The relevant data show that the status quo of self-management of diabetes patients in China is not optimistic,and the patient's self-management ability is generally low.Therefore,this study hopes to improve the disease self-management ability of elderly diabetic patients and promote their scientific and effective disease management through the use of group work methods.By means of literature research,the author understands the status of self-management of diabetes in China,and surveys the elderly diabetes patients in the Y community of Jizhou District by means of questionnaires,interviews and observations,in order to understand the present situation,problems and needs of self-management.Eight elderly diabetic patients with low self-management ability and strong willingness to participate were selected as the service targets.According to cognitive behavior theory and social support theory,the group's working methods are used to provide services for their cognitive,behavioral,and psychological aspects,thereby improving the group members 'ability to self-manage diseases,promoting their scientific and effective disease management,and ultimately improving their life and quality of life.Through combing and analyzing the whole practical process,the author finds that the group work has played the functions of education,support and mutual assistance in improving the disease self-management ability of the team members.First of all,through group activities to teach team members about diabetes and correct disease management methods to improve their cognitive ability,behavioral management ability and psychological adjustment ability of diabetes;Secondly,through group activities,it not only enhances the connection between team members,team members,social workers,and volunteers,expands its social support network,but also promotes the harmonious relationship between team members and family and friends,and consolidates and improves its original social support network;Third,through the development of group activities,the team members can use the concept of helping people help themselves to help each other in group activities and ordinary life,and jointly improve the ability of disease self-management.At the same time,it also reflects on the practical process of the group.First,due to the relatively large age ofthe clients,they are more passive when participating in activities,and the social workers have strong dominance and can not inspire the subjective initiative of the group members.Secondly,the author mainly intervenes from the individual level in practice,but not enough from the family level and the community level,ignoring the importance of human in the context and ecosystem theory.Thirdly,because each individual is unique,the work of the group can not meet the needs of each group member.Therefore,in the future practical research,the author should stimulate the enthusiasm of the service object to participate,and comprehensively use the three major methods of social work,so that the practical efficiency reaches or even exceeds expectations.
