A Multi-center Randomized Controlled Clinical Observation Of Self-made Wuling Qushi Ointment In The Treatment Of Chronic Hand Eczema

Chronic hand eczema is a kind of hypersensitive inflammatory skin disease which occurs in the hand and is often changed from subacute eczema to chronic.Chronic hand eczema is characterized by local skin lesions,infiltration and hypertrophication,on which scales,scratches and blood scabs can be seen,accompanied by severe itching,repeated attacks,and difficult to heal.Chronic hand eczema,as one of the common and frequently-occurring diseases in dermatology,has a trend of increasing prevalence year by year,which not only makes patients suffer from pain,affects aesthetics and hinders social contact,but also brings huge social and psychological burden to patients.However,it is difficult to determine the cause of the disease,and many endogenous and exogenous factors can induce and aggravate the disease.Therefore,it is particularly important to seek safe and effective treatment methods,reduce the recurrence rate of the disease,alleviate and relieve the patients' pain,improve their quality of life,and finally achieve physical and mental health.ObjectiveObserve the clinical effect of Self-made WuLingqushi Ointment on chronic hand eczema in multiple centers.MethodA multicenter,randomized controlled trial was conducted.The experimental group was treated with Self-made WuLingqushi Ointment,and the control group was treated with QingPeng Ointment,one time for external use in the morning and evening,4 weeks as a course of treatment.The efficacy and safety of the two topical creams were evaluated by comparing the two groups of subjects' overall evaluation of IGA score,VAS score for itching and pain,HECSI score for improved hand eczema severity index and DLQI score for skin disease before and after treatment at week 2 and 4 after treatment.ResultA total of 94 cases of chronic hand eczema were included in this study,including 38 cases in hospital A(20 cases in the experimental group and 18 cases in the control group),34 cases in hospital B(18 cases in the experimental group and 16 cases in the control group),and 22 cases in hospital C(12 cases in the experimental group and 10 cases in the control group).The general information(gender,age and course of disease)of the two groups of subjects as well as the IGA score,the VAS score,the DLQI score and the HECSI score before treatment showed no significant difference(P>0.05),which was comparable.Comparison between the experimental group and the control group:after one course of treatment,IGA score,VAS score,DLQI score and HECSI score of the subjects in the two groups were significantly lower than before treatment(P<0.05),which proved that Self-made WuLingqushi Ointment and QingPeng Ointment were effective in the treatment of chronic hand eczema.Comparison between experimental group and the control way:after a course of treatment,two groups of subjects of IGA score,itching and pain VAS score,HECSI score had significant difference(P<0.05),DLQI score had no significant difference(P>0.05),shows that Self-made WuLingqushi Ointment was significantly better than QingPeng Ointment in the treatment of chronic hand eczema,relief of skin itching and pain.Finally,among the 50 cases in the experimental group,5 cases were cured clinically,17 cases were obviously effective,and 20 cases were effective.The total effective rate was 84.00%,and no adverse reactions occurred.Among the 44 cases in the control group,2 cases were cured clinically,10 cases showed significant effect,and 19 cases were effective,with a total effective rate of 70.45%,and no adverse reactions occurred.The difference in efficacy between the two groups was statistically significant(P<0.05).One month after the end of the clinical trial,the recurrence rate of the experimental group and the control group was calculated through follow-up.Finally,there were 4 cases of recurrence in the experimental group,and the recurrence rate was 9.52%.In the control group,9 cases relapsed,with a recurrence rate of 29.03%.ConclusionThe efficacy of Self-made WuLingqushi Ointment in the treatment of chronic hand eczema is significantly better than QingPeng Ointment,without adverse reactions,safe and effective,worthy of further clinical promotion.
