| Objective:Through a retrospective study of 509 cases hospitalized in the dermatology department of the first affiliated hospital in Liaoning University of Traditional Chinese Medicine from January 2018 to December 2019 and managed by the clinical pathway of traditional Chinese medicine for eczema,a curative effect of this pathway was analyzed and explored,and clinical basis was provided for its further improvement and promotion.Materials and Methods: 509 cases managed by the clinical pathway of TCM for eczema in the dermatology department of the first affiliated hospital in Liaoning University of Traditional Chinese Medicine from January 2018 to December 2019 were collected and sorted out by means of electronic medical record system.The general data(age,gender)of patients,course of disease,previous medical history,allergy history,hospitalization days,skin lesion characteristics,treatment methods and related laboratory examinations were statistically and retrospectively analyzed.Results:1.Among 605 patients included in the clinical pathway of TCM for eczema in this study,191 patients have variation,the variation rate is l31.57%,96 patients have exfoliation,and the exfoliation rate is 15.87%.2.Among 509 valid cases in this study,the ratio of men to women was about 0.89:1,with an average age of 61.22±14.350 years old.The shortest course of disease is 1 day and the longest is 50 years.The course of relapse is within 10 years.The average length of hospital stay is11.18±2.330 days.3.Onset seasons: The onset occurs in all seasons of the year.The first onset is usually in spring and summer.4.Predisposing factor: 88 cases(17.29%)in this study indicated that there was a clear cause of the disease.5.Past history of disease: In this study,291 cases(57.17%)of the patients had at least one disease histories,among which,hypertension and diabetes are relatively high.6.Allergic history: In the study,148 cases(69.12%)of the patients had at least one or moreallergic history of drugs or foods.7.Features of skin lesions: In line with the characteristics of eczema pleomorphism,papules,erythema scratches and scabs are the most frequent,with 507 cases,504 cases,387 cases and369 cases respectively.8.Laboratory tests: Among the 509 cases materials,the positive rate of serum total Ig E was206 cases(51.08%),the count of peripheral blood eosinophils exceeded the reference range in115 cases(22.59%),112 cases(22%)had fasting blood glucose higher than the reference range,109 cases had abnormal liver function and 66 cases had abnormal renal function.The positive rate in 392 food intolerance tests was 97.45% and three foods with the highest positive rate were egg white and yolk,wheat and soybean;total positive rate of 408 inhaled allergens 62.01%;in 499 immunomomotic tests,480 cases(96.19%)had immune dysfunction,and 220 cases(73.58%)of 299 blood lipid tests had blood lipid abnormalities.9.TCM Syndrome Types:413 cases were immersion of dampness-heat in skin,55 cases were dampness retention due to deficiency of spleen,41 cases were blood vacuity win-dryness.10.According to the evaluation criteria of curative effect,56 cases were cured,424 cases were markedly effective,27 cases were improved and 2 cases were ineffective,with a total effective rate of 99.61%.Conclusion:1.Practice has proved that the clinical pathway of TCM for eczema in our hospital,which is mainly based on traditional Chinese medicine therapy and assisted by western medicine in basic therapy,has significant advantages in relieving itching,improving skin lesions and symptoms,and is helpful to standardize diagnosis and treatment. |
