Objective: In clinical, obstructive jaundice(OJ) is a common pathologic status, which could cause many systemic organs injuries and corresponding pathophysiological changes.Acute renal failure (ARF) is the most proponent complication and one of most important reason,morbidity is 8-10% mortality is high to 82%[1]. At present, the mechanism of renal injury with OJ is not entirely illuminated yet.In recent years, The research lay on the microcirculation and gene is the focus of basic and clinical study. becaues of this, it would be very significant to elaborate the mechanisms. Astragalus memberanaceus (AM), which has wide physiological activity, could regulate immuno-system, scavenge OFRand dilate vascular,In this study we do the models of OJ by ligating the rats'commom bile duct. and observe the hemoperfusion, the caliber of blood capillary, the density change of blood capillary. Detect the renal function ,endothelin (ET), nitric oxide (NO)levels of serum. Study the effectiveness of AM to the renal microcirculation disturbance. We use AM to deal with some of the models. It is our purpose to observe the protective effect of AM on renal microcirculation of rats and investigate the mechanism of ARF in OJ and the mechanism AM on protecting the renal injury of rats with OJ. Thus we can provide with theoretic gist for clinical application.Method: 72 healthy SD rats, weighing 200±25g, were randomly divided into nine groups, each with eight, including: shame-operated (SO) 3,7,10 days; OJ model (Bile duct ligated, BDL ) 3,7,10 days; AM 3,7,10 days group. OJ model group, AM group: the common bile duct was ligated to induce obstructive jaundice. The administration was given by intraperitoneal injection .The quantity based on the time and weight. Observe the general changes of the kidney, measure the renal hemoperfusion by LISCA Laser Doppler Hemoperfusion image instrument and observe the renal microcirculation by WX-9B microscope on time. measure the diameter, density and haemorrhage of micrangium. Detect the level of total bilirubin (TBIL), and serum creatinine (Cr), blood urea nitrogen (BUN) changes in Serum, the level of ET, NO in plasma. At the same time, observe the pathologic changes of kidney.Results:1 The kidney observed in BDL group, it became obviously swelling, thickening and darkening. Above all decreased significantly in AM groups than in BDL group (P<0.05)2 Compare with SO group, in BDL group, the TBIL, Cr, BUN level of serum increased significantly (P<0.05), and as the time prolong, the level had a tendency to advance.3 Compare with SO group, in BDL group, the renal hemoperfusion,the diameter, density and haemorrhage of micrangium decreased significantly. the different is significant (P<0.05).4 Compare with SO group, in BDL group, the ET, NO level of serum increased significantly(P<0.05). and as the time prolong, the level had a tendency to advance.5 Comparison with BDL group, in AM group, the kidney displayed unobviously in size, lighter swelling and darking. The TBIL, Cr and BUN level decreased significantly in AM groups than in BDL group (P<0.05). The renal hemoperfusion,the diameter, density and haemorrhage of micrangium and the ET ,NO level decreased significantly in AM groups than in BDL group (P <0.05).Conclusion:1 Ligated the common bile duct can induce OJ model, this is a duplicated, valuable and reliable method.2 In OJ, there are obvious pathologic changes on kidney, also, the disturbance of renal function, and as the time prolong, the conditions become worse.3 In the early stage of OJ, there is a disturbance on microcirculation of kidney, and as the time prolong, the conditions become worse.4 In OJ, the level of the ET, NO increase, the dis- turbance of microcirculation of kidney become aggravated. It indicates that the ET, NO are the important factors of the disturbance of microcirculation.5 In OJ, the use of AM can decrease the levels of ET, NO in plasma, and then improve the renal microcirculation, reduce the changes of renal histopathology, finaly pretect renal injury.
