ObjectiveThrough carring out the retrospective analysis of in-patients of Herpes Zoster from 2006-2008,this research summarizes the former clinical practical experiences,provides basis for the design of the clinical pathway for Herpes Zoster of TCM and contrastive data for the prospective effect evaluation of clinical pathway.MethodsThis research investigates the related situations of in-patients of Herpes Zoster in the department of dermatology of Guangdong Provincial Hospital of TCM from January 2006 to December 2008,by acquiring information from medical records and making comparisons between years.The main evaluating indicators include:①general clinical materials②hospitalization days③hospitalization costs④relevance between hospitalization days and hospitalization costs⑤treatments and medications⑥complications⑦outcome of disease.ResultsThis research has collected 175 cases totally,among which there are 63 cases in 2006,66 cases in 2007,46 cases in 2008.There is no obvious difference between groups on common clinical materials.The average length of stay has a obvious trend of escalation(P=0.031),from 9.68±4.743 to 11.65±4.453 days. The total cost increases 517.966 Yuan per day.The average hospitalization cost rises from 5467.36±2735.76 Yuan to 6967.46±2656.72 Yuan,which has a obvious trend of escalation(P=0.008).The expenses of Chinese herbal medicine, Chinese patent medicine,the western medicine,the radiation and the chemical examinations have increased.On the other hand,the examination expense and the treatment expense have decreased.The main therapy is based on the cooperation of Chinese and Western medicine.The traditional Chinese medicine includes Chinese herbal and patent medicine.The western medicine is comprised of antiviral drug,anti-inflammatory drug,anodyne,nutrient for nerve, immunoregulant,glucocorticoid and antibiotic.Besides,external treatment and nursing care,as well as the characteristic therapy,play an important part in the whole therapy.The formation rate of complications is 21.7%.The complications mainly include conjunctivitis and bacterial infection of lesion. The recovery rate is 42.3%.53.1%cases have marked effect.4.6%cases are improved while discharging.ConclusionsBoth the average hospitalization days and hospitalization costs have increasing trend in 2006-2008.Although the clinical curative effect improves, we still have to improve every link of the clinical pathway such as the therapeutic medications and the examinations.It' s necessary for the clinical pathway not only to show the characteristics of Chinese medicine and the cooperation of Chinese and Western medicine,but also to perfect the formulation and management of the clinical pathway.Based on the quality of medical service,the clinical pathway could hopefully play a vital role in reducing the days of hospital stay and the hospital expense,as well as enhancing the clinical curative effect.This research also provides the essential basis for the further design and perfection of the clinical pathway for Herpes Zoster of TCM.
