Abstract/Summary:
Purpose:To comprehensively understand the quality of life of patients with cerebral ischemic stroke and the need for transitional care after returning family(or community)from hospital,and to study the relationship between the need for transitional care model and the quality of life of patients with cerebral ischemic stroke and the related effect factors.In order to arouse more attention to the transitional care needs and quality of life of patients with cerebral ischemic stroke,to comprehensively understand the individualized and transitional care needs of patients after discharge,and to strengthen transitional care after discharge of cerebral ischemic stroke patients.The secondary prevention effect can improve the compliance of patients with cerebral ischemic stroke,improve the patients’ quality of life,and provide a basis for the development of an individualized stroke rehabilitation system.Material and method:A convenience sampling method was used to select 218 patients with cerebral ischemic stroke who were hospitalized in the Department of Neurology at a top three hospital in Liaoning Province from January 2019 to June 2019.This study used questionnaire survey methods,includes general demographic data questionnaires,transitional care demand questionnaires for cerebral ischemic stroke patients,and concise health status questionnaires(SF-36),etc.,to study subjects’ transitional care needs and quality of life conduct an investigation.The results of the questionnaire were conducted in a double entry mode.SPSS 20.0 was used for statistical analysis of the data,and the composition ratio,mean,and standard deviation were used to describe the demographics,transitional care needs,and quality of life of patients with cerebral ischemic stroke.Univariate analysis of variance(ANOVA)and chi-square test were used to compare the transitional care needs and quality of life in different demographics.Pearson / Spearman correlation analysis and multiple stepwise linear regression were used to study the transitional care needs and patients of cerebral ischemic stroke patients.Relationship between quality of life.Results: 1.The average age of the study subjects was 60.49 ± 10.7,of which the majority were patients>60 years old,a total of 125 cases,accounting for 57.34%.Among the choices of follow-up nursing follow-up methods,the top three with the strongest demand were 112 patients(51.38%)for telephone follow-up,76 patients(34.86%)for home visits,and 70(32.11%)for the QQ group or We Chat communication group.2.The results of the survey of the transitional care needs of the study subjects showed that the top five items of the most transitional care needs of the study subjects were: stroke knowledge(4.51 ± 0.7),safe medication(4.39 ± 0.8),and rehabilitation methods(4.39 ± 0.8),psychological emotion control and regulation(4.25 ± 0.9),and blood pressure self-monitoring(4.22 ± 0.9),and a higher score indicates a stronger need for transitional care.Age,marital status,occupational status,and medical payment methods have statistical differences in the scores of transitional care needs of patients with cerebral ischemic stroke(p<0.05).Patients of different ages have self-monitoring of blood pressure,psychological and emotional control and regulation,There were statistically significant differences in dietary guidance,home nursing skills,social support,blood glucose and lipid control and monitoring,psychological counseling,and family environment reform scores(p<0.05).The 50~59-year-old group excluded the need for psychological counseling to extend nursing care.In addition to the scores,the remaining scores were significantly higher than those of the other two groups.For the need for psychological counseling and transitional care,the highest score was in the group older than 60 years.The results of Pearson correlation analysis showed that age was positively related to the needs of home nursing skills,social support,blood sugar control,and home environment modification(p<0.05).3.The higher average score of SF-36 in patients with cerebral ischemic stroke is mental health(64.14 ± 4.60),vitality(61.55 ± 4.81),affective disorder(54.95 ± 5.57),and the SF-36 scale scoring standard indicates the score The higher,the better.The highest score was for mental health,and the lowest score was for physiological function(37.55 ± 5.81).The quality of life of patients is affected by age,marital status,professional identity,education,family income per capita,medical payment methods,stroke duration,incidence of illness,living style,family environment reform,stroke knowledge,safe medication,rehabilitation methods,Blood pressure self-monitoring,psychological and emotional control and regulation,dietary guidance,home nursing skills,social support,blood glucose control and monitoring,blood lipid control and monitoring,and psychological grooming were significantly different(p<0.05).4.The results of correlation analysis between continuity nursing needs and quality of life show that rehabilitation exercise methods are significantly positively correlated with patients’ physical pain(BP),affective disorder(RE)and safe medication,blood pressure self-monitoring,psychological and emotional control and regulation,Dietary guidance,home care skills,social support,blood glucose and lipid monitoring were significantly positively correlated.Home nursing skills were significantly negatively related to physiological function(RP),and family environment reform was significantly negatively related to physiological function(PF).Age,education,living style,medical payment method,stroke duration,family monthly income,incidence Frequency and continuous nursing needs are independent risk factors affecting quality of life,p<0.05,the difference is statistically significant.The result of multiple linear regression analysis of quality of life in patients with cerebral ischemic stroke is R2 is 0.500.Conclusion: 1.Transitional care model of patients with cerebral ischemic stroke are at a high level.Age,marital status,occupational status,and medical payment methods have an impact on the transitional care model of the study subjects.2.The quality of life of patients with cerebral ischemic stroke is relatively low.The quality of life of the study subjects is affected by age,marital status,professional identity,education,family income per capita,medical payment methods,stroke duration,number of incidences,living style,and family environment reform,stroke knowledge,safe medication,rehabilitation exercise methods,blood pressure self-monitoring,psychological and emotional control and regulation,dietary guidance,home care skills,social support,blood glucose control and monitoring,blood lipid control and monitoring,and psychological counseling.3.There is a significant correlation between transitional care model and quality of life in patients with cerebral ischemic stroke.4.The clinical medical staff can fully understand the needs of transitional care after cerebral ischemic stroke patients return to home or community after discharge from hospital,and their quality of life status,formulate a comprehensive nursing intervention and health education system,and improve the quality of life of the research subjects.
