Based On Data Mining Technology To Explore Mr. Zhu Renkang's Clinical Experience In Treating Pruritus

Background:pruritus is one of the common clinical symptoms in dermatology,which often causes different degrees of mental pressure and physical pain to patients.The complicated mechanism of pruritus brings some difficulties to antipruritic treatment.Both traditional Chinese medicine and modern medicine are constantly exploring more effective antipruritic methods.Objective:Zhu Renkang had been engaged in dermatology of traditional Chinese medicine for more than 70 years,and accumulated rich clinical experience.Therefore,based on the basic theory of traditional Chinese medicine,combined with data mining technology,we hope to find the experience from Zhu Renkang,to guide the anti-itch treatment of traditional Chinese medicine,improve the clinical efficacy,and serve the majority of patients.Research methods:Using Excel software to establish database.Medical case information is extracted from medical cases that meet the inclusion criteria,including original medical cases,symptoms,pruritus regions,tongue proper,tongue coating,pulse condition,TCM diagnosis,TCM syndrome differentiation,western medicine diagnosis,TCM treatment,prescription name,prescription nature,prescription composition.Delete the duplicate medical records in the original data,keep the more complete one,and standardize the data retained after deletion.Statistical analysis on the frequency of traditional Chinese medicine,skin lesions,pruritus sites and diseases by IBM SPSS statistics 20;use the ancient and modern medical case cloud platform to carry out four Qi,five flavors,meridian tropism,efficacy statistics,skin lesions and traditional Chinese medicine cluster analysis,and use association rules to find out the rule of antipruritic medicines in different diseases and regions;use Liquorice to find out the core prescriptions and common additions and subtractions,to analyze the clinical experience and academic thoughts of Zhu Renkang.Results:This study was finally included in 919 cases,involving 42 kinds of skin diseases,including eczema,urticaria,pruritus,seborrheic dermatitis,neurodermatitis,psoriasis,etc.Zhu Renkang used 302 kinds of traditional Chinese medicines,which were commonly used traditional Chinese medicines for clear away heat and cooling blood,clear away heat and toxic material,clear away heat and dry dampness,evacuating wind-heat and promoting eruption.The more frequently used traditional Chinese medicines were Sheng Gan Cao,Sheng Di Huang,Chi Shao,Dang Gui,Jing Jie,Mu Dan Pi,Huang Qin,Dan Shen,Chan Tui,Ze Xie.The core prescription is composed of Sheng Di Huang,Huang Qin,Jing Jie,Fang Feng,Chan Tui,Ji Li,Dang Gui,Mu Dan Pi,Chi Shao,Dan Shen,Hua Shi Fen,Sheng Gan Cao.Cluster analysis showed that pruritus with skin lesions can be divided into four groups:wheal;scab,scratch;erythema,scale,papule;erosion,blister.The core prescription of anti-itching treatment for pruritus with wheal is composed of Jing Jie,Fang Feng,Chan Tui,Qiang Huo,Dang Gui,Sheng Gan Cao;the core prescription of anti-itching treatment for pruritus with scab and scratch is composed of Sheng Di Huang,Mu Dan Pi,Chi Shao,Dan Shen,Dang Gui,Huang Qin,Bai Xian Pi,Ze Xie,Di Fu Zi,Sheng Gan Cao;the core prescription of anti-itching treatment for pruritus with erosion and blister is composed of Sheng Di Huang,Mu Dan Pi,Chi Shao,Huang Qin,Ze Xie,Hua Shi Fen,Sheng Gan Cao;the core prescription of antipruritic treatment for pruritus with erythema,scurf and papule is composed of Sheng Di Huang,Chi Shao,Dang Gui,Ze Xie,Hua Shi Fen,Sheng Gan Cao.This study also explored the clinical experience of Zhu Renkang in the treatment of pruritus in different regions.The core prescription for pruritus of lower limbs is composed of Sheng Di Huang,Chi Shao,Dang Gui,Huang Qin,Ze Xie,Che Qian Zi,Mu Tong,Hua Shi Fen,Sheng Gan Cao;the core prescription for facial pruritus is composed of Sheng Di Huang,Huang Qin,Chi Shao,Dang Gui,Ze Xie,Hua Shi Fen,Sheng Gan Cao;the core prescription for systemic pruritus is composed of Sheng Di Huang,Chi Shao,Mu Dan Pi,Dang Gui,Ji Li,Bai Xian Pi,Jing Jie,Chan Tui,Sheng Gan Cao;the core prescription of upper limb pruritus is composed of Sheng Di Huang,Huang Qin,Chi Shao,Lian Qiao,Ze Xie,Hua Shi Fen,Sheng Gan Cao;the core prescription of trunk pruritus is composed of Sheng Di Huang,Chi Shao,Dang Gui,Jin Yin Hua,Jing Jie,Ze Xie,Sheng Gan Cao.Conclusion:Zhu Renkang thought,the occurrence of pruritus is closely related to wind,dampness and heat.Pathogenic wind can be divided into internal wind,caused by blood deficiency,blood-heat,blood stasis and blood dryness.And exogenous wind in six pathogenic factors;Pathogenic damp can be divided into pathogenic damp in six pathogenic factors and pathogenic damp caused by spleen and stomach dysfunction;Pathogenic heat can also be divided into pathogenic heat(fire)in six pathogenic factors and internal heat caused by dysfunction of five viscera and abnormal emotion,such as heart-fire hyperactivity,liver depression forming fire.Therefore,Zhu Renkang often used the methods of clearing away heat,cooling blood,promoting blood circulation,and removing wind.Sometimes combined with nourishing yin and dehumidification.The core prescription is composed of Sheng Di Huang,Huang Qin,Jing Jie,Fang Feng,Chan Tui,Ji Li,Dang Gui,Mu Dan Pi,Chi Shao,Dan Shen,Hua Shi Fen,Sheng Gan Cao.In addition,the itch occurred in different regions,with different types of skin lesions or with different types of skin diseases,Zhu Renkang's prescription is also different.
