Abstract/Summary:
Malnutrition and inflammation are prevalent in the long-term care (LTC) population and have a great impact on the quality and length of life. Nutrition provides the common link between these areas. Literature defines and illuminates the needed components of a tool for a registered dietitian to assess elderly patients in a LTC facility. Few available forms, if any, incorporate the current Nutrition Care Process (NCP) and Minimum Data Set (MDS), while also addressing inflammation and fluid status, specifically for the LTC population.;The purpose of this project was to create a nutrition assessment form to incorporate a full subjective and objective review, with attention to inflammatory identification that incorporated NCP and MDS triggers. The goal of developing this assessment tool was to provide a form to appropriately assess factors related to elderly patients in a LTC facility in order to implement the most fitting intervention and plan.
