The eczema-like psoriasia is to point to be diagnosed as psoriasis(PS)at the same time accompanied by exudation,blisters,erosions,scabs,scratchs,lichenification and other characteristics of eczematous skin lesions.In Clinic,some patients with psori--asis have eczema-like lesions during the course of the disease,and it is not uncommon for psoriasis patients to be misdiagnosed as eczema-like dermatitis.According to clinical observations,patients with the eczema-like psoriasia can be relieved quickly after targeted eczema treatment.At present,there are few reports on the mechanism of psoriasis with eczema-like skin lesions.A large number of researches indicated that there is a certain connection between the pathogenesis of psoriasis and allergic skin diseases with eczema-like skin lesions,especially in terms of skin barrier changes and immune disorders.The literature suggests that Thymic stromal lymphopoietin(TSLP),T-oll like receptor(TLR)2,and immunoglobulin E(IgE)play an important role in the development of allergic skin diseases such as eczema(Eczema)and atopic dermatitis(AD).Blood-heat syndrome(progressive stage)of psoriasis is the obvious stage of psoriasis inflammation,and it is an important period to observe and study the eczema-like psoriasia.Based on the pathogenesis of allergic skin diseases,this experim-ent explored the correlation between the eczema-like psoriasia and the above three factors.[Object]By detecting the serum TSLP,TLR2 and IgE expressions of the enrolled patients,comparing the expression differences of the three factors between the experimental group and the control group and the correlation between the three factors and psoriasis(blood-heat syndrome)with eczema-like changes,and then exploring TSLP,TLR2,IgE in the eczema-like changes of psoriasis vulgaris(blood-heat syndrome).It provides theoretical support for patients with the eczema-like psoriasia to undergo targeted eczema treatment at the same time,and also provides experimental data support for in-depth exploration of the pathogenesis of psoriasis and eczema.Compare the differe-nces in basic data(PASI value,BMI,pruritus)between the experimental group and the control group,and explore which factors are involved in the changes of eczema-like changes in psoriasis vulgaris(blood-heat syndrome).[Methods]A total of 55 patients with psoriasis vulgaris who met the blood-heat syndrome of traditional Chinese medicine and attended the Department of Dermatology of the Air Force Center for Characteristic Medicine from September 2019 to August 2020 were collected.Patients were divided into psoriasis vulgaris with eczema-like changes group(trial group)and psoriasis vulgaris without eczema-like group(control group)according to whether they had eczema-like changes.There are 30 cases in the control group and 25 cases in the trial group.The general data of patients were recorded,including height,weight,age of onset,course of disease,inducement of onset,PASI score,itching degree and other indicators.ELISA was used to test the contents of TSLP,TLR2 and IgE in patients' serum.SPSS22.0 software package was used for data statistical analysis.Chi-square test was used for Count data.If the measurement data are normally distributed and the variance is uniform,the independent sample T test is used for comparison between the two groups,and the one-way variance analysis is used for comparison among multiple groups,which is described by the mean standard deviatio-n,and pearson is used to analyze the correlation between the two factors;Non-parametr-ic test was used for those with non-conforming distribution or uneven variance,and spearman was used as the correlation between the two factors,with P<0.05 as a signifi-cant statistical significance.[Result]1.the expression levels of TSLP and TLR2 in serum of trial group were higher than those of control group,and the difference was statistically significant(p<0.05);the expression of IgE in the trial group was higher than that in the control group,with no statistically significant difference(P=0.236>0.05).2.There was a difference in pruritus between the two groups(P=0.043<0.05).3.BMI and PASI in the trial group were lower than those in the control group,and there was no statistically significant difference between the two groups(P>0.05).4.There was a positive correlation between serum TLR2 and TSLP in the trial group(r=0.853,P=0.00<0.01);IgE was positively correlated with TLR2(r=0.670,P=0.00<0.01)and TLSP(r=0.675,P=0.00<0.01).5.There was a correlation between TSLP(AUC=0.718,P=0.006<0.01),TLR2(AUC=0.752,P=0.001<0.01)and the eczema-like psoriasia.[Conclusion]1.the expression of TSLP and TLR2 in serum of patients with eczema of psoriasis(blood-heat syndrome)is significantly higher than that of those without eczema.It suggests that TSLP and TLR2 are involved in the changes of eczema-like changes in the skin lesions of patients with psoriasis.2.The correlation between Body mass index,PASI were not significantly correla-ted with eczema-like changes in psoriasis.3.There is a significant correlation between pruritus symptoms of psoriasis and the eczema-like psoriasia.4.There is a correlation between TLR2,TSLP,IgE in patients with eczema-like change of psoriasis(blood-heat syndrome).TLR2-TSLP has predictive value in diagno-sis of eczema-like change of psoriasis.
