| Objective:To observe the effect of tonifying kidney and invigorating spleen prescription on the renal function of the CKD5 stage non dialysis patients of spleen-kidney qi deficiency style.Method:The 60 patients who met the inclusion criteria were randomly divided into 30 patients in the control group and 30 in the treatment group.The control group was treated with conventional basic treatment which was basic treatment of western medicine and Chinese medicine.The treatment group was treated by Kidney-nourishing prescription based on routine treatment.Intervention for 6 months,the changes of glomerular filtration rate(eGFR),serum creatinine(SCR),blood urea nitrogen(BUN),uric acid(UA),cystatin C(Cys-C)and TCM syndrome score were observed in prior treatment,three months after treatment and six months treatment.analyzing its the effect of syndrome and Comprehensive therapeutic effect,and evaluating its safety.Results:(1)The effect of traditional chinese medicine(TCM)syndrome integral:The same group compared before and after treatment:The TCM syndrome score of the treatment group was significantly lower than before,and the difference was statistically significant(P<0.05).the TCM syndrome score of the control group was slightly reduced,but there was no statistical significance(P>0.05).Comparision between the two groups after treatment:the TCM syndrome score of the treatment group was significantly lower than that in the control group,which was statistically significant(P<0.05),and the treatment was better than the control group.(2)The influence of primary symptom of TCM:Comparision between the two groups after treatment:The difference was statistically significant in the symptoms of languid,eating less,heaviness of limbs,and the treatment group was better than the control group(P<0.01).Tthere was a statistically significant difference between the two groups of patients in the symptoms of shortness of breath and laziness to speak,nausea,lumbar genu aching and limp,nausea and vomiting and lumbago,and the treatment group was better than the control group(P<0.05).There was no statistically significant difference between the two groups after treatment in the symptoms of gloomy complexion(P>0.05).(3)The effect onTCM syndrome effect:The total effective rate of TCM syndromes in the treatment group was 70%,and the total effective rate of TCM syndromes in the control group was 43.3%,and the difference between the two groups was statistically significant(P<0.05),and the treatment effect was better than the control group.(4)The effect on comprehensive curative effect:After 6 months of treatment,the stability rate of the treatment group was 70%,the total effective rate was 16.7%,and the control rate was 67%,the total effective rate was 6.7%,and the difference between the two groups was statistically significant(P<0.05),and the treatment group was superior to the control group.(5)Effects on the renal function:Through the detection of renal function in prior treatment,three months after treatment and six months treatment.the eGFR of treatment group showed going up in turn,and the SCR,BUN,UA,Cys-C showed going up in turn respectively,the control group eGFR showed an decreasing trend in trun,and the SCR,BUN,UA,Cys-C showed an increasing trend in turn respectively;there was no significant difference between the eGFR,SCR,BUN,UA,Cys-C of the treatment group and the control group before and after treatment(P>0.05);Comparision between the two groups after 6 months treatment:eGFR:the average values in the treatment group and the control group were 10.85±2.7,9.3±2.42 respectively,comparison between the two groups,and the difference was statistically significant(P<0.05).SCR:the average values in the treatment group and the control group were 433.1±73.43 and 491.1±97.74 respectively,comparison between the two groups,and the difference was statistically significant(P<0.05);BUN:the average values in the treatment group and the control group were 18.38±2.14 and 19.5±1.96 respectively,comparison between the two groups,and the difference was statistically significant(P<0.05);UA:the average values in the treatment group and the control group were 431.83±33.23 and 455.33±40,83 respectively,comparison between the two groups,and the difference was statistically significant(P<0.05);Cys-C:the average values in the treatment group and the control group were 4.64±0.4 and 4.89±0.51 respectively,comparison betweenthe two groups,and the difference was statistically significant(P<0.05);and the treatment group was superior to the control group resultst of between all of the above groups.(6)Entering into the number of hemodialysis:after 6 months of treatment,one patient in the treatment group was into dialysis,and 3 patients in the control group were into dialysis.(7)Evaluating the safety:during the treatment,in the treatment group,there were a few patients who took the drug appearing completely mashed feces which had many times.Through the dialectic treatment of Chinese medicine.the symptoms could be alleviated.Three patients had high potassium in the treatment group,and the diuretic could be reduced to normal by diuretic therapy.There were no abnormal changes in the electrocardiogram,blood routine and liver function of the in each group.Conclusion:(1)The renal function of patients who were not dialysis of the CKD5 stage of Spleen-kidney qi deficiency type with tonifying kidney and invigorating spleen prescription is relatively stable,to delay the time of dialysis.(2)The tonifying kidney and invigorating spleen could improve the TCM sym-ptoms and the quality of life of the CKD5 stage non dialysis patients of spleen-kidney qi deficiency style. |
