Objective To evaluate the effect of nursing intervention of TCM fumigation combined with auricular point sticking pressure on pruritus symptoms,comfort level,life quality and nursing satisfaction degree of patients with diabetic pruritus.Method A randomized controlled research was used in this study.187 inpatients were divided into three age groups,each age group was divided into three intervention group in the department by stratified randomization,the research was carried out in endocrinology of two hospitals of traditional Chinese medicine in Shanghai from April 2017 to January2019.A total of 60 cases were included in the Chinese medicine fumigation group,64 cases in the auricular point pressing group,and 63 cases in the Chinese medicine fumigation combined with auricular point pressing group.The control group was given conventional nursing and fumigation with traditional Chinese medicine,the auricular pressure group was given conventional nursing and auricular pressure,and the traditional Chinese medicine fumigation combined with auricular pressure group was given conventional nursing,fumigation with traditional Chinese medicine and auricular pressure.To evaluate the changes of Modified Duo's VAG Scale,Dermatology life quality index,General comfort questionnaire,TCM nursing intervention compliance and Newcastle satisfaction with nursing scales after intervention in three groups of patients.The lost cases were analyzed based on intentional analysis.The main outcome indexes were analyzed through the Full analysis set(FAS),the Modified Duo's VAG Scale(Duo's VAG),and the secondary outcome indexes were analyzed by the Per Protocol set(PPS).And the evaluation forms include Dermatology life quality index(DLQI),Kolcaba General comfort questionnaire(GCQ),Newcastle satisfaction with nursing scales(NSNS)and TCM nursing intervention compliance and satisfaction evaluation.SPSS21.0 software was used for analysis,chi-square test was used for enumeration data,paired t-test or non-parametric test was used for comparison withinthe measurement data set(Wilcoxon bilateral test),multivariate analysis of variance was used for comparison between groups,and LSD and SNK methods were used for multiple comparison.Results1.Balanced line test analyzed in patients of all three groups There was no statistically significant difference in comparison of sex,occupation,educational level,age,height,weight,BMI,medical history,history of allergies,skin itching,season,diabetes duration,itchy skin course,Hb A1 c,fasting blood glucose,and2-hour postprandial plasma glucose among three groups of patients(P>0.05).2.Main outcome indicator After the intervention,the difference of total score of Duo's VAG was statistically significant among the three groups(P<0.05).Fumigation of traditional Chinese medicine combined with auricular point sticking pressure can improve the pruritus symptoms of diabetic pruritus patients of different ages.There was no statistically significant difference in the score of Duo's VAG score among the three age groups(P>0.05).There was no correlation between groups and age(P>0.05).The results of multiple comparisons showed that the effect of Chinese medicine fumigation combined with auricular pressure group was better than that of Chinese medicine fumigation group and auricular pressure group,and Chinese medicine fumigation group was better than auricular pressure group.In terms of improving the interference of pruritus on sleep quality,the effect of Chinese medicine fumigation combined with auricular point pressing is better in the age group of 71-80 years.3.Secondary outcome indicators(1)DLQI: there were statistically significant differences in DLQI scores among the three groups of patients of all ages(P<0.05).Multiple comparison results showed that the effects of fumigation of traditional Chinese medicine and traditional Chinese medicine fumigation combined with auricular point sticking pressure were better than auricular point sticking pressure in improving the life quality of patients with diabetic pruritus.(2)GCQ: there were statistically significant differences in comfort level among the three groups of patients aged 50-60 years and 71-80 years after the intervention(P<0.05).The results of multiple comparisons showed that the effect of traditional Chinese medicine fumigation combined with auricular sticking pressure on comfort feeling ofpatients with diabetic pruritus was better than patients using TCM fumigation and auricular sticking pressure.(3)Compliance and satisfaction of TCM nursing techniques: there were no statistically significant differences in compliance and satisfaction between the three groups(P>0.05).The compliance rate of the three groups was higher than that of traditional Chinese medicine fumigation combined with auricular pressure.The satisfaction rate of patients in the three groups: TCM fumigation combined with auricular pressure was higher than TCM fumigation combined with auricular pressure.(4)NSNS: there was no statistical difference in nursing satisfaction degree among the three groups of patients of all ages after intervention(P>0.05),and the two combined nursing intervention had no effect on the overall nursing satisfaction level.Conclusion1.TCM fumigation combined with auricular point pressing nursing intervention can improve the pruritus symptoms of diabetic patients with pruritus,improve the pruritus degree,site,frequency and influence of sleep,improve the life quality of patients,and comfort level was improved more.2.This nursing method is convenient,safe and effective.No adverse effects occurred during the intervention.The degree of satisfaction is high.Therefore,the nursing method is worthy of further clinical application and promotion.
