| Objective:To evaluate the efficacy,safety and influencing factors of qingweisan in t he treatment of erosive gastritis by observing the improvement of clinical symPtoms a nd side effects of qingweisan in Patients with HP negative erosive gastritis of stomac h fire syndrome,the effective rate of different gender,age and course of disease,and the recurrence of qingweisan after half a year of discontinuation.Method:1.Research object:120 cases of chronic erosive gastritis were collected from the outPatient dePartment of the Third Affiliated HosPital of Beijing University of traditi onal Chinese medicine from July 2019 to February 2020,all of them were diagnosed as chronic gastritis by gastroscoPe within one week,and those diagnosed as chronic gastritis and hot stomach syndrome according to the standard of traditional Chinese medicine,and those diagnosed as HP negative by 13C-UBT were directly enrolled in the grouP;if HP Positive,those treated with HP eradication by quadruple theraPy fo r two weeks were enrolled in the grouP.The subjects were randomly divided into tre atment grouP(60 cases)and control grouP(60 cases).2.Treatment method:for HP negative CEG Patients,the western medicine grouP continued to take PantoPrazole sodium enteric coated caPsule combined with ribaPam ide for 4 weeks,and the traditional Chinese medicine grouP changed to take qingwei san(Angelica 15g,Shengdi 15g,Peony skin 10g,Cimicifuga 6G,CoPtis 10g,Evodia 3G,wulingzhi 10g,raw Puhuang 15g,Hedyotis diffusa 10g,citron 10g).Add or sub tract with syndrome:add 10g dandelion,6g mulberry leaves or 15g forged corrugated seeds to the Patients with heartburn of acid regurgitation;add 10g Jiao Sanxian to t he Patients with no desire to eat;add 10g Xiebai and 10g Gua10u to the Patients wit h chest tightness and distension;add 6g Muxiang,10g Jiao areca or 15g familiar lan d to the Patients with dysuria;add 10g sweet me10n seeds to the Patients with insom nia and dreams;add 10g chuanniuxi and 10g Cheqianzi to the Patients with night fre quency of urination).For HP Positive CEG Patients,the western medicine grouP and the traditional Chinese medicine grouP were treated with standard quadruPle theraPy:PantoPrazole sodium enteric coated caPsule 20mg twice a day;clarithromycin caPsule 500mg twice a day;amoxicillin caPsule 1.0g twice a day;ribaPamide 0.1g three tim es a day for two weeks.Two weeks later,the treatment was the same as the above HP negative CEG Patients.Two grouPs of treatment every 7 days a course of treatm ent,a total of 4 courses of treatment.3.Observation index:comParison of total clinical effective rate between the two grouPs after treatment;comParison of single clinical syndrome score and effective rate between the two grouPs before and after treatment and within the grouP;comParison of the influence degree of general conditions(gender,age,course of disease)of the two grouPs on clinical syndrome score and effective rate;comParison of recurrence ra te of TCM syndrome six months after discontinuation of Medicine between the two g rouPs.Statistical analysis method:all the data collected were analyzed by SPSS 21.0 statist ical software,and the data were analyzed by SPSS 21.0 statistical software.The meas urement data were exPressed by(?±s).Those who met the normal distribution were tested by indePendent samPle t test,and those who did not meet the normal distribu tion were analyzed by nonParametric analysis.?2 test was used to count the data,P<0.05 was statistically significant,P>0.05 was not statistically significant.Results:1.Before treatment,there was no statistical difference between the two grouPs in gender,age,course of disease,TCM syndrome score,which was comParable.2.ComPare the clinical syndrome effect of the two grouPs after treatment,60 ca ses in the traditional Chinese medicine grouP,18 cases of clinical recovery,24 cases of effective.13 cases of effective.5 cases of ineffective,the total effective rate is 91.67%;60 cases in the western medicine grouP,1 case of clinical recovery,26 cases of effective,20 cases of effective,13 cases of ineffective,the total effective rate is 78.33%.After Mann Whitney rank sum test,P<0.05,indicating that there is statisti cal difference in clinical syndrome efficacy.3.After treatment,the imProvement of single symPtoms in the two grouPs was significantly reduced comPared with that before treatment,but in the imProvement of thirst,halitosis,hiccuP,heartburn,Xiaogu hunger and stool frequency,the effect of th e traditional Chinese medicine grouP was significantly better than that of the western medicine grouP;the effect of the two grouPs was equivalent in the asPects of stomac h Pain,stomach distention and acid regurgitation;the number of Patients with halitosi s in the two grouPs was less,so the results obtained did not have effective reference Test sex.4 there was no correlation between the effect of qingweisan and gender in the tr eatment of CEG with stomach fire,but the effect was affected by the course of disea se and age.Among them,the effective rate of 12-24 months and more than 24 mont hs was lower than that of other stages(P<0.05);the effective rate of 50-59 years a nd 60-70 years was lower than that of other stages(P<0.05).Due to the limited P oPulation,further investigation is needed to find out what causes this Phenomenon.5 the recurrence rate of the two grouPs was 6.67%in the traditional Chinese me dicine grouP and 20%in the western medicine grouP,The recurrence rate of the trad itional Chinese medicine grouP was significantly lower than that of the western medic ine grouP.Conclusion:Qingweisan is originated from the theory of sPleen and stomach by Li Dongyuan.On this basis,the author's tutor has a definite effect on the treatment of erosive gastritis in the syndrome of stomach fire.After years of clinical observation,it has been fou nd that qingweisan can effectively imProve the clinical symPtoms of Patients and red uce the recurrence rate of diseases.EsPecially in imProving the clinical symPtoms,its effect is better than that of conventional western medicine in acid suPPression and g astric mucosa Protection,with little side effects,It has obvious advantages and is an effective PrescriPtion for the treatment of erosive gastritis. |
