| Objective:To analyze the clinical characteristics of colorectal cancer in Yanbian area and its correlation with type 2 diabetes mellitus,so as to provide clinical reference for early diagnosis,treatment and screening of colorectal cancer.Methods:A total of 821 patients with colorectal cancer treated in the Hospital of Yanbian University from November 2014 to November 2019 were divided into two groups according to the location and age of the disease.The group with type 2 diabetes mellitus was taken as the observation group(n=141),and the group without type 2 diabetes mellitus as the control group(n=680).The clinical data of each group were compared and analyzed,including sex,age,nationality,BMI,smoking history,drinking history,location,pathological type,TNM stage of colorectal cancer,blood routine,tumor markers CEA,CA199 and CA125,etc.Analysis of the clinical characteristics of colorectal cancer in Yanbian area and its correlation with type 2 diabetes mellitu.Results:(1)In 821 cases of colorectal cancer,there were 369(44.9%)patients with rectal cancer and 452(55.1%)patients with colon cancer.In colon cancer,male 269(59.5%),Korean 241(53.6%),rectal cancer male 246(66.7%),Korean 165(46.5%).In colon cancer,PLT 260.49±95.98x10^9/L,CA1259.80(6.13-15.80)U/mL,rectal cancer PLT 224.30±71.32x10^9/L,CA125 8.10(5.50-12.50)U/mL.There were significant differences in sex,nationality,PLT and CA125 between the two groups(P<0.05).There was no significant difference in age,pathological type,smoking history,drinking history,WBC,HGB,CEA and CA199 between the two groups.(2)Among the different age groups,there were 21 cases in the 18-45years of age group,514 cases in the 46-69years of age group and 286 cases in the≥70years of age group.The number of patients in the 46-69years of age group was the highest,with 514 cases.Adenocarcinoma accounted for the most in all age groups,accounting for 85.7%,85.4%,86.4%,respectively.There was no significant difference in pathological types of colorectal cancer among different ages(P>0.05).The main symptoms of each age group were abdominal pain,abdominal discomfort and hematochezia,accounting for 85.6%,56.2%and 64.0%respectively.There was no significant difference among different age groups(P>0.05).(3)A total of 803 cases of surgical resection.The levels of CEA and CA125 in TNM stages Ⅱ,Ⅲ andⅣ were higher than those in stage Ⅰ;The level of CA199 in stage Ⅳ was higher than that in stageⅠ;The difference is statistically significant(P<0.05).The levels of three tumor markers in stage Ⅳwere higher than those in stage Ⅱ and stage Ⅲ.The difference is statistically significant(P<0.05).(4)The prevalence rate of type 2 diabetes in patients with colorectal cancer(17.17%)was higher than that in Chinese adults(10.4%)and Yanbian adults(4.3%).There was no statistical difference in TNM staging between the group of colorectal cancer with type 2 diabetes and group without type 2 diabetes.The stage Ⅰ,stage Ⅱ and stage Ⅲ and stage IV group of colorectal cancer without type 2 diabetes mellitus were 629(94.6%)and 36(5.4%)respectively,while those in the group with type 2 diabetes mellitus were 122(88.4%)and 16(11.6%),respectively.There was significant difference between the two groups(P<0.05).The proportion of adenocarcinoma in the group of colorectal cancer with type 2 diabetes and group without type 2 diabetes was 88.7%and 85.1%respectively,and there was no significant difference between the two groups(P>0.05).(5)The level of CA-199 in the colorectal cancer complicated with type 2 diabetes mellitus group was was 12.00(5.60-34.75),which was higher than that in the group without type 2 diabetes mellitus.The difference is statistically significant(P<0.05).(6)Obesity and hypertension accounted for 34.0%and 36.2%in the colorectal cancer complicated with type 2 diabetes mellitus group,20.6%and 23.8%in the group without type 2 diabetes mellitus respectively.The prevalence rates of obesity and hypertension in colorectal cancer with type 2 diabetes were higher than those without type 2 diabetes.The difference was statistically significant(P<0.05).Conclusion:(1)Colorectal cancer in Yanbian area is mainly male;Korean nationality accounts for a high proportion of colon cancer.(2)The levels of PLT and CA125 in colon cancer are higher than those in rectal cancer,which can be used as a reference index to distinguish the location of tumor.(3)Patients with colorectal cancer complicated with type 2 diabetes mellitus have relatively late tumor stages and are prone to distant metastasis..(4)The level of CA199 in patients with colorectal cancer complicated with type 2 diabetes is higher,and the abnormal results caused by type 2 diabetes should be considered when evaluating the condition.(5)People with type 2 diabetes mellitus complicated with obesity or hypertension in Yanbian area should be highly alert to the incidence of colorectal cancer. |
