Objective:To compare the difference between the electroacupuncture and deep thorn "lumbar disc herniation five points" therapy and conventional western medicine conservative therapy for lumbar disc herniation,and to explore the superior patient population of lumbar disc herniation through subgroup analysis,and provide evidence for clinical optimization of treatment plan.Method:This study used a prospective cohort study into clinical patients,according to different treatment methods,the patients were divided into the exposed group and the control group,and the exposed group was treated with electroacupuncture deeply acupuncture"lumbar disc herniation five points".The treatment method of the control group is conventional Western medicine conservative therapy.The JOA score,VAS score and TCM semi-quantitative score of the two groups were observed before and after treatment.The therapeutic effect was compared.Subgroup analysis was performed according to gender,age,BMI,course of disease,lesion segment,TCM syndrome,etc.Exploring the influencing factors affecting the efficacy of electroacupuncture deep puncture "lumbar disc herniation five points".Results:A total of 161 patients,including 96 in the exposed group and 65 in the control group.The general data were not statistically significant,and the baseline data were similar and comparable.In the exposed group,82 patients with improved clinical symptoms had a total effective rate of 85.42%;50 patients with improved clinical symptoms in the control group had a total effective rate of 76.92%.There was a statistically significant difference in VAS scores between the two groups after treatment(Z=-2.074,P=0.038).The JOA scores after treatment were statistically different(Z=-2.737).,P=0.006).Subgroup analysis showed that in the comparison of pain relief in the two groups,the exposed group was female,51-60 years old,BMI was overweight,the disease course was 1-5 years,the daily working time was 4-6 hours,the lesion was double segment,and there were In the subgroup with pathological and blood stasis syndrome,it may have more pain relief;the control group is more relieve pain in the BMI with normal BMI,less than 0.5 years,daily work in 2-4 hours,and damp-heat syndrome subgroup.In the comparison of the improvement of lumbar function in the two groups,the exposed group was female,61-70 years old,BMI was overweight,the disease course was 1-5 years,the daily working time was 2-4 hours,the lesion was double segment and three segments and above,no In the subgroup with pathological changes and blood stasis syndrome,it may have the effect of improving lumbar function.In the control group,the BMI was normal,the disease course was more than 5 years,the daily work was more than 6 hours,and the damp-heat syndrome subgroup was more effective in improving lumbar function.Conclusion:1.Electroacupuncture deep puncturing "lumbar disc herniation five points" for the treatment of lumbar disc herniation has good clinical effect.2.Compared with conservative treatment,electroacupuncture deep puncture"lumbar disc herniation five points"therapy can reduce the VAS pain score,which can significantly increase the JO A score.3.The blood stasis syndrome type may be a suitable TCM syndrome type for electroacupuncture deep thorn "lumbar disc herniation five points" therapy.4.Electroacupuncture deep puncture"lumbar disc herniation five points" therapy has a specific advantage in the treatment of lumbar disc herniation compared with conventional Western medicine conservative therapy.
