Research On The Implementation Effect And Improvement Strategy Of Evaluation Activities In Chinese Medicine Hospitals

Objectives:This paper,to discuss the main implementation effect of evaluation activities of two types of TCM hospitals,and taking the TCM hospital administration year in 2010 and the Inspection of maj or TCM hospitals in Beijing in 2015 as examples.Combining with the implementation status of existing evaluation activities of TCM hospital,and drawing on the advantages of the internationally representative evaluation system to provide suggestions for the evaluation of TCM hospital.Contents:1.The main implementation effect of evaluation activities both the TCM hospital Administration Year in 2010 and the Inspection of Major TCM hospitals in Beijing in 2015.2.on the basis of the implementation effect analysis,along with the results of expert interviews,we analyze the status of evaluation and summarize deficiencies in the evaluation of TCM hospital.3.Putting forward the proposal by learning from the advantages of international hospital evaluation systems.Methods:Difference-in-difference(DID)model and Interrupted Time Series(ITS)were performed to quantitatively analyze the impact of the TCM hospital administration year in 2010 and the Inspection of major TCM hospitals in Beijing in 2015.Literature research was used to summarize and identify the problems that existed in current evaluation of TCM hospital.Expert interview was organized among experts in the field of TCM hospital management,administration,and personnel engaged in TCM information,the aim of which is to provide suggestions on the evaluation of TCM hospital.Results:1.TCM hospital administration year in 2010 exerted significant positive impact on the proportion of TCM revenue in the 1st year after this evaluation.Utilization rate of hospital beds showed a trend of decline after this evaluation(?3<0,p<0.05).The impact on the number of discharges and the number of total visits showed an increase in the instantaneous level and a decrease in the trend level(p<0.05).As for the Inspection of major TCM hospitals in Beijing in 2015,among the first evaluated hospitals,outpatient expenditure per visit was affected significant instantaneously(p<0.05),but there was no significant change in the trend.Among the second evaluated hospitals,significant change in level and trend(P<0.05)occurred in TCM prescription proportion.For these two TCM hospital evaluation activities,the impact did not show on the average hospitalization expenses,outpatient TCM with non-drugs,and the proportion of TCM doctor practitioners(%),medical income(%),Chinese medicine income(%).The results revealed that the impact of TCM hospital evaluation activities on the operation of TCM hospitals is limited.2.The evaluation of the organization of TCM hospitals is the main factor which affects the evaluation results.The results of expert interviews show that the objectivity of evaluation results was influenced by the multivariate intersection and integration of evaluation and management for TCM hospitals.Besides,the form of payment by the evaluation applicant leads to a lack of impartiality in the evaluation results.Furthermore,the imperfection of the personnel evaluation system affected the homogeneity of the result,and the lack of methodological support in the evaluation process made it impossible to evaluate the truly level of hospital medical services and the operation of the system.Moreover,lack of strict follow-up application and supervision of the evaluation results lead to the lack of motivation for the follow-up rectification of hospitals.3.Comparing of the evaluation system of TCM hospital and the evaluation system of hospitals in the world,we find the practice of JC in the United States and JCT in Taiwan,a third party as the evaluation subject to participate in the evaluation of TCM hospitals is recommended.For the evaluation personnel system,the relevant system and training mechanism of the German KTQ is useful to attach importance to the construction of the hospital evaluation expert team,and guarantee the homogeneity of evaluation.Meanwhile,the evaluation process,should learn from the tracking method adopted by JC in the United States and JCI in many countries,which could ensure TCM hospitals are evaluated by the perspective of patients and hospital operating systems.In addition,the application of the evaluation results can refer to the experience of JC in the United States,KTQ in Germany and CQC in the United Kingdom to publicize the evaluation results timely,and link with medical insurance to strengthen the social supervision,promoting the continuous improvement of the hospital,and achieve the ultimate goal of hospital evaluation.Suggestions:There are four suggestions for the evaluation of TCM hospitals by comparing with the international hospital evaluation systems.First of all,A third parties should be introduced as the main part of evaluation to ensure the objectivity of the evaluation results.Secondly,the personnel system and the homogeneity of evaluation results of TCM hospitals evaluation needs to be improved.In addition,the evaluation of TCM hospitals should be based on the original data and evidence to improve the quality and efficiency of the review.Finally,the evaluation result of TCM hospitals should be published timely and be supervised by society to promote their sustainable development.
