Juzhu Siwu Decoction Combined With PFNA In The Treatment Of Elderly Femoral Intertrochanteric Fractures (qi Deficiency And Blood Stasis Syndrome) Clinical Observation

Objective:To observe and study the clinical effect of Juzhu Siwu decoction combined with Proximal Femoral Nail Anti-rotation in the treatment of intertrochanteric fractures in the elderly.It provides a new idea for the treatment of intertrochanteric fracture of Femur by the combination of ancient Formula Juzhu Siwu decoction and operation.Methods:This study included 60 patients from December 2018 to December 2019 who met the inclusion and exclusion criteria for intertrochanteric fractures in the Trauma Department of The Second Affiliated Hospital of Traditional Chinese Medicine,the patients were randomly divided into treatment group(Juzhu Siwu decoction combined with PFNA)and Control Group(PFNA)with 30 cases in each group.PFNA fixation was performed by the same group of physicians in both groups.Infection prevention,antithrombotic therapy and nutritional support were performed routinely after operation.The treatment group was given Juzhu Siwu decoction on the basis of routine treatment for 30 days.Visual analogue scale(Vas),Harris hip score,TCM syndrome points and efficacy and postoperative complications were observed before and after treatment.Results:1.Pain Relief(Vas):In the aspect of relieving pain,comparing the Vas score of the treatment group with that of the Control Group on the first day,1 week and 2 weeks after operation,the results showed that there was no significant difference between the Treatment Group and the Control Group on the first day after operation(P>0.05)?There was significant difference in VAS scores between the two groups at 1 and 2 weeks postoperatively(P<0.05).Therefore,the treatment group is better than the control group in relieving the postoperative pain of intertrochanteric fracture.2.Improvement of HIP function(Harris):In improving hip function,there was significant difference between the Treatment Group and the control group before and after treatment(P<0.05),the Harris scores in the treatment group were significantly different from those in the Control Group 2 weeks,1 month and 3 months after operation(P<0.05).Therefore,the improvement of hip function in the treatment group was better than that in the control group.3.TCM syndrome points and efficacy:The scores of TCM syndromes in the two groups were significantly lower after treatment than before treatment(P<0.05).There was no significant difference between the two groups before treatment(P>0.05).The result of treatment group was lower than the control group,the difference was statistically significant(P<0.05).In the ASPECT OF TCM Syndrome curative effect,the Treatment Group is superior to the Control Group in Improving Syndrome curative effect,the difference is statistically significant(P<0.05).4.Complications:No complications such as limb shortening,COXA Vara,loosening or rupture of internal fixation implant,lower limb thrombosis,decubitus,pulmonary infection,pulmonary embolism and urinary tract infection were found in both groups.Conclusion:Juzhu S i wu decoction can significantly relieve postoperative pain,improve clinical symptoms,improve hip joint function and promote fracture healing by promoting blood circulation,strengthening spleen and relieving pain,supplementing Qi and nourishing blood.Juzhu S i wu decoction combined with PFNA is safe and effective in the treatment of intertrochanteric fracture of Femur(Qi deficiency and blood stasis syndrome).
