ObjectivesTo explore the effects of transitional care programme on kidney transplant patients after discharge,and evaluate the results from patient’s self-efficacy, self-mangement ability,quality of life and pulmonary complications rate.Methods85kidney transplant recipients after discharge were selected by using purposive sampling method,and were assigned into intervention group and control group by sortition randomization method,43case in the intervention group,42case in the control group.Control group received standard nursing and discharge guidance,while intervention group received the same service and transitional care programme for three months. Data were collected at two times,namely pre-post intervention,after intervention,with questionnaire of demographic data of kidney transplant recipients, Self-efficacy of Chronic Disease,Renal Transplant Recipients’ self-management scale,Quality of life of Renal Transplant Recipients and the rate of pulmonary complications to evaluate the intervention effect.SPSS17.0was used by data analysis, the descriptive statistics,chi-square Text and T test statistic methods were used. Results1The score of self-efficacy, self-mangement, quality of life of kidney transplant recipients in the intervention group were higher than control group after intervention, the difference was statistically significant (P<0.01). The rate of pulmonary complications of kidney transplant recipients in the intervention group was lower than control group after intervention, the difference was statistically significant (P <0.01).2The score of self-efficacy,self-mangement and quality of life of kidney transplant recipients in the intervention group after intervention were higher than the score before intervention,the difference was statistically significant (P<0.01). The rate of pulmonary complications of kidney transplant recipients in the intervention group were lower than the score before intervention,the difference was statistically significant (P<0.01).3The score of self-efficacy, self-mangement,quality of life and the rate of pulmonary complications of kidney transplant recipients in the control group after intervention were not different from the score before intervention,there was not statistically significant (P>0.05).,.ConclusionsTransitional care programme can improve self-efficacy,ability of self-mangement and quality of life of kidney transplant recipients after discharge and decrease the rate of pulmonary complications. So the transitional care programme can be used and expanded widely.
