| Purpose:Heat sensitive moxibustion is a new type of moxibustion therapy,which has the advantages of simple operation,safety and noninvasive,convenient application and wide indications.With the development of research in the field of moxibustion therapy,the number of literatures is increasing rapidly,and the types of literatures are complex.The purpose of this study is to use bibliometric method to sort out the Chinese literature in the related fields of "moxibustion with heat sensitivity" in CNKI,Wanfang and VIP databases,and to summarize and analyze the annual publication,journal source distribution,fund support,distribution of doctoral dissertation awarding units,highly cited literature,authors,research institutions and key words of 1305 literatures In order to provide a reference for the research and development of moxibustion,this paper discusses the hot spots and frontiers related to this field,and evaluates the efficacy and safety of Moxibustion in the treatment of hot spots in clinical research.Material and method:This paper is divided into two parts according to the purpose of the study.The first part is the bibliometric analysis of relevant literature in the field of moxibustion.Specific methods:(1)data source: retrieve the Chinese related literature with the theme of "thermal moxibustion" from January 1,2006 to November 1,2019 in CNKI,Wanfang and VIP databases.(2)By using bibliometric method,the annual publication of relevant literature in the field of thermal moxibustion,the distribution of included literature in major journals,the distribution of funds and degree awarding units,and the analysis of highly cited literature(journals and doctoral theses)were analyzed.(3)Cite Space software is used to analyze the author cooperation network,research institutions and their geographical distribution and keyword co-occurrence in the field of moxibustion research;the sudden test and analysis of core authors and relevant research institutions are carried out,and the scientific knowledge map is drawn respectively,so as to more intuitively present the core authors,research institutions and hot spots and frontiers related to the field of moxibustion research.(4)VOSviewer software was used to map the relationship between authors and keywords in the research field and the keyword density map.The second part is to use systematic evaluation and meta-analysis to evaluate the clinical efficacy and safety of moxibustion on the hot spot disease-"lumbar disc herniation" analyzed in the first part.Specific methods:(1)data sources: in CNKI,Wanfang,VIP,Chinese biomedical literature journal,Pub Med,Springer database,referring to the authoritative diagnostic standards of lumbar disc herniation in the "standard of diagnosis and curative effect of TCM disease syndromes" issued by the administration of traditional Chinese medicine,the inclusion criteria and exclusion criteria of literature were formulated,From January 1,2008 to November 1,2019,the randomized controlled and simple randomized controlled trials of moxibustion therapy for lumbar disc herniation in the observation group were searched.(2)Literature evaluation: two researchers independently evaluate the basic characteristics and methodology of these studies.(3)In this study,Revman 5.3 software was used for meta-analysis of therapeutic effects(clinical efficacy,clinical cure rate,VAS score,m-joa score,plasma IL-6level,adverse reactions,follow-up recurrence rate).Results:1.Since the development of thermal moxibustion in 2006,the number of published literatures in related research fields of thermal moxibustion has been increasing year by year;the included literatures are mainly Journal Papers(accounting for 91.88% of the total),which are distributed in 186 journals;in 2010,there were 106 doctoral papers published in related fields by 2019.Guangzhou University of traditional Chinese medicine(31 papers)is the most awarded doctoral degree in the field of moxibustion research.25.52% of the total data are supported by fund projects,among which 71.78% are supported by national fund projects.There are 8 articles with cited frequency ? 50 times in periodical literature,2 articles with cited frequency ? 100 times in this field,and 2 articles with cited frequency ? 20 times in doctoral dissertation.2.There are 322 authors in this study,and Professor Chen Rixin is the high-yield author.The research team of Jiangxi University of traditional Chinese medicine and its affiliated hospital,represented by him,has made the greatest contribution.Many stable research teams have been formed in the field of moxibustion related research,but there is less cooperation among the teams.The main form of author cooperation network in this field is inter agency cooperation.3.In recent years,Xie Dingyi,Liu Fushui,Huang Xianbao and other researchers are active in the field of moxibustion.4.The institutions included in this study are distributed in 30 regions,and their geographical distribution is mainly concentrated in East China,South China and central China.Jiangxi University of traditional Chinese medicine and its affiliated hospital are the main research institutions in the field of thermal moxibustion,most of which are closely related to each other,and each institution in different regions is relatively scattered.The multi-center cooperation among research institutions in the field of thermal moxibustion still needs to be strengthened.5.In recent years,the leading hot research institutions in this field are affiliated hospital of Guangzhou University of traditional Chinese medicine,Affiliated Hospital of Gannan Medical College,Chengdu University of traditional Chinese medicine,Shandong University of traditional Chinese medicine and Xinjiang Medical University.6.The research focuses of the diseases related to the heat sensitive moxibustion therapy are mainly lumbar disc herniation,knee osteoarthritis,periarthritis of shoulder,cervical spondylosis of vertebral artery type,primary dysmenorrhea,peripheral facial paralysis and bronchial asthma.7.Meta analysis included 29 articles,a total of 3009 cases of lumbar disc herniation,including 1509 cases in the observation group and 1500 cases in the control group.8.The results of meta analysis of clinical effective rate showed that compared with the control group,[RR = 1.13,95% CI(1.08,1.17),z = 5.91,(P < 0.00001)].The observation group is better than the control group in improving the total effective rate of patients.9.The total results of meta analysis of clinical cure rate showed that compared with the control group,[RR = 1.66,95% CI(1.48,1.86),z = 8.30,(P < 0.00001)],the observation group was significantly better than the control group in improving the cure rate of patients.10.Meta analysis of m-joa score showed that compared with the control group,[SMD =-0.16,95% CI(-0.45,0.13),z = 1.06,(P = 0.29)],the observation group was better than the control group in improving the clinical symptoms of lumbar disc herniation.11.Meta analysis of VAS score showed that compared with the control group,[MD =-0.19,95% CI(-0.57,0.19),z = 0.98,(P = 0.33)],the thermal moxibustion observation group was better than the control group in reducing the subjective feeling of pain.12.The results of meta analysis of plasma IL-6 showed that the level of plasma IL-6 in the treatment group of moxibustion combined with traditional Chinese medicine was better than that in the control group [MD =-1.04,95% CI(-9.92,7.84),z = 0.23,(P = 0.82)].13.The meta analysis results of the follow-up of the cure rate showed that compared with the control group,[RR = 2.05,95% CI(1.55,2.72),z = 5.02,(P < 0.00001)],the long-term effect of the observation group was significantly better than that of the control group.14.Meta analysis results of m-joa score showed that compared with the control group,[SMD= 0.04,95% CI(-0.09,0.17),z = 0.60,(P = 0.55)],the long-term effect of the thermal moxibustion observation group in improving the clinical symptoms of patients with lumbar disc herniation was significantly better than that of the control group.15.The follow-up meta-analysis results of VAS score showed that [MD =-0.10,95% CI(-0.54,0.34),z = 0.43,(P = 0.66)],the hot moxibustion observation group was better than the control group in reducing the long-term effect of patients' subjective pain.15.In 29 articles included in meta analysis,12 studies mentioned adverse reactions,all of which reported that there was no adverse reaction in the treatment of lumbar disc herniation by using heat sensitive moxibustion.Conclusion:1.Since 2006,the research of correlation in the field of moxibustion has been paid more attention and has high research value.2.Jiangxi traditional Chinese medicine,Journal of Jiangxi University of traditional Chinese medicine,Shanghai Journal of acupuncture and moxibustion,modern distance education of Chinese traditional medicine and practical clinical integration of Chinese and Western medicine are the main journals in this field.3.Guangzhou University of traditional Chinese medicine is the most important institution to confer doctoral degree in the field of moxibustion research,followed by Jiangxi University of traditional Chinese medicine.The amount of funding for the literature included in the study is relatively small,so we should pay attention to the funding project investment in this field.4.Professor Chen Rixin is the high-yield author in the field of moxibustion research.The research team of Jiangxi University of traditional Chinese medicine and its affiliated hospital,represented by him,has made the greatest contribution.The main form of the author cooperation network in this field is in-house cooperation,and a wider range of cooperation among authors has potential.5.The regional distribution of the research institutions of thermal moxibustion is mainly concentrated in East China,South China and central China.In recent years,it has gradually spread all over the country(Xinjiang,Liaoning,Jilin,Heilongjiang,Yunnan,Hainan,Inner Mongolia,Gansu,etc.)6.The type of literature research in this field is mainly clinical research.Because of its stable safety and wide applicability,the thermal moxibustion therapy is also fully emerging in clinical nursing.Knee osteoarthritis,lumbar disc herniation,scapulohumeral periarthritis,cervical spondylosis of vertebral artery type,primary dysmenorrhea and peripheral facial paralysis and other painful diseases are the hot spots of moxibustion.7.The application of simple heat sensitive moxibustion(or + acupuncture / needle knife /electroacupuncture / traction / fumigation + traction / traditional Chinese Medicine)is superior to the control group in improving the total clinical cure rate,reducing pain and improving clinical symptoms of patients with lumbar disc herniation.8.The heat sensitive moxibustion therapy is simple,green and safe,without obvious side effects.The curative effect of the treatment of lumbar disc herniation is long-lasting and stable,and it is not easy to recur,which is worthy of clinical promotion. |
