Clinical Observation Of Jianpi Yiqi Decoction In The Treatment Of Swelling And Pain After Artificial Total Hip Arthroplasty (Qi Deficiency And Blood Stasis Type) For Femoral Neck Fracture

Objective: to observe the effect of "Jianpi Yiqi decoction" on swelling and pain of affected limbs,and the influence of serum CRP,erythrocyte sedimentation rate(ESR)-related inflammation in elderly patients after total hip surgery,and to observe the effect of "Jianpi Yiqi decoction" on swelling and pain of the affected limbs.And the effect of early functional recovery of hip joint on the affected side,which provides the clinical theoretical basis for the treatment of early swelling and pain after total hip replacement with the drugs of invigorating spleen and tonifying water and invigorating qi and supplementing blood.In order to optimize the clinical postoperative treatment and rehabilitation program to provide more reference,to provide the elderly patients with postoperative swelling and pain to provide a new way.Methods: from January 2018 to January 2019 in the Department of Orthopaedics and Joint of Changde first traditional Chinese Medicine Hospital,the patients were diagnosed as femoral neck fracture.Sixty elderly patients who met the following inclusion criteria and underwent unilateral total hip arthroplasty(Qi deficiency and water stagnation type)for the first time in our hospital were included in the criteria according to the diagnostic criteria,which recorded the age,sex,course of disease and other related data of the patients.The 60 patients were divided into treatment group(n=30)and control group(n=30)according to the method of random numerical table.The patients in control group were treated with routine detumescence and pain relief after operation,and the treatment group was given orally on the basis of this." Jianpi Yiqi decoction for 2 weeks.On the 1st,3rd,7th,14 th day after operation,the 10 cm of the upper edge of the affected knee was measured,and the difference of swelling at the same site before operation was calculated.The VAS score and CRP,erythrocyte sedimentation rate(ESR)of the affected knee were observed and recorded 1 day before operation,1,3,7,14 days after operation.The Harris score of hip joint was recorded 1 day before operation and 14 days after operation.The data were analyzed statistically with SPSS 22.0 software,and the data was set as a statistically significant standard(P < 0.05).Results: 1.In this study,one patient in the treatment group withdrew from the treatment group because of asking for early discharge and refused to continue hospital treatment,and 29 patients in the control group were actually completed,while one patient in the control group withdrew because of the strong reaction of the gastrointestinal tract to other drugs.There were 29 cases remaining in the treatment group and 29 cases in the control group,and the rate of withdrawal was 3.3%.2.There was no significant difference in age and length of surgery between the two groups(P > 0.05),but there was no significant difference in sex and location(left / right hip)between the two groups(P > 0.05).3.There were statistical differences between the two groups in terms of limb swelling,VAS score,Harris function score of hip joint,serum CRP,erythrocyte sedimentation rate(ESR)and TCM syndrome score before and after operation(P > 0.05),and there was no significant difference between the two groups(P > 0.05).There was no significant difference in swelling value,VAS score,Harris function score of hip joint,CRP,erythrocyte sedimentation rate(ESR)and TCM syndrome score between the two groups before operation.The swelling value of the affected limbs in the two groups was 1,3,3 after operation,and there was no significant difference between the two groups.There was no significant difference between the two groups on the 14 th day(P > 0.05).The difference of between the two groups,serum CRP,on the 1st and 14 th day after operation There was no significant difference in Harris function between the two groups on the 14 th day after operation(P < 0.05),and there was a significant difference in the VAS score and hip joint function score between the two groups on the 14 th day after operation(P < 0.05).Conclusion:"Jianpi Yiqi decoction" combined with clinical western medicine in the treatment of early swelling after total hip replacement,pain has a good effect.It has obvious help for early functional exercise of the affected side hip joint,and it is worth popularizing and applying in clinical practice.
