as the sarcopenia group,and 30 elderly women without sarcopenia at the same time were selected as the non-sarcopenia group.Their general information including height,weight,body mass index,comorbidity,medical history,smoking and alcohol abuse were collected.Appendicular skeletal muscle mass index(ASMI)was determined by dual energy X-ray absorptionmetry(DXA),maximum grip strength was determined by handgrip force meter,and 4 m step speed was determined by stopwatch.Lumbar(L1-L4),hip and femoral neck BMD were measured by dual-energy X-ray bone density meter,and T-score was calculated.And the fasting venous blood was separated,the serum alkaline phosphatase and vitamin D levels were detected using ELISA experimental determination,blood calcium phosphorus were detected with and azo arsenic ? method and molybdophosphate photometric method.Results:(1)data comparison between elderly women with and without sarcopenia There was no significant difference in age,height,weight,basic complications,smoking,drinking,walking speed,blood calcium,blood phosphorus and alkaline phosphatase between the two groups of elderly women(P > 0.05).The body mass index,SMI,grip strength,BMD of lumbar vertebra,BMD of femoral neck,BMD of l hip,serum 25-hydroxyvitamin D level in the muscular dysplasia group and the T-score of lumbar vertebra,T-score of femoral neck,T-score of l hip were all lower than those in the non-muscular dysplasia group,and the differences were statistically significant(P < 0.05).(2)risk factors for myosis Logistic regression analysis showed that SMI < 5.45 kg /m2,grip strength < 18 kg,lumbar T-score ?-2.5,femoral neck T-score ?-2.5,hip T-score ?-2.5,serum 25 hydroxyvitamin D < 18.2nmol /L were risk factors for myolipomia(OR > 1.0,P < 0.05).(3)the relationship between myosuria SMI and various indicators Pearson test results showed that lumbar |T-score|,femoral neck |T-score| and hip |T-score| were negatively correlated with SMI(r=-0.645,-0.658,-0.718,P < 0.05),and serum 25-hydroxyvitamin D levels were positively correlated with SMI(r=0.800,0.697,P < 0.05).(4)the predictive value of each indicator for myosis ROC curve analysis showed that lumbar |T-score|,femoral neck |T-score| and hip |T-score|,serum 25-hydroxyvitamin D levels had a certain predictive value for myosia(AUC=0.694 ~ 0.830,P < 0.05).Conclusion: 1.Low 25-hydroxyvitamin D level is an important pathological feature of oligomyosis in elderly women,which may be involved in the occurrence and development of myosuria...2.The incidence of osteoporosis in sarcopenia group is higher than that in non-sarcopenia group,which may be closely related to skeletal muscle and may be related to biomechanics and biochemistry.3.lumbar BMD,femoral neck BMD and hip BMD,serum 25-hydroxyvitamin D have good predictive value for oligosis and can be used as auxiliary indicators for clinical diagnosis and disease evaluation.
