| Purpose:Meta-analysis was used to systematically evaluate the efficacy and safety of back blood-letting puncture therapy compared with other therapies such as oral Chinese medicine,western medicine,ointment for external use,and common acupuncture for acne.The frequency of common acupoints was summarized and the curative effect was evaluated to provide evidence-based basis for back blood-letting puncture therapy for acne.Material and method:According to the retrieval strategies,inclusion and exclusion criteria,the full-text databases of Chinese journals(CNKI),Wanfang data,Sino Med,VIP and PubMed,Cochrane library,Embase and Web of Science were retrieved and established until 2018.Monthly published literature on the treatment of acne with back pricking and bloodletting therapy.The type of literature research is randomized controlled trial,the language is Chinese and English;the original literature experimental group is mainly treated with back blood letting therapy,which can be combined with cupping or other auxiliary treatment measures,while the control group is different from other methods of back blood letting.According to inclusion criteria and exclusion criteria,the included literature is screened for data extraction and methodological quality.Quantitative analysis,using Review Man 5.3 software for meta-analysis of outcome indicators,to compare the difference of outcome indicators between back Pricking Bloodletting Therapy(or other treatment measures)and other therapies in the treatment of acne.Results:1.Eighteen articles were included in the meta-analysis.The total number of cases included was 1667,including 865 patients in the treatment group and 802 patients in the control group.Meta-analysis showed that among acne patients,the skin lesion regression rate of patients with back Pricking Bloodletting Therapy(or other treatment measures)was significantly higher than that of the control group,RR = 1.17,95% CI = 1.13-1.21,I2 = 8%,P < 0.00001,with statistical significance.Six of the research papers were treated with simple back PrickingBloodletting therapy,RR = 1.15,95% CI =(1.08-1.23),I2 = 0%,P < 0.0001;12 of them were treated with back Pricking Bloodletting Therapy Combined with other therapies,RR = 1.17,95% CI =(1.12-1.22),I2 = 25%,P < 0.00001,the difference was statistically significant.2.Among 865 patients in the experimental group,there were 21 cases of adverse reactions,including 2 cases of mild diarrhea and self-healing without treatment;2 cases of facial erythema desquamation;1 case of xerophthalmitis;but the symptoms of the two groups were alleviated after oral drug reduction or withdrawal,so the occurrence of adverse reactions was related to the use of oral drugs.The skin of 16 cases was purple at cupping and disappeared within 2 weeks after treatment.3.According to the frequency of selected acupoints,Dazhui 905,Feishu 715,Pishu 520,Geyu 479,Xinshu 374,Ganshu 308,Lingtai 211 times,Dachangshu 206,Shenshu 130,Danshu 66,Bitong point 60 and Gaoxue 30 were selected in the experimental group of 18 studies.Some studies mentioned the selection of acupoints based on syndrome differentiation.Of them,7 studies adopted the selection of acupoints based on syndrome differentiation.Of the 6 studies on acne with heat accumulation in the lung and stomach,5 of them jointly selected acupoints for the treatment of Dazhui and Feishu.In the four studies of liver depression and blood stasis,two jointly selected points as Dazhui and Ganshu,and the other two jointly selected points as Dazhui and Geshu.Dazhui and Weishu were included in the three studies on intestinal and stomach damp-heat.In the two studies of phlegm and blood stasis,the same points were selected as Dazhui and Lingtai.Conclusion:1.Treatment of acne with back pricking and bloodletting therapy has remarkable curative effect.2.Back Pricking Bloodletting Therapy has high safety and low incidence of adverse reactions.3.The first side of the bladder meridian corresponds to Zangfushu points: Feishu,Pishu,Geshu,Ganshu,Xinshu,etc.and Dumai points: Dazhui and Lingtai.The application rate of acne acne point selection in back bleeding therapy is higher,which provides a certain reference for clinicians to select acupoints in the treatment of acne. |
