Objectives:1.To survey of parents' transitional care of needs for the Children with type 1 diabetes and ways and contents.2.Investigation the influence of transitional nursing for Quality of Life of Children with type 1 diabetes.Methods:Experimental study and front and back control.From January to March in 2017,with Inclusion and discharge criteria extracted of the 110 cases from discharged children with type 1 diabetes and their parents in two First-class Hospital at grade 3 with the method of random sampling through self-designed questionnaire.With Inclusion and discharge criteria,a total of 44 5?12 years old children with type 1 diabetes and their parents in the one Hospital of Chang Sha from January to June 2017 were selected as control group and carried out routine diabetes care,the control group was divided into two groups:5?7 years old and 8?12 years old group.A total of 44 5?12 years old children with type 1 diabetes and their parents in the one Hospital of Chang Sha from July to December 2017 were selected as experiment group and carried out transitional nursing,the experimental group was divided into two groups:5?7 years old and 8?12 years old group.PedsQLTM 4.0 scale was used to assess the quality of life for children with type 1 diabetes of the time when they were left hospital and after discharged 1st?3rd month respectively,and two groups of children were evaluated for their quality of life(Four aspects of physiology,emotion,social interaction and role)at different stages.And parents of children with type 1 diabetes of experiment group were graded according to the knowledge questionnaire of parents at the time of discharge and 3 months from the hospital.This experiment used ?2 test,Pared-samples T test,T-test,Repeated measures analysis of variance,Mann-Whitney U test to analyze the data,through software SPSS 22.0.Results:1.Through surveyed 110 children who were about to leave hospital and their parents,the proportion of parents who wanted to receive transitional care were 95.45%.In 20 items of transitional care needs content,parents had demand for blood glucose monitoring guidance,accounting for 100%,the demand for the use of insulin pen and psychological counseling and severe emergency treatment were 97.27%and 95.45%,95.45%respectively.In addition,parents of children also have some needs for parent child interaction maintenance,follow-up management,dietary guidance,consultation and guidance.The top 3 patients' demand for continuing care is telephone,WeChat account for 89.09%,computer network communication for 63.64%,and outpatient visits for 78.18%,among them,family visits are the lowest in need(45.45%).2.In the implementation of transitional nursing mode,(1)The total score of quality of life of 5?7 years old experimental groups in the time of discharged from hospital,discharged 1st?3rd month was 15.01 ±7.27?18.25±5.37?20.35± 2.45;the control group was 14.98±6.53?16.45 ± 6.43?17.87± 4.01.These two groups increased gradually in the score of quality of life,the score of the experimental group was higher than that of the control group and had statistical differences(P<0.05).The total score of quality of life of 8?12 years old experimental groups in the time of discharged from hospital,discharged 1st?3rd month was 14.05±6.23?16.25±3.23?19.35±1.23;the control group was 13.47±6.53?15.45±2.45?16.87±2.21.These two groups increased gradually in the score of quality of life,the score of the experimental group was higher than that of the control group and had statistical differences(P<0.05).(2)The score of physiology of 5?7 years old experimental groups in the time of discharged from hospital,discharged 1 st?3rd month was 21.98±8.90?24.98±7.64?28.98±3.24;the control group was 21.34±8.17?23.31±6.35?25.34±8.52.These two groups increased gradually in the score of quality of life,the score of the experimental group was higher than that of the control group and had statistical differences(P<0.05).The score of physiology of 8?12 years old experimental groups in the time of discharged from hospital,discharged 1st?3rd month was 21.65±7.90?25.56±6.34?28.45±3.01;the control group was 21.54±5.17?23.05±5.23?25.02±4.52.These two groups increased gradually in the score of quality of life,the score of the experimental group was higher than that of the control group and had statistical differences(P<0.05);(3)The score of emotion of 5?7 years old experimental groups in the time of discharged from hospital,discharged 1st?3rd month was 10.47±6.55?13.47±5.37?16.47±3.25;the control group was 11.36±6.53?13.45±5.76?15.36±4.56.These two groups increased gradually in the score of quality of life,the score of the experimental group was higher than that of the control group and had statistical differences(P<0.05).The score of emotion of 8?12 years old experimental groups in the time of discharged from hospital,discharged 1st?3rd month was 12.34±5.56?14.47±3.37?18.47±1.25;the control group was 13.04±7.53?14.45±5.43?16.36±3.87.These two groups increased gradually in the score of quality of life,the score of the experimental group was higher than that of the control group and had statistical differences(P<0.05);(4)The score of social interaction of 5?7 years old experimental groups in the time of discharged from hospital,discharged 1st?3rd month was 13.23±6.17?14.47±3.46?17.47±2.25;the control group was 13.35±6.45?13.94±5.08?15.36±4.75.These two groups increased gradually in the score of quality of life,the score of the experimental group was higher than that of the control group and had statistical differences(P<0.05).The score of social interaction of 8?12 years old experimental groups in the time of discharged from hospital,discharged 1st?3rd month was 11.47±7.15?16.34±3.46?18.47±1.04;the control group was 11.35±6.45?15.78±3.21?17.36±2.43.These two groups increased gradually in the score of quality of life,the score of the experimental group was higher than that of the control group and had statistical differences(P<0.05);(5)The score of role of 5?7 years old experimental groups in the time of discharged from hospital,discharged 1st?3rd month was 14.33±4.62?15.89±4.56?17.47±1.25;the control group was 13.87±4.67?14.55±4.37?15.36±4.53.These two groups increased gradually in the score of quality of life,the score of the experimental group was higher than that of the control group and had statistical differences(P<0.05).The score of role interaction of 8?12 years old experimental groups in the time of discharged from hospital,discharged 1st?3rd month was 12.33±4.62?15.65±3.32?18.42±1.22;the control group was 12.35±3.67?15.21±4.37?16.98±3.11.These two groups increased gradually in the score of quality of life,the score of the experimental group was higher than that of the control group and had statistical differences(P<0.05);(6)In addition,the scores of parents' care knowledge questionnaire increased gradually in the experimental group discharged 1st(7.75±2.71)and 3rd(11.98±1.65)had statistical differences(P<0.05).Conclusion:1.The parents of children with type 1 diabetes have the transitional nursing needs for 95.45%.2.The implementation of transitional nursing improves the quality of life of children with type 1 diabetes,and strengthens parents' knowledge of diabetes which is benefit for the clinical work.3.This study provides data basis and reference basis for the development of transitional nursing for children with type 1 diabetes.
