Objective Gastrointestinal surgery is a hazardous department in nutrition,bee ause the gastrointestinal tumor patients have to surgery.For the influential patients,nu trition support can improve clinical outcome.But the unfitted nutrition support will in crease the patients’ complication,medical charges and the length of stay.The author t hrough to the gastrointestinal nutrition and hernia surgery with nutritional risk refers t o the disease and nutritional support(nutrition support,NS)of the patients with indivi dualized pharmaceutical care,in the process of PN and EN support,discusses the prac tice of pharmaceutical care point.Methods Select a gastrointestinal surgery,163 cases of 3 armour hospital nut rition score 3 points or more,ranging in age from 18 to 90 years old,was in the hospit al and more than 1 day of patients for pharmaceutical care,nutritional support therapy for temporary<nutrition score 3 points of adult patients with weekly nutrition screen ing again,if a score of 3 or more points and nutritional support of patients back into th e monitoring management.Pharmaceutical guardianship methods:(1)the succession,medicine,daily rounds,pharmacy rounds of records of the patient’s diet,nutrition,and medical history,the importance of nutrition support to communicate with patients’ fa milies,as well as the matters needing attention during the nutrition support,writing m edication education convenient patients’ families better care for patients with nutrition al support,improve the compliance of patients;(2)in patients with medical combinatio n of cases and the status quo,analysis of patients’ biochemical indicators,combined w ith doctor’s treatment principle and treatment needs,computational demand of nutritio n and reasonable fat than sugar,hot nitrogen ratio;(3)review the day before the nurses with liquid parenteral enteral nutrition prescription,told the points for attention durin g the liquor and process;(4)cases of holidays through the Hospital Information manag ement System(Hospital Information System,HIS)for Information collection,record t he contents of the basic Information,including patients admitted to Hospital diagnosis,nutritional support scheme,the type and usage and dosage of nutritional support,whe ther drug complications and adverse reactions,the biochemical indicators.Results During the period of custody,finding unreasonable prescription for 29 times,number of successful intervention for 27 times,the success rate is 93.1%.Ac cording to the different patients with disease,the rest 2 times are modified after consu ltation with physicians.Finding that patients with parenteral complications of enteral nutrition 26 times,accounting for 15.9%of the total patients,comparing with previou s years,reasonable pharmaceutical care can shorten the hospitalization days and reduc e patient’s hospitalization expenses,and can prevent most nutrition support brings a se ries of complications.Physicians for energy calculation,the three major nutrients sup ply ratio,ion content,vitamin biological efficiency as well as practitioner of liquor co nsiderations and compatibility taboo and so on still need to participate by the clinical pharmacists with professional knowledge,and complications often appear when the u se of nutritional support;to ensure the safe,effective and economical efficiency of pat ients with medication,need an individualized care clinical pharmacists.Conclusion Due to the complexity of the application of clinical nutrition,pha rmacy related professional,and patients with higher incidence of complications,the c linical nutrition support need for the individualized pharmaceutical care and medical e ducation Rationalization of patients with pharmaceutical care can shorten hospitalizati on expenses and hospitalization days and reduce patients complications,etc.,to form t he study of parenteral enteral nutrition and nutrition support way to research on the eff ects of clinical outcomes,not the PN and EN guardianship system research of path me thod,so this paper on the nutritional support of guardianship has certain innovative,th is research can play the role of recommended in the clinical work,provide relevant ref erence for doctors and nurses are,which guarantees patients receive better treatment.
