Recent studies have shown that vitamin D(Vit D)plays an important role in immune defense and pathogenesis of allergic diseases.However,the effect of Vit D levels on infant eczema is still unclear.TSLP and IgE play an important role in the occurrence and maintenance of allergic immune responses.Vit D deficiency and high levels of TSLP,IgE can increase the risk of allergic diseases.This study investigated the association of cord blood 25-hydroxyvitamin D(25(OH)D),TSLP and IgE levels with the incidence and severity of eczema in infants and analyzed other relevant factors affecting eczema in infants at 42 days.From October 2018 to October 2019,176 cases of umbilical cord blood were collected from healthy term newborns born in the department of obstetrics and gynecology of the Air Force Medical Center,PLA(Air Force General hospital,PLA);The levels 25(OH)D,TSLP and IgE of 176 cases were tested by Enzyme Linked Immunosorbent Assay(ELISA);The related information of 176 pairs of newborns and pregnant women and the history of allergic diseases in mothers are recorded in questionnaire(cord blood number),and then followed up at the end of 42 days to record whether the baby had eczema and severity.Experimental data were analyzed by using SPSS 20.0 software.The difference of Chi-square test/Chi-Square Goodness-of-Fit Test for 42 days infant eczema was analyzed by various factors.Comparison of 25(OH)D,TSLP and IgE levels in 42 days infant eczema group and no-eczema group;A statistical analysis of the relationship between the levels of cord blood TSLP,IgE and mother’s allergic history was conducted.According to EISA score,infant eczema was divided into mild,moderate and severe eczema group.The levels of cord blood 25(OH)D,TSLP and IgE were found to be homogeneous by kruskal-wallis test of nonparametric test statistical analysis.The levels mann-whitney test 25(OH)D,TSLP and IgE levels of cord blood in severe,moderate,mild eczema groups were analyzed by non-parametric test of nonparametric test.The comparison of measurement data is by t test and the correlation between indexes is analyzed by pearson linear correlation in25(OH)D,TSLP and IgE levels of cord blood.The median concentration of 25(OH)D in 176 neonates was 13.48ng/ml,with an average of 15.348±9.844 ng/m1,of which 98(55.68%)had25(OH)D deficiency,28(15.9%)had 25(OH)D deficiency,while only55(28.42%)had 25(OH)D level normal.176 infants,95 in eczema group and 81 in no-eczema group.This shows that the incidence of eczema in infants at 42 days was 53.98%.History of mother allergy,mixed feeding,unsupplemented cod-liver oil after birth and mother’s consumption of seafood during lactation have an effect on the occurrence of 42 days infant eczema,which can greatly increase the risk of eczema in offspring.Comparison of 25(OH)D,TSLP and IgE levels in eczema group and no-eczema group:(1)Cord blood 25(OH)D:10.285±5.659 ng/ml in eczema group;21.287±10.403 ng/ml in no-eczema group;The 25(OH)D level of cord blood in eczema group was significantly higher than that no-eczema group,which indicated that the difference was statistical(P<0.05).(2)Cord blood TSLP:71.714±47.462 ng/ml in eczema group;19.809±40.126 ng/ml in no-eczema group;The TSLP level of cord blood in eczema group was significantly higher than that no-eczema group,which indicated that the difference was statistically significant(P<0.05).(3)Cord blood IgE:66.902±70.795 ng/ml in eczema group;22.949±37.817 ng/ml in no-eczema group;The IgE level of cord blood in eczema group was significantly higher than that no-eczema group,which indicated that the difference was statistical(P<0.05).The TSLP and IgE levels of cord blood in being mother’s history of allergy was 60.123±52.188 ng/ml,60.492±70.206 ng/ml;The TSLP and IgE levels of cord blood in no-mother’s history of allergy was 32.015±45.482 ng/ml,28.908±39.042ng/ml;The TSLP and IgE levels of cord blood in being mother’s history of allergy were significantly higher than those of mother without history of allergy,which indicated that the difference was statistical(P<0.05).There was no statistically significant in cord blood 25(OH)D,TSLP and IgE levels in eczema severity(P<0.01;x2:27.522,20.805,9.977).(1)There was statistically significant in cord blood 25(OH)D levels of infant with severe,moderate and mild eczema groups had significantly lower than those no-eczema group(P<0.05);There was no statistically significant in25(OH)D levels of cord blood in infants with severe,moderate,mild eczema(P>0.05).(2)There was statistically significant in cord blood TSLP levels of infant with severe,moderate and mild eczema groups had significantly higher than those no-eczema group(P<0.05);There was no statistically significant in TSLP levels of cord blood in infants with severe,moderate,mild eczema(P>0.05).(3)There was statistically significant in cord blood IgE levels of infant with severe,moderate and mild eczema groups had significantly higher than those no-eczema group(P<0.05);There was no statistically significant in IgE levels of cord blood in infants with severe,moderate,mild eczema(P>0.05).Correlation analysis among25(OH)D,TSLP and IgE in eczema group:(1)There was no correlation between cord blood 25(OH)D level and cord blood TSLP and IgE level(r=0.3027;r=0.535,P>0.05).(2)There was positive correlation between cord blood TSLP level and cord blood IgE level(r=0.4674,P=0.0001).At 25(OH)D≤20 ng/ml in cord blood,mean of TSLP level was60.174±52.815,mean of IgE level was 59.847±68.07;At 25(OH)D>20ng/ml in cord blood,mean of TSLP level was 16.709±29.292 ng/ml,mean of IgE level was 13.479±16.807 ng/ml;The analysis showed that There was statistically significant in the IgE and TSLP levels was significantly higher at 25(OH)D deficient(P<0.05).This study showed that the incidence of eczema in infants at 42 days was 53.98%.History of mother allergy,mixed feeding,non-supplement of cod liver-oil after birth and mother’s consumption of seafood during lactation may be high risk factors for eczema.cord blood 25(OH)D,TSLP,IgE levels are associated with the occurrence of eczema in infants,and low levels of cord blood 25(OH)D lead to increased incidence of infant eczema.Thus,25(OH)D low level of cord blood is a risk factor for the occurrence of eczema in infants.There are no correlation between cord blood 25(OH)D,TSLP,IgE levels and eczema EASI scores and infant eczema severity.There was no correlation between cord blood 25(OH)D level and IgE,TSLP level,but There was correlation between TSLP and IgE level in cord blood.The TSLP and IgE levels in cord blood of infant with eczema increased significantly,which suggesting that these two cytokines may play an important role in eczema pathogenesis.
