Clinical And Experimental Study Of Jiangzhi Granules In The Treatment Of Non-alcoholic Fatty Liver Disease

Purpose: To summarize Professor Lu Bingjiu's clinical experience and academic viewpoints in the treatment of non-alcoholic Fatty liver disease(NAFLD),to observe the clinical efficacy of Jiangzhi Granule,to establish a reasonable animal model of NAFLD,to clarify its pharmacodynamics,and to verify the clinical value of classic prescription,to explore the mechanism of the treatment of NAFLD from the molecular biological point of view,to explain the scientific connotation of the treatment of NAFLD by classical prescriptions,and to provide a solid theoretical basis for the wide application and popularization of classical prescriptions in the treatment of NAFLD.Materials and methods:Based on Professor Lu Bingjiu's clinical experience in the treatment of NAFLD,this paper expounds Professor Lu's unique views,and makes a retrospective analysis of Professor Lu Bingjiu's NAFLD patients who were treated with traditional Chinese medicine in 2017 to explore the medication rules of Professor Lu Bingjiu in the treatment of NAFLD.Clinical research: Based on Professor Lu Bingjiu's clinical experience in the treatment of NAFLD,this paper expounds Professor Lu's unique views,and makes a retrospective analysis of Professor Lu Bingjiu ' Alt,AST,GGT,TC,TG and other factors related to liver function and blood lipid were taken as indexes.The symptoms and signs of traditional Chinese medicine were quantified and integrated.The clinical effect of the prescription was comprehensively evaluated by the comparison and analysis before and after treatment.Experimental research: NAFLD mice model was established by high-fat diet induction method.Based on the liver pathological state,liver function index and blood lipid index,the model was verified by comparing the data of blank group and model group.The traditional Chinese medicine group was given different doses of lipid-lowering granules,the western medicine group was given yishanfu,and the open group was given normal saline for 4 weeks.The pathological state of liver was observed by lipid-lowering granules Based on the sting signal pathway,Western blot was used The expression of sting,p-igf3 protein and sting,tbk1,IRF3,IL-1 ?,IL-6,TNF-? gene before and after the administration of Jiangzhi granules were detected by blot and PCR,and the mechanism of Jiangzhi granules in the treatment of NAFLD was discussed.Results:Clinical research: Professor Lu's method of differentiation and treatment of NAFLD by stages is mainly to eliminate fat and evil,supplemented by strengthening the body.There are two groups of commonly used drugs: the first group is Alisma,cassia seed,Hongqu,hawthorn,and the second group is tangerine peel,Poria cocos,big belly peel,Atractylodes macrocephala and Atractylodes macrocephala.Jiangzhi granule,composed of Alisma orientalis,cassia seed,Hongqu and hawthorn,has the effect of promoting blood circulation and removing blood stasis.After the treatment with Jiangzhi granules,each observation index has different degree of improvement,body mass index,liver function index(ALT,AST,GGT)and blood lipid index(TC,TG)were significantly lower than that before treatment.The degree of fatty liver was significantly reduced,80% of the patients were treated effectively,13.33% of them were cured;the comprehensive evaluation showed that the formula had a significant clinical effect in the treatment of NAFLD.Experimental research: After 8 weeks of modeling,there were significant differences between model group and blank group in general condition score,liver function index(ALT,AST,GGT),blood lipid index(TC,TG)and liver pathological state(P < 0.05),indicating that the modeling was successful;(2)Compared with the model group and the open group,the serum levels of liver function(ALT,AST,GGT)and blood lipid(TC,TG)in the traditional Chinese medicine group and the western medicine group decreased significantly(P < 0.05).Compared with the western medicine group,the traditional Chinese medicine group was closer to the blank group after treatment,indicating that the traditional Chinese medicine group had a better therapeutic effect on NAFLD;(3)After drug treatment,compared with the model group and the deserted group,the expression of related proteins and related genes in the liver tissue gradually tended to the blank group,indicating that Jiangzhifang may achieve the purpose of treating NAFLD by regulating STING pathway.Conclusion:1.Professor Lu thinks that the basic mechanism is liver stagnation and qi stagnation,spleen deficiency and dampness accumulation,and phlegm and blood stasis.The treatment should be mainly to dredge liver,invigorate spleen and Qi,remove dampness and phlegm,and promote blood circulation and blood stasis.2.The clinical effect of Jiangzhi Granule on Nash is definite.3.Jiangzhi granule can reduce inflammation and blood lipid by regulating STING-TBK1-IRF3 pathway and related pathway,so as to improve liver injury and achieve therapeutic effect.
