Purpose: Taking patients with primary nephrotic syndrome as the research object,and using the theory of traditional Chinese medicine to carry out syndrome differentiation and classification,to explore the relationship between different TCM syndrome types,pathological types,and physical and chemical indicators,in order to contribute to the quantification of TCM,and provide some advice for clinical treatment of TCM.Material and method:This study was conducted on 219 PNS patients hospitalized in the nephrology department from July 1 to December 31,2018 of the general hospital of northern theater command(former the general hospital of shenyang military command),analyze its syndrome pattern,At the same time,basic data,clinical manifestations,pathological types,physical and chemical indicators and data of human body composition analyzer were collected.Various indicators of patients with different syndromes were analyzed with SPSS 21.0 software to find out the differences and correlation between different syndromes and various indicators.Results:1.Basic information: 153 cases(68.3%)were males and 66 cases(31.7%)were females,with a male to female ratio of about 2.31:1.The average age was 45.164 15.88 years.There were 93patients(42.5%)with hypertension and 126 patients(57.5%)without hypertension.2.Distribution of TCM syndromes of PNS: spleen-kidney Yang deficiency syndrome was the most(94 cases),followed by fengshui flooding syndrome(53 cases),damp-heat accumulation syndrome(34 cases),liver-kidney Yin deficiency syndrome(29 cases),and kidney collaterals stasis syndrome(9 cases).3.The relationship between TCM syndromes and biochemical indexes of PNS: there was a significant difference in the content of Hb between different syndromes(p<0.01),and the syndrome of liver-kidney Yin deficiency was significantly lower than the other.There were significant differences in BUN content among different types of syndromes(p<0.01).Therewere significant differences in TP and ALB contents among different syndromes(p<0.01),and liver and kidney Yin deficiency syndrome was significantly higher than spleen and kidney Yang deficiency syndrome.There was a significant difference in TC content among different syndromes(p<0.01),and spleen-kidney Yang deficiency syndrome was significantly higher than other syndromes.There was a significant difference in GFR content among the syndromes(p<0.01),and fengshui flooding syndrome was significantly higher than kidney collatarasis syndrome.4.Relationship between TCM syndromes and pathological types of PNS: there was a significant difference between each syndrome type and pathological types(p<0.01).In MCD,spleen-kidney Yang deficiency syndrome was the most in 28 cases(29.80%).In IgAN,liver-kidney Yin deficiency was prevalent,with 9 cases(31.00%).5.Relationship between TCM syndromes and immune indexes of PNS: IgG content of each syndrome was significantly different(p<0.01),and the syndrome of liver-kidney Yin deficiency was significantly higher than the other syndromes.6.Relationship between PNS TCM syndromes and human body composition analyzer data:there were significant differences among different syndromes in terms of ECW,Segmental Water(RL),Segmental Water(LL),average moisture of lower limbs,ECW/TBW(RA),and average extracellular fluid/total Water content of upper limbs(p<0.01),and spleen-kidney Yang deficiency syndrome were significantly higher than other syndromes.There were significant differences in the content of ECW/TBW(LA)between the syndromes(p<0.01),and the syndrome of kidney collateral stasis was significantly higher than that of other syndromes.7.Among the 219 patients,MN was the most,a total of 139 cases(63.47%),followed by MCD49 cases(22.37%),IgAN23 cases(10.50%),4 cases of FSGS and MsPGN(all accounting for 1.83%).Conclusion:1.In PNS,the most traditional Chinese medicine certificate is the spleen and kidney.Thelevel of cholesterol and the degree of edema in the spleen-kidney Yang deficiency syndrome are significantly higher than the other syndromes,and the level of total protein and albumin is significantly lower than other certificates.2.In PNS,the renal function of renal stasis is lower than other certificates.3.In PNS,the IgG of the liver and kidney deficiency is significantly higher than that of other syndromes,and the level of hemoglobin is significantly lower than that of other certificates.4.There is a certain relationship between pathological types and TCM syndromes.The most common syndromes are spleen and kidney Yang deficiency(MN),spleen and kidney Yang deficiency(MCD),and liver and kidney Yin deficiency(IgAN).
