Purpose:This study counts and analyses the characteristics of the distribution of TCM syndrome type and related factors of biochemical indexes of the DKD patients in Department of Endocrinology,Affiliated Hospital of Liaoning University of Traditional Chinese Medicine.To summarize the relationship between various TCM syndromes and various indicators,risk factors of DKD,in order to provide the more targeted and individualized prevention and treatment measures in the clinic.so as to provide reference for relevant medical professionals.Material and method: 1.Screening 219 Type 2 diabetic patients from the Affiliated Hospital of University of TCM Department of Endocrinology hospital,May 2017 to ovember 2017,and 164 cases of DKD and 55 cases of simple type 2 diabetic patients.Using a retrospective analysis method to counts and analyses.Dialectic of traditional Chinese Medicine reference Chinese medicine medicine nephropathy branch in 2007 released "diabetic nephropathy diagnosis,syndrome differentiation and efficacy evaluation standards(Trial)",DKD is divided into yin deficiency and dryness heat syndrome and Qi and Yin deficiency syndrome,spleen kidney deficiency syndrome,yin and Yang deficiency syndrome.2.Collection of information:The basic data of the patients,blood pressure,indicators related to glycometabolism,lipid metabolism related indexes,renal function related indicators,urea and C-reactive protein.3.Seting up the observation Excel database,and using the modern analysis software SPSS 20 to carry out the data statistics and analysis.Results: 1.The incidence of group DKD has no significant difference in sex.The average age,fasting blood glucose,triglyceride,total cholesterol,low density lipoprotein,systolic blood pressure,pulse pressure,blood urea,creatinine and uric acid of DKD group were higher than control group,with statistical significance(P < 0.05),the course is longer than the type 2 diabetes group,with statistical significance(P < 0.05).,2.Logistic analysis showed that the course of disease(OR=1.127),FBG(OR=1.192),TG (OR=1.702)and pulse pressure difference(OR=1.038)were statistically significant(P<0.05),they are risk factors for DKD and positively correlated with the incidence of DKD.Yin hot syndrome and Qi and Yin deficiencys,DKD patients are divided into 4 syndrome types,including 34 cases of yin deficiency and dryness heat syndrome(20.73%),Qi and yin deficiency syndrome in 80 cases(48.78%),spleen kidney deficiency syndrome in 31 cases(18.90%),19 cases of yin and Yang deficiency syndrome(11.59%),The number of patients with Qi and Yin deficiency syndrome is the largest,and the number of Yin and Yang deficiency syndrome patients is relatively small.By Spearman rank correlation analysis,the results showed that TCM syndrome progression are consist with DKD clinical staging,and there is a significant positive correlation(P<0.01,Rs=0.536).4.There are significant differences in the age,course,pulse pressure,microalbuminuria,urine microalbumin /Ucrl(ACR),serum creatinine and urea between 4 types TCM syndromes of DKD,with statistical significance(P<0.01).There are differences in systolic blood pressure(P<0.05).5.In TCM syndrome types of DKD,Yin and yang deficiency syndrome were used as reference,course of disease,age and microalbuminuria were risk factors for the development of yin deficiency and dryness heat syndrome(P<0.05).And age and microalbuminuria are risk factors for the development of both Qi and yin deficiency syndrome and spleen kidney deficiency syndrome.Conclusion: 1.The incidence of DKD has a certain relationship with the older age and a long course of disease.2.abnormal Lipid metabolism and glucose metabolism and high blood pressure may be the risk factors for DKD.3.DKD TCM syndromes are consistent with its clinical stages.4.Systolic blood pressure,pulse pressure,microalbuminuria,ACR,creatinine and urea increasing may indicate that TCM syndrome is progressing.
