Background:Type 2 diabetes mellitus(T2DM)and colorectal cancer(CRC)are worldwide public health issues that endanger human health.Over the past 20 years,the number of people dying of diabetes and colorectal cancer worldwide has increased by 90% and 57%,respectively.In recent years,with the rapid increase in the prevalence of obesity and type 2 diabetes,the number of patients with type 2 diabetes and colorectal cancer has also increased significantly.Compared with non-diabetics,type 2 diabetes patients have a higher incidence of colorectal cancer Rate and mortality.Therefore,it is of great significance to study the susceptibility of patients with type 2 diabetes and colorectal cancer.In recent years,it has been found that adipose tissue is not only a tissue that stores energy,but also an important endocrine organ.Adipose tissue can produce a variety of physiologically active adipose factors through autocrine,paracrine and remote secretion,including adiponectin,interleukin-6,tumor necrosis factor-α,leptin,resistin and adiponectin.These factors have become the research focus of obesity,type 2diabetes and even colorectal cancer,but the expression of these factors in T2DM&CRC is rare.Genome-wide sequencing showed that the related site of adiponectin gene was the susceptible site of type 2 diabetes.Genes related to fat and obesity can also affect type 2 diabetes through a variety of mechanisms.Clinical studies have also confirmed that SNPs,which regulates metabolism and obesity-related gene loci,is closely related to T2 DM.CRC and T2 DM share common etiology and similarity in population genetic background.The screening of susceptible genes often comes from candidate genes of obesity and type 2 diabetes.Therefore,it is necessary to explore the relationshipbetween adiponectin gene,fat weight and obesity-related gene SNPs and T2DM&CRC patients,in order to find its genetic markers.Objective:To analyze the relationship between the changes of adiponectin levels in patients with T2DM&CRC and type 2 diabetes mellitus complicated with colorectal cancer,and to explore the relationship between adiponectin rs1501299,adiponectin rs2241766,FTO rs9939609 gene polymorphism and type 2 diabetes mellitus complicated with colorectal cancer,in order to obtain more significant risk factors and create conditions for early diagnosis,prevention and treatment of T2DM&CRC.Methods:In this study,a case-control association analysis method was used to select 151 cases of type 2diabetes hospitalized in our hospital from September 2017 to December 2019 as the case group,including patients with colorectal cancer 78 patients,83 patients without colorectal cancer,and 60 healthy people who were examined in our hospital during the same period were selected as the normal control group,whose age and gender matched the case group.Enzyme-linked immunosorbent assay(Enzyme-linked immunosorbent assay,ELISA)was used to determine serum adipokine levels;SNa Pshot sequencing method was used for genotyping analysis.Statistical analysis was performed using Spass 22.0 software.Inspection level α = 0.05.Results:1.Comparison of clinical data:(1)There was no significant difference in age,sex,smoking history,drinking history and family history of diabetes among the three groups(P > 0.05);(2).The level of BMI in T2DM&CRC group was significantly lower than that in T2 DM group,while that in T2 DM group was significantly higher than that in control group(P < 0.05).(3)Compared with the control group,the levels ofFPG,2h PG,Hb A1 c,TC and TG in T2DM&CRC group and T2 DM group were higher than those in control group,and the level of TG in T2DM&CRC group was significantly higher than that in T2 DM group(P <0.01).(4)The level of CD4+/CD8+ in T2DM&CRC group was significantly lower than that in T2 DM group and normal control group,and the difference was statistically significant(P < 0.01),Ig A level in T2DM&CRC group and T2 DM group was significantly higher than that in control group(P < 0.05).2.Comparison of adipokine levels: Compared with the control group,the levels of IL-6 and TNF-α in the T2DM&CRC group and the simple T2 DM group were significantly increased(P <0.01),and the APN level was significantly lower than the control group(P <0.01),the difference was statistically significant;compared with T2 DM Compared with the group,the IL-6 level in the T2DM&CRC group was significantly higher than that in the T2 DM group(P <0.01),and the APN level was significantly lower than that in the T2 DM group(P <0.01).The difference was statistically significant;TNF-α had no statistics compared with the T2 DM group Academic difference(P >0.05)..3.Multivariate Logistics regression analysis showed that IL-6 was a risk factor for T2DM&CRC,OR=1.222,P < 0.05),APN was a protective factor for T2DM&CRC,and OR=0.989,P < 0.01),TG was a risk factor for T2DM&CRC(OR=1.365,P < 0.05).4.The results of SNPs detection and comparison: the SNP distribution of adiponectin rs2241766 in normal control group,T2 DM group and T2DM&CRC group was significant(P < 0.01).The risk of),GG genotype with CRC was 4.013 times higher than that of TT genotype,and the risk of GG+TG genotype with CRC was 2.278 times higher than that of TT genotype.There was no significant difference in adiponectin rs1501299 and FTO rs9939609 SNPs among the three groups(P > 0.05).5.Comparison of different genotypes of adiponectin rs2241766: Compared with TT genotype,the serum APN level of GG + TG genotype was significantly lower,the IL-6 level was significantly increased,and the TG level was significantly increased(P < 0.05).Conclusion:1.Patients with type 2 diabetes and colorectal cancer in Henan Han population have higher IL-6levels and lower APN levels.APN is a protective factor for type 2 diabetes with colorectal cancer,and IL-6is type 2 diabetes with colorectal cancer.2.Adiponectin rs2241766 genotype is related to the occurrence of T2DM&CRC in Henan Han population,which may promote the occurrence of T2DM&CRC by affecting the serum APN and IL-6levels.3.Adiponectin rs1501299,FTO rs9939609 genotypes may not be related to T2DM&CRC in Henan Han population.
