Vascular parkinsonism(VP)is a secondary parkinsonism caused by ischemic cerebrovascular disease,and its clinical presentations are bilateral symmetrical gait disorder,increased muscle tension in both lower extremities,cognitive decline and urinary/fecal dysfunction.At present,there is no systematically diagnostic criterion for VP,and the current diagnosis mainly relies on symptoms and cranial imaging examination.Its nuclear magnetic imaging examinations are mainly characterized as diffuse white matter damage or multiple infarcts in the basal ganglia.Currently,there is no standardized treatment program for this disease,and the cognition on the response of dopamine preparation is mixed.Traditional Chinese medicine treatment has certain characteristics in treatment due to its advantage of multiple target points,so my tutor,Director Chen Zhigang,combined with his many years of clinical experience to propose the "Congshan Kidney Nourishing Recipe" for treating vascular parkinsonism,which could reduce the clinical symptoms of patients and delay the progression of the disease.However,the research in large samples is still required to verify the study on its action mechanism and the observation of its clinical efficacy.Objectives:?Assess the clinical efficacy of "Congshan Kidney Nourishing Recipe "by the Chinese and Western Medicine Scale;? Preliminarily discuss the relationship between traditional Chinese medicine syndrome characteristics of vascular parkinsonism and its scale indicators;? Preliminarily discuss the interrelationship of the assessment indicators in each scale.Methods:A total of 50 patients with vascular parkinsonism,who met the inclusion criteria from September 2016 to January 2018 at the Dongfang Hospital of Beijing University of Traditional Chinese Medicine,were screened and selected(inclusion criteria refer to the diagnostic criteria for vascular parkinsonism proposed by Winikates et al.and the diagnosis&efficacy criteria of Chinese medicine for elderly patients with tremor syndrome).All the patients were subjected to Chinese and western medicine scale assessment at the time of enrollment(Part III of the Unified Parkinson's Disease Rating Scale(UPDRS),Montreal Cognitive Assessment Scale(MoCA),Mini Mental State Assessment Scale(MMSE),Ability of Daily Living Scale(ADL)and Pittsburgh Sleep Quality Index(PSQI).After enrollment,they were divided into 40 cases of Chinese medicine group and 10 cases of western medicine group.Among them,the Chinese medicine group was subjected to the regular medication of self-proposed recipe for 6 months,and the western medicine group was subjected to oral levodopa-benserazide 50/12.5 mg three times a day.The scales were assessed on the two groups at the time of enrollment and 1,3,and 6 months after enrollment.The statistical software SPSS 22.0 was used for compilation,statistics and analysis.Results:After 6 months of treatment,40 patients in the Chinese medicine group and 10 patients in the western medicine group presented no statistical differences in the scores of the Part ? of UPDRS,MoCA,MMSE,and ADL scales(P>0.05).After treatment,the PSQI scores of the Chinese medicine group were significantly better than that at enrollment(P<0.05).After the treatment of traditional Chinese medicine,the patients' mental power,daytime function and urination control were better than that at enrollment,and the scores were statistically different(P<0.05).There was no statistical difference in the scores ofChinese medicine treatment for dyskinesia,especially gait disorder.Conclusions:Chinese medicine treatment could help the mental power,sleep quality and urinary control symptoms of VP patients;and it could delay the progression of gait disorder and cognitive disorder.The traditional Chinese medicine syndromes of patients mainly focused on the syndrome of liver&kidney deficiency,phlegm heat and blood stasis.No significant disease progression was observed in patients with these three syndromes after treatment,indicating that Chinese medicine may play a role in delaying the disease.
