Meta-analysis Of The Curative Effect Of Chinese Herbal Decoction In The Treatment Of Hypercoagulable State Of Lung Cancer And Analysis Of The Drug Rule

Purpose:Through the literature analysis,the efficacy of traditional Chinese medicine decoction in the treatment of hypercoagulable state of lung cancer was evaluated,and its medication law was summarized and analyzed,so as to provide evidence-based medical evidence for the clinical application of traditional Chinese medicine in the treatment of hypercoagulable state of lung cancer.Material and method:The inclusion criteria of the literatures were established,and the literatures related to the randomizzed controlled clinical trials of Chinese traditional medicine decoction in the treatment of hypercoagulability of lung cancer from January 2010 to February 2020 were searched by computer from CNKI,VIP,wanfang database,and PubMed database.The risk of bias was assessed according to the risk assessment tool of bias of Corchrane collaboration,and the Meta analysis was carried out with RevMan5.3 software.The prescription of Chinese medicine decoction in the literature was summarized to explore the law of medication in the treatment of hypercoagulability of lung cancer.Results:1.Literature retrieval results:according to the strategy developed by the author,13 RCT studies on the treatment of hypercoagulable lung cancer with traditional Chinese medicine decoction were finally obtained,with a total of 1018 patients,510 in the treatment group and 508 in the control group.2.Improvement of platelet(PLT):After treatment,the amount of platelet effect MD=36.67,95%CI:[-48.92,-24.42],Z=5.87,P<0.00001.The rhombus in the forest map was located to the left of the midline,and the platelet value in the treatment group was lower than that in the control group,with statistically significant difference.3.Improvement of platelet(PLT):After treatment,the D-D effect amount SMD=2.54,95%CI:[-3.63,-1.44],Z=4.54,P<0.00001.The rhombus in the forest map was locat:ed to the left of the midline.The d-d value in the treatment group was lower than that in the control group,and the difference was statistically significant.4.Improvement of FIB:After treatment,the effect amount of FIB MD=1.31,9S%CI:[-1.67,-0.95],Z=7.16,P<0.00001.The rhombus in the forest map was located to the left of the midline,and the value of FIB in the treatment group was lower than that in the control group,with statistically significant difference.5.Improvement of prothrombin time(PT)and activated partial thrombin activity time(APTT):After treatment,the PT effect amount MD=0.81,95%CI:[-0.42,2.03],Z=1.29,P=0.20>0.05,the APTT effect amount MD=1.68,95%CI:[-0.88,4.24],2=1.28,P=0.20>0.05,the rhombus in the forest map crossed the central invalid line,and there was no diference in the PT and APTT values between the control group and the treatment group after treatment.6.Improvement of quality of line:After treatment,the effect amount of KPS score MD=4.31,95%CI:[2.45,6.16],Z=4.56,P<0.00001.The rhombus in the forest map was located to the right of the midline.The KPS scare in the treatment group was higher than that in the control group,and the difference was statistically significa nt.7.Law of medication:the drug with the highest medication frequency in the treatment of lung cancer with Chinese medicine decoction was angelica(11 times),followed by safflower(S times),astragalus(7 times),red peony root(7 times),chuanxiong(7 times)and peach kernel(7 times).In the efficacy category of traditional Chinese medicine,the most frequently used drugs were blood-activating and blood-stasis removig drugs(41.93%),followed by those to replenish deficiency drugs(24.51%),phlegm-relieving and antiasthmatic drugs(9.67%),heat-clearing drugs(7.74%),and qi regulating drugs(5.16%).Conclusion:1.Compared with conventional western medicine treatment,the traditional Chinese medicine medicinal broth in treatment of lung cancer high coagulation state in platelets,D-dimer,fibrinogen,KPS score on all have advantages,the prothrombin time,activated partial no difference on blood coagulation time live enzymes,shows that Chinese medicine medicinal broth can reduce the patient’s platelets,D-dimer,fibrinogen,improve high coagulation state,improve the quality of survival,but the prothrombin time,activated partial clotting time live enzymes had no obvious effect.2.Among the prescriptions of traditional Chinese medicine decoction for the treatment of high coagulation of lung cancer,the most frequently used drugs are angelica,followed by safflower astragalus,red peony root,chuanxiong and peach seed,etc.The most frequently used drugs are blood-activating and blood-stasis rernoving drugs,which provide reference for clinical application.
