Objective:By observing the clinical effect of Yang Xue Rou Gan Acupuncture on spastic paralysis of upper extremity after ischemic stroke,to explore the effectiveness and mechanism of Yang Xue Rou Gan Acupuncture for the treatment of spastic paralysis after stroke,so as to provide new ideas and methods for the clinical treatment of spastic paralysis.Methods:A total of 60 patients with upper limb spastic paralysis after ischemic stroke who met the inclusion criteria were collected,and the patients were randomly divided into the observation group and the control group by the method of generating random Numbers,with 30 patients in each group.Under the premise of basic treatment,the control group was treated with ordinary acupuncture,while the observation group was treated with Yang Xue Rou Gan Acupuncture on the basis of the control group.Both the observation group and the control group received treatment once a day,6 times a week,for a total of 6 weeks.The Modified Ashworth Scale(MAS),upper extremity Fugl-Meyer Assessment(FMA-UE)and Barthel Index(BI)were used to evaluate the patients' changes in muscular tension,improvement of upper limb motor function and improvement of daily living ability at three times: before treatment,3 weeks after treatment and 6 weeks after treatment.The score results of the two groups before and after treatment were statistically analyzed with SPSS 26.0.Result: 1 General data comparative analysis: the baseline data of two groups of patients(such as gender,age,course of the disease)statistical analysis,there were no statistically significant difference(P > 0.05),comparable.The MAS grade,FMA-UE score and BI score of upper limbs of the two groups before treatment were statistically analyzed,and the difference was not significant(P > 0.05),comparable.2 Analysis of total efficacy: In the observation group,7 cases were cured,12 cases were effective,7 cases were effective and 4 cases were ineffective,the total effective rate of observation group was 86.67%.In the control group,2 cases were cured,7 cases were effective,13 cases were effective and 8 cases were ineffective,the total effective rate of control group was 73.33%.Through statistical analysis,the difference in efficacy between the two groups was statistically significant(P < 0.05),indicating that the total efficacy of the observation group was better than that of the control group.3 Comparison of MAS grading changes before and after treatment between the two groups: after 3 weeks of treatment,there was no significant difference between the two groups(P > 0.05);after 6 weeks of treatment,the difference between the two groups was statistically significant(P < 0.05),This indicated that the improvement of upper limb muscle tension in the observation group was better than that in the control group.4 Comparison of FMA-UE scores before and after treatment in the two groups: after 3 weeks of treatment,the FMA-UE scores of the observation group were compared before and after treatment,and the difference was significant(P < 0.05),there was no significant difference in FMA-UE score of the control group before and after treatment(P > 0.05);After 6 weeks of treatment,the FMA-UE scores of the two groups were compared before and after treatment,and the difference was very significant(P < 0.01),It is suggested that the FMA-UE scores of the two groups are significantly improved after 6 weeks of treatment,and the observation group has taken effect after 3 weeks of treatment,suggesting that the observation group takes effect earlier than the control group;There was no significant difference in FMA-UE scores between the two groups(P > 0.05)after 3 weeks of treatment.There was significant difference in FMA-UE scores between the two groups after 6 weeks of treatment(P < 0.01).It can be seen from the above that after 6 weeks of treatment,the fma-ue scores of the two groups were improved compared with those before treatment,and the improvement of the observation group was more obvious,and the effect time of the observation group was earlier than that of the control group.5 Comparison of BI scores before and after treatment in the two groups:After 3 weeks of treatment,the BI score of the observation group was compared before and after treatment,and the difference was significant(P < 0.01),and there was no significant difference in BI score before and after treatment in the control group(P > 0.05);After 6 weeks of treatment,the BI scores of the two groups were compared before and after treatment,and the difference was very significant(P < 0.01),It is suggested that BI scores of the two groups are significantly improved after 6 weeks of treatment,and the observation group has taken effect after 3 weeks of treatment,suggesting that the observation group takes effect earlier than the control group;There was no significant difference in BI score between the two groups after treatment for 3 weeks(P > 0.05),there was significant difference in BI score between the two groups after treatment for 6 weeks(P < 0.01).It can be seen from the above that after 6 weeks of treatment,BI scores of the two groups were improved compared with those before treatment,and the improvement of the observation group was more obvious,and the effect time of the observation group was earlier than that of the control group.Conclusion:1 Yang Xue Rou Gan Acupuncture combined with conventional acupuncture can effectively treat spastic paralysis of upper limb after ischemic stroke,and its curative effect is better than that of conventional acupuncture group.It is worthy of clinical promotion.2 Yang Xue Rou Gan Acupuncture combined with conventional acupuncture can effectively reduce the muscle tension of patients with spastic paralysis of upper limbs after ischemic stroke.It can improve the upper limb movement function and the ability of daily activities,and the curative effect is better than the conventional acupuncture method in improving upper limb motor function and daily living ability.And the effective time is earlier than the ordinary acupuncture method.3 It is verified that the lesion of spastic paralysis of upper limbs after stroke is located in the tendons,involving the liver and spleen.The pathogenesis is that blood deficiency cannot moisten and nourish the tendons and veins.The treatment method is to start with the liver and spleen,invigorate the spleen and nourish blood,soften the liver and relax the tendons.
