This paper is divided into three parts:literature review,theoretical research and experimental research.All three parts are tightly wrapped around"condensed heat will turn the patient's skin as yellowish color,since the corresponding color of the spleen is yellow".Literature review sorting out the characteristics of zhongjing's treating jaundice,theoretical research and analysis of the decoction of lard and human hair,the experimental part of the combination of the two,starting from "blood stasis",select liver microcirculation disorders as the key point,replication of ANIT induced intrahepatic cholestatic jaundice rat model,and set empirical research for"Dcoction of Lard and Human Hair Can Be Adopted in all Cases of Jaundice".Literature review is divided into two part.Part one includes the related discussion of zhongjing on jaundice in treatise on miscellaneous diseases of typhoid fever,and author sorted out the syndrome differentiation of six meridians of jaundice.Part two is about the clinical research achievements of Jing-Fang in the treatment of jaundice in recent years.Theoretical research is divided into three parts:textual theoretical discussion,clinical application analysis and the interpretation of ancient doctors.Decoction of lard and human hair is set for jaundice and flatus vaginalis.Through various cases,author find out the decoction of lard and human hair is used for illness with various types of intestinal dryness and qi and blood disorder.In the part of experimental research,author set "Decoction of lard and human hair can be adopted in all cases of jaundice " as an example,study the biological mechanism of therapeutic effect on jaundice of decoction of lard and human hair through hepatic microcirculation.Part I Objective Evidence on "Decoction of Lard and Human Hair can be adopted in all cases of jaundice"Objective To observe the effect of decoction of lard and human hair on jaundice model ratsMethods 50 SD male were divided into 4 groups by random number table method as each 10 for control group,decoction of lard and human hair group and decoction of lard group,20 for model group.Jaundice model rats were induced by 2%ANIT olive oil solution gavage,and the normal control group was given the same amount of olive oil.After 48 h,the decoction was infused to stomach of rats,twice a day and the model group and the normal group were given the same amount of distilled water.On the 9th day,all rats were sacrificed.Liver tissue were collected and stained with conventional HE for pathological observation.TBIL,DBIL,IBIL,ALT,AST,ALP,GGT,TG were measured by automatic biochemical analyzer.24 h urine volume of rats was collected by metabolic cage.After 48 h,10 rats in the model group were randomly selected for serum biochemical detection to verify the successful modeling.ResultsLiver Tissue Histopathological ObservationIn the control group,the hepatic lobule structure was intact and the hepatic cells were line up.There was no obvious swelling,inflammatory cell infiltration or necrosis.Model group rats district bridge knot shaped connection between regional necrosis,hepatic cord cells loosely arranged,the form will disappear,unclear boundaries,liver cell swelling,and inflammatory cells infiltration,part of the hepatic cells significantly offse.Compared with model group,decoction of lard and human hair group,lard group rats liver damage were alleviated,accompanied by a small amount of inflammatory infiltrates.Result of Serum Biochemical TestAfter 48 h,the serum levels of ALT,AST,TBIL,DBIL,IBIL,ALP and GGT in the model group were higher than those in the normal group(P<0.05),indicating modeling successfully.The serum levels of ALT,AST,TBIL,DBIL,IBIL,ALP,GGT were significantly different between modeling after 48 h and 7 days through the absence of intervention.Compared with the control group,the serum levels of TBIL,DBIL,IBIL,ALP and GGT were increased(P<0.05)and serum ALT and AST were increased(P>0.05)in the model group(9d).Compared with the model group(9 d),serum biochemical levels of TBIL,IBIL,GGT and ALP were decreased in decoction of lard and human hair group(P<0.05).Compared with the model group(9 d),serum biochemical levels of TBIL,IBIL and ALP were decreased in decoction of lard group(P<0.05).Compared with the control group,the serum level of TG was increased in the model group(P<0.05).Compared with the model group,the serum level of TG was decreased in decoction of lard and human hair group and decoction of lard group.Result of 24 h urine volumeCompared with the control group,24 h urine volume in model group were significantly increased(P<0.05).Compared with the model group,24 h urine volume in the lard group were significantly decreased(P<0.05).ConclusionThe results indicate decoction of lard and human hair can alleviate ANIT-introduced jaundice rats liver function and bilirubin metabolism.Lard is not only the human hair solvent,but also can reduce the level of bilirubin and improve liver function in ANIT-introduced rats.Part II To Explore the Biological Mechanisms of "Decoction of Lard and Human Hair Can Be Adopted in all Cases of Jaundice"Objective To explore the biological mechanism of therapeutic effect of decoction of lard and human hair in the perspective of liver microcirculation that based on condensed heat will turn the patient's skin as yellowish color,since the corresponding color of the spleen is yellow.Methods UCDA group were added,and in each group 10 SD male rats were selected.Modeling,drug invention serum biochemical test and liver tissue H&E staining were the same as part I.plasma viscosity,low shear rate,medium shear rate and high shear rate were measured by blood viscometer.PT,APTT,TT and FIB were determined by platelet aggregation coagulation factor analyzer.The serum level of NO was determined by microplate,serum ET was detected by ELISA,and the expression of iNOS and ET-1 mRNA in liver tissue were detected by RT-PCR.ResultsHemorheological comparisonCompared with the control group,the levels of low,medium and high shear rates in the whole blood of the model group were all increased(P<0.05).Compared with the model group,the middle shear rate and high shear rate of whole blood in decoction of lard and human hair group were decreased(P<0.05),the high shear rate of whole blood in lard group decreased(P<0.05),and the low shear rate,medium shear rate and high shear rate of whole blood in the UCDA group decreased(P<0.05).Comparison of coagulation functionCompared with the control group,PT in the model group was significantly prolonged(P<0.05),APTT was shortened(P>0.05),TT was prolonged(P>0.05),and FIB content was increased(P>0.05).Compared with the model group,PT was significantly shortened in all medicine administration group(P<0.05),TT was shortened(P>0.05),and FIB content was reduced(P>0.05).Comparison of serum NOCompared with the control group,the serum level of NO in the model group was significantly increased(P<0.05).Compared with the model group,the serum level of NO in decoction of lard and human hair group,lard group and UCDA group were significantly decreased(P<0.05).Comparison of serum ET-1Compared with the control group,the serum level of ET-1 in the model group was significantly increased(P<0.05).Compared with the model group,the serum level of ET-1 in decoction of lard and human hair group,lard group and UCDA group were significantly decreased(P<0.05).Expression of iNOS mRNA and ET-1 mRNA in liver tissueCompared with the control group,the expression of iNOS and ET-1 mRNA in liver tissue of rats in the model group were up-regulated(P<0.01).The expression of ET-1 mRNA in the liver tissue of rats in decoction of lard and human hair group,the lard group and UCDA group were down-regulated than those in the model group(P<0.01),the expression of iNOS mRNA in the liver tissue of rats in decoction of laed and human hair group and lard group were downward than those in the model group(P<0.01).Compared with the model group,the expression of iNOS mRNA in the liver tissue of rats in UCDA group showed no significantly up or down regulation.ConclusionDecoction of lard and human hair can improve liver function of jaundice model rats,the mechanism of which may be related to reducing the level of ET-1 and NO,and ameliorating hepatic microcirculation.That is as Zhang Zhongjing said condensed heat will turn the patient's skin as yellowish color,since the corresponding color of the spleen is yellow.
