Objectives:To describe the status of the demographic characteristics and the related information of stroke disease. To analyze the effect of transitional care model on brain stroke patients at rehabilitation stage on activities of daily living (barhtel index, BI), Fugl-meyer assessment and quality of life, definite the effect of implementalion of transitional care model on brain stroke patients at rehabilitation stage. Then to provide references for hospitals to build the brain stroke rehabilitation transitional care system.Methods:Using convenience sampling methods 100 patients, new episodes of ischemic stroke patients in department of neurology of X hospital were included in the study. The subjects were randomly divided into the intervention group 55 cases and the control group 55 cases. Using SPSS17.0 statistical software as Statistical Methods, the demographic characteristics and the related information of stroke disease of the subjects was described by percentage, independent samples t-test was used to analysis differences of the subjects on BI index, Fugl-Meyer Movement and quality of life.Results:(1) BI index and BI index comparison:BI index in the intervention group (91.55±16.607) and in the control group (82.64±21.470) had statistical significance (P<0.05). BI index increasing in the intervention group (30.91±18.436) and in the control group (25.45±14.184) had not statistical significance (P>0.05).(2) Fugl-Meyer assessment and Fugl-Meyer assessment comparison:Fugl-Meyer assessment in the intervention group (92.22±13.148) and in the control group (85.62± 21.330) had not statistical significance (P>0.05). Upper extremity motor function in the intervention group (60.33±9.961) and in the control group (55.04±15.870) had statistical significance (P<0.05). Lower extremity motor function in the intervention group (31.89±3.938) and in the control group (30.55±5.931) had not statistical significance (P>0.05). Fugl-Meyer assessment increasing in the intervention group (23.25±17.711) and in the control group (15.47±13.420) had statistical significance (P<0.05). Upper extremity motor function increasing in the intervention group (16.67±12.757) and in the control group (10.58±10.001) had statistical significance (P<0.05). Lower extremity motor function increasing in the intervention group (6.58±5.921) and in the control group (5.04±5.062) had not statistical significance (P>0.05).(3) Quality of life and Quality of life increasing comparison of brain stroke: Quality of life in the intervention group (52.58±10.645) and in the control group (45.69±12.640) had statistical significance (P<0.05). Each other the 12 domains of the SS-QoL comparison in the intervention group and in the control group:Self-care ability, Mobility, Upper extremity function, Social Roles and Energy had statistical significance (P<0.01); Working ability, Thinking, Family Roles, Personality and Mood had statistical significance (P<0.05); Only Language ability and the Vision had not statistical significance (P>0.05). Quality of life increasing in the intervention group (20.65±9.264) and in the control group (16.35±8.843) had statistical significance (P<0.05). Each other the 12 domains of the SS-QoL increasing comparison in the intervention group and in the control group:Self-care ability increasing and Upper extremity function increasing had statistical significance (P<0.01); Mobility increasing, Thinking increasing and Energy increasing had statistical significance (P<0.05); Language ability increasing, Vision increasing, Working ability increasing, Family Roles increasing, Social Roles increasing, Personality increasing and Mood increasing had not statistical significance (P>0.05).Conclusions:(1)Transitional care model could improve Fugl-Meyer assessment and the quality of life of patients with brain stroke at rehabilitation stage.(2)Transitional care model could improve daily living of patients with brain stroke at rehabilitation stage.
