Objective : To investigate the clinical pathological characteristics of colorectal cancer with type 2 diabetes mellitus, analysis the impact of type 2 diabetes mellitus and the control of usually glycemic and the influence of duration of diabetes on colorectal cancer clinical pathological characteristic(eg: TNM stage, invasion depth, lymph node metastasis,distant metastasis, differentiation, gross type, histological type, distribution of lesions, cancer nodules, Vessel carcinoma embolus),in order to further clarify whether type 2 diabetes is an independent risk factor for the occurrence and development of colorectal cancer.Methods: 1.By methods of retrospective study, analysis the clinical data of 822 cases of colorectal cancer were diagnosed by pathology. The patients were divide into type 2 diabetes mellitus(103 cases) and non-diabetic mellitus(719 cases) group. We analyzed the differences in two group of gender, age, BMI,other general condition and clinical pathological features. Logistic multiple regression analysis of factors which influencing the clinical pathologic TNM stage of colorectal cancer.2.Analysis the clinical data of 103 cases of colorectal cancer with type 2 diabetes mellitus were diagnosed by pathology. According to the Blood sugar level, the patients were divide into Good glycemic control(56 cases) and Poor glycemic control(47 cases) group. We analyzed the differences in two group of gender, age, BMI,other general condition and clinical pathological features.3. Analysis the clinical data of 103 cases of colorectal cancer with type 2 diabetes mellitus were diagnosed by pathology. According to the course of type 2 diabetes, the patients were divide into Duration of diabetes less than 5 years(58 cases) and Duration of diabetes more than 5 years(45 cases) group. We analyzed the differences in two group of gender, age, BMI,other general condition and clinical pathological features.Results: 1.Analysis of colorectal cancer in patients with type 2 diabetes group and non-diabetic mellitus group clinical pathological characteristics.1.1 In 103 cases(12.5%) patients with colorectal cancer suffering from type 2 diabetes mellitus, the average age of type 2 diabetes group was(65.7±9.1) years which is higher than non-diabetic patients [(60.1±12.0)years, P < 0.01]. Type 2 diabetes group and non-diabetic mellitus group differences in gender, BMI, smoking and drinking history, triglyceride, cholesterol and tumor invasion depth, differentiated degree, gross type, histological type has no statistical significance(P > 0.05). The two groups in the history of hypertension, coronary heart disease and tumor TNM stage, lymph node metastasis, distant metastasis, distribution of lesions, Vessel carcinoma embolusasion, tumor nodules differences have statistical significance(P < 0.05).1.2 Logistic multiple regression analysis of factors influencing the clinical pathologic TNM stage of colorectal cancer prompting: Type 2 diabetes mellitus is one of the risk factors that affect the TNM stage(OR=1.841, P<0.05). But the colorectal cancer patient's age, BMI, history of hypertension, history of coronary heart disease, triglyceride, cholesterol have not correlated with clinical pathological TNM stage(P>0.05).2. Analysis of Blood sugar control situation affect the pathological characteristics in Colorectal cancer with type 2 diabetes mellitus patients with good and poor glycemic control.2.1 There were 56 patients(54.3%) in 103 cases with type 2 diabetes mellitus patients has good control of blood sugar, the average fast blood sugar was(6.03±1.36) mmol/L, glycosylated hemoglobin(Hb A1c) average was(6.3 ±0.39)%; Poor control in 47 cases(45.7%), fast blood sugar was controlled on average(9.85±3.56) mmol/L, and the average control of glycosylated hemoglobin(Hb A1c) was(9.27±1.78)%. Colorectal cancer with type 2 diabetes mellitus patients with good or poor glycemic control between the two groups in sex, age, body mass index(BMI), smoking history, drinking history, history of hypertension, coronary heart disease history, diabetes duration, triglyceride, cholesterol, the differences has no statistical significance(P > 0.05).2. 2.It's showed that there has statistical significance(P<0.05)in Colorectal cancer with type 2 diabetes mellitus patients with good or poor glycemic control between the two groups of clinical pathological TNM stage, lymph node metastasis, distant metastasis, Vessel carcinoma embolusasion,tumor nodules. The two groups in distribution of lesions,depth of invasion, differentiation, gross type and histological type differences has no statistically significant(P>0.05).3.Analysis of pathological characteristics in Colorectal cancer with type 2 diabetes mellitus patients with the duration of type 2 diabetes less or more than 5 years between the two groups.3.1 In the 103 cases, 58 cases' s course were less than or equal to 5 years(56.3%), the average course was(1.8±1.67) years. 45 cases' s course of disease was more than 5 years(43.7%), the average course was(10.11±3.43) years. It has no statistical significance(P >0.05) in Colorectal cancer complicated with type 2 diabetes patients between the two groups in age, gender, body mass index(BMI), smoking history, drinking history, history of hypertension, coronary heart disease, triglyceride, cholesterol, fast blood sugar difference.3.2 It has no statistical significance(P > 0.05) in Colorectal cancer complicated with type 2 diabetes patients between the two groups with diabetes duration is less than or equal to 5 years, more than 5 years in clinical pathological TNM stage, depth of invasion, lymph node metastasis, distant metastasis, Vessel carcinoma embolusasion, tumor nodules, tumor location, degree of differentiation, gross type, histological type difference?Conclusion:The tumor pathological stage is relatively late, prone to lymph node metastasi, Vessel carcinoma embolusasion, tumor nodule formation and distant metastasis in Colorectal cancer patients with type 2 diabetes mellitus.Type 2 diabetic patients with good blood sugar control can reduce the risk of lymph node metastasis and distant metastasis of colorectal cancer. Type 2 diabetes may be an independent risk factor for the occurrence and development of colorectal cancer,we should enhance the screening and follow-up of colorectal cancer in patients with type 2 diabetes.
