The present study is focused on the combination of theoretical research and clinical research,summarizing Professor Shumin Jiang's academic thoughts and clinical experience in liver cirrhosis therapy.Part one Theoretical research Objective: Summery of Professor ShuminJiang's understanding of etiology and pathogenesis for liver cirrhosis and his clinical experience in the treatment of liver cirrhosis.Clinical data and research methods: by collecting outpatient cases(including patients' complains,history of present illness,previous history of illness,symptoms and diagnosis,therapy and drug record,etc.),and by orally recording Professor Shumin Jiang's experience in his daily clinical process of liver cirrhosis therapy,which is regarded as the basis of his academic thoughts,so as to summarize Professor Shumin Jiang's experience in the treatment of liver cirrhosis.Conclusion: 1.Professor Shumin Jiang believes that liver cirrhosis is caused by six external causes,excessive food and drink,emotional frustration,development of other diseases,liver impairment by parasite and toxin,and internal injury by overwork.2.The pathogenesis of liver cirrhosis includes liver damages by diseases,liver deficiency,sluggish circulation of blood,blood stasis,spleen affected by liver deficiency,spleen deficiency,disturbance of water metabolism,and water retention.3.Professor Shumin Jiang thinks highly of protecting liver,integrating treatment based on the differentiation of syndrome,symptom,and disease,with focuses on phased treatment.For early phase,moderately improve liver deficiency and overcome diseases;for middle phase,adjust liver and spleen,strength spleen;for later phase,disperse blood stasis and remove water retention.For clinical practice,it is suggested to start with small dosage,and to add more dosage later,to accurately estimate the dosage at every phase so as to succeed with mild dosage.Part two Clinical ResearchObjective: observe the clinical effects of Chinese herb decoction ‘Ruan Gan Jian'prescribed by Professor Shumin Jiang on early phase of liver cirrhosis with spleen deficiency symptomsClinical data and research methods: 52 cases of patients recorded from October 2016 to October 2017 in Affiliated Hospital of Liaoning University of Traditional Chinese Medicine by Professor Shumin Jiang.Patients are divided into treatment group and control group according to the random numerical table with 26 patients each group respectively.The two groups were given life intervention,including reasonable work and rest,changed diet,prohibition of alcohol,prohibition of any medicine harmful for spleen,etc.Based on Professor's experience prescription,the treatment group takes one decoction dosage two times a day.Control group takes Polyene Phosphatidylcholine capsules,three times a day,0.456 g each time.The treatment lasts for 3 months,and afterwards the effects of it are calculated.Results: 1.Clinical effect comparison of two groups: effective rate for treatment group is 100%,while effective rate for control group is 96.15%,thus there is a significant discrepancy for two groups(P<0.05).2.Symptom and sign checklist comparison of two groups: the checklist scores for treatment group declined from 21.27±4.45 to 3.38±2.00.There is a significant discrepancy for treatment group since the treatment(P<0.01);and also significant for two group comparison(P<0.05).3.Four parameters ofliver function comparison: there is a significant discrepancy for treatment group since the treatment in the aspects of ALT,AST and TB parameters(P<0.01);and also significant for two group comparison(P<0.01).There is a significant discrepancy for treatment group since the treatment in the aspect of ALB(P<0.05).4.Color ultrasound index comparison of two groups: there is a significant discrepancy for treatment group since the treatment in the aspect of internal diameter of the vein,and also significant for two group comparison(P<0.05);there is a significant discrepancy for treatment group since the treatment in the aspect of internal diameter of the spleen vein,and also significant for two group comparison(P<0.01);there is a significant discrepancy for treatment group since the treatment in the aspect of spleen thickness,and also significant for two group comparison(P<0.05).Conclusion: 1.Patients in the treatment group(experience prescription)and control group(Polyene Phosphatidylcholine capsules)areboth improved regarding their clinical symptoms and physicochemical indexes in the early phase of liver cirrhosis;However,treatment group has more significant effect reflected by Symptom and sign checklist.2.Patients in the treatment group and control group are both improved in the four parameters of liver function.Treatment group has more significant effect on AST,ALT and TB than control group does;however,no significant effect for ALB was found in the two groups.3.Patients in the treatment group are observed to have significant effect for their internal diameter of the vein,internal diameter of the spleen vein and spleen thickness;however,patients in the control group are observed no obvious effects.
