ObjectiveBy analyzing the correlation between blood routine and the severity,activity and TCM syndromes of children with psoriasis vulgoriasis,this study aims to find a more reliable and easy to detect index for the assessment of psoriasis in clinic and to provide possible objective markers for TCM syndrome differentiation of psoriasis.At the same time,the main indexes of liver and kidney function of 20 children with mild psoriasis treated with oral Chinese traditional medicine were compared with those of 27 healthy children,so as to evaluate the safety of Chinese traditional medicine compound in treating psoriasis,so as to provide reference for clinical use.Methods1.Choose from March 2019 to January 2020 in our hospital skin pediatric clinic on 76 cases of patients with psoriasis vulgaris as a group,with the same period to our pediatric outpatient service of 55 cases of skin healthy children as control group,comparing the difference between the part of the routine blood indexes between the two groups: neutrophils absolute value,lymphocyte absolute value,number of platelets,red blood cell distribution width of SD,mean platelet volume,neutrophils and lymphocytes ratio(NLR),platelet and lymphocyte ratio(PLR).2.According to PASI score,76 children with psoriasis vulgaris were divided into two groups: mild group(PASI < 10)moderate and severe group(PASI?10);According to the clinical staging standard of psoriasis,the patients were divided into 3 groups: progressive group,stationary group and regressive group.According to TCM syndrome types,the patients were divided into blood heat syndrome group,blood dryness syndrome group and blood stasis syndrome group.The differences in neutrophil absolute value,lymphocyte absolute value,platelet number,erythrocyte distribution width,SD,mean platelet volume,neutrophil to lymphocyte ratio(NLR),and platelet to lymphocyte ratio(PLR)were compared among groups.3.The correlation between PASI score and neutrophil absolute value,lymphocyteabsolute value,platelet number,erythrocyte distribution width,SD,mean platelet volume,neutrophil to lymphocyte ratio(NLR),platelet to lymphocyte ratio(PLR)was analyzed.4.In 76 cases of patients with psoriasis vulgaris,select the system treatment with oral Chinese medicine preparation plan only 20 cases of mild patients with psoriasis vulgaris as observation group and control group of 27 cases of healthy children,comparison between the two groups of some major differences between the liver and kidney function index: Alt,aspertate aminotransferase,blood urea nitrogen,creatinine,uric acid.Results1.Comparison between the sick group and the healthy control group: compared with the healthy control group,NEU,LYM was significantly increased,while RDW was significantly lower(P < 0.05).2.Comparison between the disease group and the healthy control group by gender:(1)Compared with the male healthy control group,the RDW of male children with psoriasis vulgaris was significantly lower(P < 0.05).There was no significant difference in NEU,LYM,NLR,MPV,PLR and PLT values between the two groups(P>0.05).(2)Compared with the healthy control group,NEU and LYM in female children with psoriasis vulgaris were significantly increased,while RDW was significantly lower(P < 0.05).There was no statistically significant difference between NLR,MPV,PLR,PLT and the healthy control group(P>0.05).3.Comparison between mild psoriasis group(PASI < 10)and moderate to severe psoriasis group(PASI?10): medium weight NEU,NLR and PLT of the psoriasis vulgaris group were higher than that of the psoriasis vulgaris group(P < 0.05).There was no significant difference in RDW,MPV,PLR and LYM between the two groups(P>0.05).4.Comparison between the three groups of march,rest and retrograde: LYM,RDW,PLR and MPV of children with psoriasis in the progression,rest and retrograde had no statistical significance(P>0.05).Compared with the progressive stage,NEU and NLR of children with psoriasis in the stationary stage and the retrograde stage were significantly reduced(P< 0.05).There was no statistically significant difference in NEU and NLR between children with psoriasis at rest and at regression(P>0.05).In addition,the PLT of children in the progression stage was significantly higher than that in the rest stage(P < 0.05).There was no significantdifference in PLT between the progressive stage and the retrograde stage,and between the stationary stage and the retrograde stage(P>0.05).5.There were no statistically significant differences in LYM,RDW,PLR and MPV among children with psoriasis with blood heat syndrome,blood dryness syndrome and blood stasis syndrome among the three groups(P>0.05).Compared with the blood dryness syndrome,the NEU of children with blood heat syndrome and blood stasis syndrome was significantly increased(P<0.05).There was no statistically significant difference in NEU between children with blood heat syndrome and children with blood stasis syndrome(P>0.05).In addition,the PLT and NLR of children with blood fever syndrome were significantly higher than that of children with blood fever syndrome(P < 0.05).There was no statistically significant difference between PLT and NLR in blood heat syndrome and blood stasis syndrome,blood dryness syndrome and blood stasis syndrome(P>0.05).6.Correlation analysis of PASI score and blood routine parameters:(1)overall analysis: PASI score in children with psoriasis vulgaris was positively correlated with NEU(r=0.749,P < 0.05).PASI score was positively correlated with NLR(r=0.856,P < 0.05).PASI score was positively correlated with PLT(r=0.314,P < 0.05).PASI score was positively correlated with PLR(r=0.234,P < 0.05).There was no significant correlation between PASI score and other indicators.(2)in the male group,PASI score of male children with psoriasis vulgaris was positively correlated with NEU(r=0.755,P< 0.05).PASI score was positively correlated with NLR(r=0.861,P< 0.05).PASI score was positively correlated with PLT(r=0.375,P< 0.05).There was no significant correlation between PASI score and other indicators.(3)in the female group,PASI score of female children with psoriasis vulgaris was positively correlated with NEU(r=0.742,P<0.05).PASI score was positively correlated with NLR(r=0.839,P<0.05).There was no significant correlation between PASI score and other indicators.7.Preliminary safety evaluation of oral Chinese medicine compound: AST and BUN in the observation group were significantly lower than those in the healthy control group(P<0.05).There was no statistically significant difference between ALT,UA,Cr and the healthy control group(P>0.05).Conclusion1.NLR?NEU may be used as one of the indicators to judge the severity of children's psoriasis vulgaris,but it cannot be used as a microscopic syndrome differentiation index between different TCM syndromes of children's psoriasis vulgaris.2.Other indicators(LYM?RDW?PLT?PLR?MPV)involved in this study cannot be used as indicators to judge the severity of children's psoriasis vulgaris due to the lack of statistical significance of the analysis results or the large difference between the results of domestic and foreign studies,nor can they be used as microscopic indicators of syndrome differentiation between different TCM syndromes of children's psoriasis vulgaris.3.The oral Chinese medicine compound preparation has a high safety in the treatment of psoriasis within the period of 1 month.
