A Clinical Study On The Treatment Of Chronic Pelvic Pain With PID Of Cold-coagulation And Blood Stasis Type By The Traditional Chinese Medicine Wentong Method

Objective:To observe the clinical effect of Hantongningtang and Huayuningkunye in treating chronic pelvic pain(CPP)caused by sequelae of pelvic inflammatory disease(SPID)differentiated as cold congealing and blood stasis syndrome.Totally 60 patients were included in the study.We aimed to explore the superiority retention enema treatment in treating CPP caused by PID,and to discover a better way to treat the disease.Methods:Sixty patients with CPP caused by SPID who consulted the doctor at Dongzhimen Hospital of BUCM from December 2018 to November 2019 were randomly divided into comprehensive medication group and oral medication group.Each group included 30 patients.The oral medication group was given hantongningtang orally.The comprehensive medication group was given hantongningtang orally and huayuningkunye by retention enema.The treatment lasted for 3 courses of treatment and each course of treatment lasted for 30 days.The treatment paused during menstruation.The associated symptoms,gynecological examination,pelvic pain(VAS score)and quality of daily life(SF-36 score)were observed before and after the treatment.SPSS 23.0 were used to conduct the statistics.P value less than 0.05 was considered to be statistically significant.Results:1)Age,duration of CPP,scores of symptoms,scores of gynecological examination,VAS score and SF-36 score were comparable between the two groups before the treatment.(p>0.05)2)After the treatment,the total efficiency rate of the comprehensive medication group was 86.7%,and 70.0%of oral medication group(p<0.05).The efficiency rate of symptoms were 93.3%(comprehensive medication group)and 60.0%(oral medication group)with p<0.05.The efficiency rate of gynecological examination were 86.7%(comprehensive medication group)and 63.3%(oral medication group)with p<0.05.The scores of symptoms,gynecological examination,VAS scores descended in both groups,and the SF-36 scores ascended(p<0.05).Compared with the oral medication group,the difference was statistically significant in the scores of symptoms,gynecological examination and VAS scores.To sum up,the therapeutic effect of the comprehensive medication group was significantly better than the oral medication group.Conclusion:1)Hantongningtang had effect on treating CPP,in the aspect of reducing the scores of symptoms and gynecological examination.Patients who were given both hantongningtang orally and huayuningkunye by retention enema had better effect.2)Retention enema treatment can enhance drug concentration,reduce gastrointestinal stimulation,avoid the first pass effect of liver and promote the absorption of inflammation.
