Objective:To explore the correlation between sural-bean bone and its surrounding soft tissue morphology and sural-bean syndrome from the clinical and imaging levels,to summarize the objective diagnostic indicators and therapeutic evaluation mechanism of sural-bean syndrome,to analyze and summarize the clinical efficacy of small needle knife in the treatment of sural-bean syndrome,and to explore its mechanism from the perspective of clinical observation;The superiority of the combination of diagnosis and modern musculoskeletal ultrasound diagnosis and treatment technology opens up a new perspective for the treatment of gastrointestinal syndrome by traditional Chinese medicine,and further inherits and develops the meridian tendon theory of traditional Chinese medicine and small needle-knife therapy.Research methods:Clinical Research 1:In this study,28 patients with gastrocnemius syndrome were recruited,all of them met the inclusion criteria of gastrocnemius syndrome and signed the informed consent.All patients were unilateral knee disease,including 17 males and 11 females,aged 33-75 years,with an average age of 54 years,without loss.VAS scores of pain symptoms at the posterolateral knee were collected at admission;the width,height and thickness of sural beans were collected by X-ray imaging;the thickness of sural fascia and the beginning of beans and fibula ligaments were collected by muscle-bone ultrasound imaging;statistical data were analyzed by SPSS 22.0 and bivariate correlation analysis.The correlation coefficients between the posterolateral knee pain level(VAS score)and the collected indicators were analyzed,and the clinical significance of the indicators in the diagnosis and evaluation of sural-soybean syndrome was discussed.Clinical Study 2:In this study,74 patients with gastrocnemius-soybean syndrome were treated with small needle knife,including VAS score before treatment,after the first treatment,after the last treatment,pain relief degree,the thickness of gastrocnemius-soybean surface myofascial fascia and the thickness of the initial site of the soybean-fibular ligament observed by muscle-bone ultrasound,and the thickness of VA 1 month after treatment.Follow-up survey of S score showed that pain relief degree,effective rate and apparent efficiency were obtained by calculation.Statistical data were analyzed by SPSS22.0.Paired sample t test was used to measure VAS score and pain relief degree before and after treatment,and P<0.05 was used to indicate the difference.The difference was significant before treatment,after the first treatment and after the last treatment.The thickness of muscle fascia on the surface of gastrocnemius and the thickness of the initial part of the fibula ligament were compared by paired sample t test.The difference was statistically significant with P<0.05.The above objective index data units were all mm.All the above data were expressed in form.According to the results,the curative effect of acupotomy in the treatment of sural-soybean syndrome was evaluated,and the mechanism was preliminarily explored from the clinical point of view according to the data.Results:Clinical study 1:The VAS score was 5.82±1.23 female:5.45±1.13,the width of sural soybean bone was 7.81 ±0.91female:7.78±0.61,the height of sural soybean bone was 13.16±1.08mm female:13.12±1.71 mm.Sural soybean bone thick-ness male:4.16±0.64 mm female:4.05±0.69 mm;male:0.9 ±0.16mm female,0.89±0.09 mm;,male:5.82±1.24mm female:3.82±0.55mm.The results were as follows:VAS score and gastrocnemal thickness | r | = 0.834,the thickness of the superficial fascia of the peroneal ligament | r | = 0.824,the thickness of the initial part of the peroneal ligament |r|=0.809,with strong correlation with the bone height |r|=0.708 of the gastrocnemius.The distribution p value is less than 0.05,which is statistically significant.Clinical study 2:After data analysis,it was concluded that the VAS score was significantly lower than that before treatment at 1 week after the first treatment,1 week after the last treatment and 1 month after treatment,and the difference was statistically significant(P<0.05).Before and after treatment,one month after treatment,there were 42 cases of complete remission,18 cases of obvious remission,6 cases of moderate remission,3 cases of mild remission and 5 cases of non-remission in 74 patients with complete remission,18 cases of obvious remission,6 cases of moderate remission,3 cases of mild remission and 5 cases of non-remission.The effective rate was 81.0%and the effective rate was 89.2%.Among them,10 patients had transient pain aggravation during treatment,and no other complications occurred.Comparison of sural bean bone table between two times The thickness of the fascia and the thickness of the initial part of the peroneal ligament(p<0.05)were significant(p<0.05).The effect of needle-knife on the treatment of gastrocnemal syndrome is significant.Conclusion:The main results are as follows:(1)the formation of sural soybean bone is affected by the stress in the process of activity,which indicates the stress state of sural soybean bone to a certain extent.Therefore,the morphology of sural soybean bone has certain guiding significance for the diagnosis and treatment of sural soy bone syndrome.(2)the thickness of muscle fascia on the surface of sural muscle,The thickness of the starting part of the peroneal ligament was significantly correlated with the symptoms of posterolateral knee pain in sural soya-bone syndrome,which had significant guiding significance as the diagnostic basis and evaluation index of curative effect of sural soya-bone syndrome.The thickness of muscle fascia on the surface of sural bean bone can be used as a reliable and objective index for diagnosis and evaluation of curative effect.(3)Needle knife based on meridian theory The treatment of sural bean bone syndrome is effective,can effectively relieve the pain symptoms of patients,and effectively improve the proliferation of muscle fascia on the surface of sural bean bone.(4)in the treatment of sural bean syndrome,Based on the theory of meridians and tendons,manual localization and ultrasonic examination of muscle and bone are combined with needle knife,which has the advantages of high safety,obvious improvement of symptoms,quick effect and so on,and has the advantages of innovation.
