Objective: To investigate the correlation between plasma endotoxin level and clinical indicators such as renal function,urinary routine,and TCM syndromes in patients with chronic kidney disease in stage 1-3 of spleen-kidney Qi deficiency syndrome,and to understand the correlation between plasma endotoxin level and disease condition in patients with chronic kidney disease.Methods: From June 2018 to January 2019,402 patients with chronic kidney diseases of spleen and kidney Qi deficiency syndrome in stage 1-3 met the inclusion and exclusion criteria in the Department of Nephrology,Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine.The levels of plasma endotoxin,urinary routine,24-hour urinary protein quantification,blood routine,renal function and blood lipid were detected,and the TCM syndromes of these patients were collected.To analyze the correlation between plasma endotoxin level and clinical indicators such as renal function,urinary routine and TCM syndromes in patients with chronic kidney disease.Results:(1)The plasma endotoxin level in patients with chronic kidney disease increased significantly,and with the gradual decline of renal function,the mean plasma endotoxin level increased gradually.(2)There was no correlation between plasma endotoxin level and age(P > 0.05).The plasma endotoxin level was positively correlated with the course of disease,and the correlation coefficient was 0.130(P < 0.05).The plasma endotoxin level was positively correlated with serum creatinine and urea nitrogen,with correlation coefficients of 0.35 and 0.119(P < 0.05),while the plasma endotoxin level was negatively correlated with glomerular filtration rate,with correlation coefficient of-0.195(P < 0.05).There was a positive correlation between plasma endotoxin level and urinary protein content,with a correlation coefficient of 0.108(P < 0.05).There was no correlation between plasma endotoxin level and urinary erythrocyte(urinary sediment).The plasma endotoxin level was positively correlated with hemoglobin,and the correlation coefficient was0.226(P < 0.05).There was no correlation between plasma endotoxin level and albumin(P > 0.05).There was a positive correlation between plasma endotoxin level and triglyceride,and the correlation coefficient was 0.285(P < 0.05).There was no correlation between plasma endotoxin level and total cholesterol,high density lipoprotein and low density lipoprotein(P > 0.05).(3)The level of plasma endotoxin was positively correlated with the total score of TCM symptoms,and the correlation coefficient was 0.443(P < 0.05);the level of plasma endotoxin was correlated with four symptoms: weak,low intake,abdominal distention,loose stool,and the correlation coefficients were 0.415(P < 0.05),0.460(P < 0.05),0.465(P < 0.05)and 0.223(P < 0.05).Among the four indicators of Water-dampness syndrome,dampness-heat syndrome,dampness-turbidity syndrome and blood stasis syndrome,the endotoxin level of patients with dampness-heat syndrome was the highest(P < 0.05).Conclusion: The plasma endotoxin level in patients with chronic kidney disease stage 1-3 of spleen-kidney Qi deficiency syndrome is elevated,and the plasma endotoxin level is related to the renal function,proteinuria,hemoglobin and lipid metabolism of patients with chronic kidney disease.The plasma endotoxin level is closely related to the symptoms of patients with chronic kidney disease,especially with four symptoms: weak,low intake,abdominal distention,loose stool,and It is closely related to dampness-heat syndrome.
