| Objective:The purpose of this study was to observe the curative effect of the Xinshang Shifu Liquid on swelling and pain before the surgery of closed ankle fractures and effects on serum inflammatory factors,to evaluate the clinical efficacy of the Xinshang Shifu Liquid,in order to provide more effective treatment for swelling and pain of ankle fracture before operation.Methods : From January 2018 to July 2019,60 cases of fresh closed ankle fracture admitted to the first Department of Traumatic Orthopedics of affiliated Hospital of Changde of Hunan University of Chinese Medicine(hereinafter referred to as our hospital),randomly divided into 30 cases of Chinese medicine and 30 cases in the control group.Both groups were braked and conventionally raised the limbs,within 24 hours after the injury local injury position on both sides of the ice,both with 20% mannitol intravenous injection(Hun an Jinjian Pharmaceuticals 50 g 250ml/bottle),drop in about 30 minutes,1 time/day,one course of treatment a week.The Chinese medicine group on the basis of the control group to external Xinshang Shifu Liquid(24 hours after admission),changed medicine 2 times a day,treatment for a week for 1 course of treatment.The pain VAS scores and the swelling value and the longest diameter of the plaque of the two groups were evaluated on the day of admission and after 1 or 4 or 7 days of treatment respectively,and levels of inflam matory factor serum CRP,TNF-?and IL-6 before and 7 days after treatment.Calculating the deflating rate as the evaluation criterion of clinical efficacy,evaluating the effect of swelling for 4 or 7 days after treatment;Clinical score table based on ankle swelling,and the waiting time before operation was calculated;The color Doppler ultrasound of the lower extremity arteries and veins was examined one day before operation to record the occurrence of deep venous thrombosis(DVT)of the lower extremities.Two groups of subjects te sted liver and kidney function after one week of treatment to evaluate adverse reactions to the drug..All data were processed and analyzed by statistical software SPSS 22..0 to eval uate the therapeutic effect of swelling and pain before the operation of ankle fracture in chinese medicine group and control group.Results :(1)The Chinese medicine group had 1 cases of skin allergy symptoms in the course of treatment,and the trial was terminated,and the control group requested 1 case of transfer,and a total of 58 cases were completed.There was no significant difference in sex ratio,age,BMI,time of treatment after injury,injury side,Langues-Hansen classification of fracture between the two groups(P>0.05).(2)The pain VAS scores and the swelling value and the longest diameter of the plaque of the two groups was no significant difference before treatment and one day after treatment(P>0.05).However,it was statistically significant in the treatment 4 days and 7 days after the Chinese medicine group significantly lower than the control group(P<0.05).(3)The difference Serum inflammatory factors CRP,TNF-?and IL-6 were not statistically significant before treatment(P>0.05),and the serum inflammatory factors CRP,TNF-?and IL-6 in the Chinese medicine group were lower than the comparison,and the difference was statistically significant(P<0.05).(4)Compared with the effect of swelling in the two groups of patients after 4 days an d 7 days of treatment,the Chinese medicine group was significantly better than the control group,and the difference was statistically significant(P<0.05).(5)Compared the pre operative waiting time of the two groups,there was significant difference between the two groups(P<0.05).(6)There was no significant difference in the incidence of DVT between the two groups during treatment(P>0.05).Conclusion : Xinshang Shifu Liquid can effectively on swelling and pain before the treatment of ankle fracture,Can significantly reduce serum inflammatory factor and shorten preoperative swelling time,it was statistically significant In the early treatment of soft tissue swelling around the ankle and the elimination of subcutaneous plaque.it's worth promoting clinically. |
